Osteosynthesis of femoral neck fracture. The sliding-screw-plate with or without compression.

In a prospective study of 220 displaced femoral neck fractures treated with a sliding-screw-plate, the cases were allocated to osteosynthesis with or without compression. In surviving patients followed for at least 1 year, union occurred in 57/85 with compression and 58/71 without compression. At 2 years, necrosis was radiographically evident in 9/37 and 7/36 united fractures. Thus, compression cannot be recommended when displaced femoral neck fractures are treated with a sliding-screw-plate.

The effect of coronary artery bypass surgery on ventricular arrhythmias.

The effect of coronary artery bypass surgery (CAB) on ventricular arrhythmias (VA) was studied in a prospective investigation involving 32 patients (mean age 54 years) who underwent CAB because of severe stable angina pectoris. Prior to CAB as well as 12 months later each patient was subjected to the following investigational programme: resting ECG, exercise ECG, 24-h ECG, selective coronary arteriography, ventriculography and cardiac catheterization. Exercise ECG showed VA in only three patients. The prevalence of VA during 24-h ECG was 56 and 66% on the two occasions (NS), while complicated VA (multiform, repetitive, R on T) was seen in 18 and 28%, respectively (NS). The persistence (number of 6-h periods showing VA) was 33 and 47% with regard to any VA (P less than 0.05), while complicated VA occurred in 13 and 15% of the 6-h periods (NS). Except for an increase in dp/dtmax/P at the postoperative measurement (P less than 0.05), no significant change in the performance of the left ventricle was seen after CAB though the graft patency was 77%. It is concluded that in patients with 'medically intractable' stable angina pectoris, CAB does not effect the occurrence of VA to any great extent--probably because left ventricular function is unchanged one year after as compared with that prior to CAB.

Comparison of computed tomography and aortography in abdominal aortic aneurysms.

In a prospectively designed study of 13 consecutive patients with proven abdominal aortic aneurysm, the diagnostic accuracy of computed tomography (CT) was compared with aortography. Computed tomography was found to be a highly informative diagnostic test providing clear delineation of lesions and extent of aneurysm and its relationship to renal and iliac arteries. Furthermore one case of retroperitoneal fibrosis and one case of hydronephrosis were demonstrated by CT. The diagnosis of aneurysm by aortography was uncertain in six cases and in another three cases the relation of the aneurysm to the renal arteries was uncertain. Thus CT was found superior to aortography in examining patients for abdominal aortic aneurysm.

The chest radiograph in chronic obstructive lung disease compared with measurements of single-breath nitrogen washout and spirometry.

Postero-anterior and lateral chest radiographs were taken in 56 patients with chronic obstructive lung disease in whom the forced expiratory volume (FEV1), total lung capacity (TLC), the residual volume (RV) and the slope of phase III (S3) on the single breath nitrogen washout test (SBN2) were known. The shape and the level of the diaphragm and the shape of the costophrenic sulci, were determined from the chest films and were found to correlate with S3 and FEV1 but not TLC or RV/TLC%.

Pulmonary airflow obstruction following lymphography.

In a series of 27 patients a significant mean decrease of forced expiratory volume in one second (FEV1) was recorded at 1/2, 2, 4 and 24 hours after lymphography with an oily contrast medium, with a maximum mean decrease at 4 hours after injection. The decrease of FEV1 was greater in patients given large amounts of contrast medium at high injection speed. Furthermore, an excessive decrease was observed in patients with metastatic lymph nodes, while a less marked decrease of FEV1 was observed in patients treated with aspirin. No serious clinical side effects were observed. It is recommended that the injection speed and the total amount of contrast medium be limited as far as possible, especially in patients with severe airflow obstruction or with metastases in the lymphatic system. Oral administration of aspirin may limit the decrease in FEV1 following lymphography.

Dose reduction in radiography of the spine in scoliosis.

Measurements of skin doses during radiography in scoliosis have been performed with a.p. and p.a. direction of the beam. In order to keep the doses to the patients low, high kV, a film-focus distance of at least 3 m, a fast screen-film combination, lead shielding, a collimator with rotating compensating filter, and a grid of the moving anti-scatter type are recommended. By using the p.a. direction the dose to the breast is reduced by a factor in the range of 88 to 97 per cent. Further dose reduction might be achieved by removal of the anti-scatter grid and use of the air gap technique. Relinquishment of the radiographic principle of bringing the object to be examined as near the film as possible did not reduce the diagnostic information.

[Sirenomelia--the caudal regression syndrome (author's transl)].

A case of sirenomelia with malformations in lumbar and sacral spine and urogenital system together with fusion of lower limbs is presented. The caryotype of the child was normal 46XY. There were no hereditary diseases or diabetes in the family, though caudal regression syndrome is known to be more frequent with diabetic mothers. No exposures were known.

Treatment of displaced fractures of the femoral neck. Smith-Petersen osteosynthesis versus sliding-nail-plate osteosynthesis.

In a prospectively planned follow-up study 383 cases of displaced medial fractures of the femoral neck were allocated either to Smith-Petersen osteosynthesis or to sliding-nail-plate osteosynthesis. Two hundred and forty-nine cases were followed for more than 2 years. The two differently treated groups were alike. The results showed that sliding-nail-plate osteosynthesis was significantly superior to Smith-Petersen osteosynthesis. We recommend that Smith-Petersen osteosynthesis be abandoned.
