Career patterns of dental hygienists in Scotland.

OBJECTIVE: To study career patterns of dental hygienists in Scotland. SUBJECTS AND METHODS: Questionnaires were sent to all the dental hygienists enrolled with the General Dental Council in 1998 who had addresses in Scotland and those who were alumni of Scottish School of Dental Hygiene within the past 10 years. RESULTS: There was a 58.7% response (334 out of 569). A net loss of trained hygienists was not found. The results suggested that most would remain in employment for at least 30 years and about a third would retire early. Half were in full-time employment and 60% had been in continuous employment. Most of the remainder had personal reasons for ceasing work. 39% had refused employment because of the poor level of pay. CONCLUSIONS: The introduction of the 2-year course has reduced output from schools of dental hygiene and demand for hygienists is high. Little hard information exists to support manpower planning in this area. This study suggests that there is little wastage of trained dental hygienists in Scotland. More data is required concerning employment opportunities and potential unmet demand for this group of PCDs to establish the number of training places required.

The new MFDS/MFD examination.

The first diet of the Membership of the Faculty of Dental Surgery/Faculty of Dentistry (MFDS/MFD) examination took place on 15 September 1998. This article takes a look at the reasons behind the establishment of this new examination and describes the methods to be used in candidate assessment.

Microleakage of retrograde root fillings: assessment using a novel measurement system.

In the past, measurement of microleakage has been based on measurement of dye or radioactive isotope penetration. A method has been developed to allow electronic monitoring of microflow in the same specimen over a period of time. This method was used to compare previously reported retrograde materials with an amalgam/sealer combination. The material that performed best was amalgam/sealer, particularly in the first week. The role of cavity varnish in limiting leakage has been controversial, and the results in the present study do not indicate any significant benefit. The range of leakage measured for specimens in the same group suggests that in vivo the expected leakage could be considerable for the materials tested and that small differences in experimental leakage between groups may not have any practical significance.

A survey of patients seen in consultant clinics in conservative dentistry at Edinburgh Dental Hospital in 1990.

A baseline study of referrals by letter to a department of conservative dentistry staffed purely by honorary consultants was conducted for one year. Distinct differences were found between the majority of patients referred from general practice and those referred from other sources. Principal reasons for referral were problems of tooth replacement, appearance, endodontics and tooth wear. A predominance of female patients was seen in all groups except those complaining of tooth wear. 'Endodontic' patients featured the highest proportion of requests for hospital treatment. Complex restorative treatment was provided for the majority (70%) of referred patients, and about 10% received treatment from dental students. The rates of referral showed evidence of dependence on travelling distance to the referral centre but not on the availability of local GDPs. This emphasises the importance of maintaining existing specialist dental referral services in major population centres and the development of new services where appropriate.

[Electronic apex localizers: useful instruments or inconvenient gadgets?].

Apex locators offer definite advantages for certain patients over conventional radiographic methods of root canal length measurement. Current instruments do not appear to differ significantly in accuracy. The choice of locator has to be made therefore on cost and convenience in use. The current best buys seem to be the Evident RCM and NeoSono MC. Both offer good value for money, the Evident is the less expensive but lacks some of the additional features of the NeoSono. The decision to buy an apex locator should only be made after a period of testing in clinical practice.

Tissue response to retrograde root fillings in the ferret canine: a comparison of a glass ionomer cement and gutta-percha with sealer.

Tissue healing was compared after apicectomy and filling of ferret lower canines with glass ionomer (Ketacfil) or gutta-percha/sealer (Tubliseal). Both materials provoked an inflammatory response after 7 days, but the response to glass ionomer was less severe. The response after 28 days was different. Mild inflammation related to the gutta-percha was still present, but no inflammation was found in relation to the glass ionomer. Bone infill was quite advanced in all specimens at 28 days but was more complete in the glass ionomer specimens, with intimate contact between the bone and cement.

A suggested role for polymorphonuclear leukocytes in the perpetuation of periapical disease.

During the course of an investigation of endodontic materials using subcutaneously implanted human roots in rats and guinea pigs, it was noted that in areas of the root canal which had not been reamed polymorphonuclear leukocytes had migrated into the dentinal tubules. It is suggested that irritation of the periapical tissues may be mediated by these polymorphonuclear leukocytes. Therefore, the occlusion of dentinal tubules which may be exposed to periapical exudate may be of prime importance in endodontic treatment.

Bone development following transplants of urinary bladder wall: a quantitative histological and ultrastructural study.

Bone formation occurred ten days after transplantation of guinea-pig urinary bladder wall to the anterior abdominal wall. A quantitative analysis showed that the bone which formed in the tissues of the transplant site grew rapidly during the following week. Thereafter, bone growth slowed and remodelling became evident histologically. The bone continued to enlarge for up to six months but growth was mainly confined to an increase in thickness. Matrix vesicles were observed in the early bone formation, but later, when bone growth slowed, these structures could not be observed on the bone surfaces.
