Dynamic tracheal collapse associated with disseminated histiocytic sarcoma in a cat.

This case report describes an unusual presentation of histiocytic sarcoma in a domestic shorthair cat. Initial investigation revealed a haemodynamically insignificant hypertrophic cardiomyopathy, bronchitis and a mild irregularity of the cervical trachea. The cat's disease progressed over a two-week period. Repeat radiography and tracheoscopy revealed a marked dynamic tracheal collapse associated with a raised plaque-like lesion within the cervical trachea. Subsequent post-mortem examination and histopathology revealed disseminated histiocytic sarcoma involving the trachea and kidneys. This is the first reported case of a histiocytic sarcoma involving the trachea in either dogs or cats.

Longitudinal glaucoma screening for siblings of patients with primary open angle glaucoma: the Nottingham Family Glaucoma Screening Study.

AIMS: To assess the prevalence and cumulative incidence of open angle glaucoma (OAG) in a cohort group of siblings of OAG probands. METHODS: Between 1994 and 2003, a group of siblings of OAG probands underwent both initial and follow up standardised ophthalmic examinations. Siblings were classified as "definite glaucoma" (primary OAG (POAG) and normal tension glaucoma (NTG)), "glaucoma suspects" (NTG suspects or ocular hypertension (OHT)), and normal. The prevalence and cumulative incidence of OAG over the follow up interval were calculated. RESULTS: At the initial study, 271 siblings (mean age 63.6 years; female to male ratio 1.2) from 156 probands were examined. 32 (11.8%) were classified as definite glaucoma and 15 (5.5%) as suspects. In the follow up study, 157 of the 224 "normal" siblings from the initial study were examined (mean interval from initial study 7.0 (SD 1.0) years). 11 (7%) were classified as definite glaucoma and 30 (19.1%) as suspects. There were significant trends of increasing prevalence and incidence of OAG with age and a lifetime risk estimated at approximately 20% by age 70. CONCLUSION: Siblings of glaucoma patients have an increased risk of developing glaucoma and the risk increases with age. An effective and repeated screening programme should be considered for this high risk group.

Changes in the Doppler waveform of the ovine femoral artery following infusion of vasoactive agents: a preliminary study.

The aim of this study was to investigate whether femoral artery pulsatility index (PI) can be used as an indicator of vasomotor tone in the digit of an ungulate species by measuring the change in PI induced by infusion of vasoactive agents in halothane-anaesthetised sheep. Intra-arterial infusion of the vasoconstrictor phenylephrine (9 microg min(-1)) tended to increase waveform PI (3.89-6.24, n=4, P=0.100). Infusion of a low dose (3 microg min(-1)) of the vasodilator sodium nitroprusside did not alter femoral artery PI, however infusion of a higher dose (30 microg min(-1)) tended to increase PI (5.39-6.70, n=5, P=0.059). During these studies heart rate, mean ABP and p(a)CO(2) did not change significantly. The tendency for PI to increase in response to vasodilation was unexpected and the mechanism involved is unclear. It appears that femoral artery PI cannot be used to predict vasomotor tone in the digit of the anaesthetised sheep.

Cellophane banding of congenital intrahepatic portosystemic shunts in two Irish wolfhounds.

Two three-month-old, male Irish wolfhound siblings were diagnosed with breed-typical left divisional congenital intrahepatic portosystemic shunts consistent with patent ductus venosus. The shunts were amenable to surgical dissection at a posthepatic location. Both dogs had cellophane banding for shunt attenuation. One dog was euthanased after developing post-ligation neurological dysfunction, which was refractory to treatment. The other dog survived and demonstrated shunt attenuation. Successful surgical management using cellophane banding of a patent ductus venosus has not been previously described in a large-breed dog.

Development of a behaviour-based scale to measure acute pain in dogs.

A composite scale for assessing pain in dogs in a hospital setting has been developed on the basis of observations of their behaviour. Initially, 279 words and expressions suggested by 69 veterinary surgeons were reduced into 47 words and expressions which were allocated into seven behaviour categories: demeanour and response to people, posture, mobility, activity, response to touch, attention to painful area and vocalisation. Three statistical methods, hierarchical agglomerative cluster analysis, Cronbach's alpha coefficient, and analysis of variance with multiple comparisons and empirical cumulative distributions, were used to validate these procedures, and a questionnaire accompanied by a list of definitions was designed around the expressions. The new composite scale is more detailed than previously reported scales for assessing pain in dogs on the basis of their behaviour, and the methods used in its development are based on sound scientific principles.

The role of nitric oxide in the responses of the ovine digital artery to vasoactive agents and modification of these responses by endotoxin and cytokines.

1. Laminitis, an important cause of lameness in domestic ungulates, occurs as a result of altered digital perfusion. Endotoxin and cytokines may mediate the vascular derangements observed through alterations in nitric oxide production. In this study, the vascular responses of the isolated ovine digital artery were examined and the influence of endotoxin and cytokines investigated. 2. Neither removal of the endothelium nor incubation with N(G)-nitro-L-arginine methyl ester (L-NAME, 300 microM) altered the response to phenylephrine (PE, 1 nM to 300 microM). Indomethacin (10 microM) decreased PE log EC(50) from -6.22+/-0.08 to -6.55+/-0.07. Acetylcholine (1 nM to 1 mM) and bradykinin (BK, 100 pM to 3 microM) induced endothelium-dependent relaxation. Bradykinin-induced relaxation was reduced by L-NAME, E(max) falling from -61.7+/-7.4 to -34.0+/-2.1%. Addition of indomethacin further reduced BK E(max) to -9.6+/-2.8%. Sodium nitroprusside (1 nM to 300 microM) produced endothelium-independent relaxation that was unaffected by L-NAME or indomethacin. 3. Following a 6 h incubation with endotoxin (3 microml(-1)), arterial responses to PE and BK did not differ from polymyxin B-treated controls (10 microg ml(-1)). Arteries incubated for 6 h with interferon-gamma (IFN-gamma, 10 ng ml(-1)) and tumour necrosis factor-alpha (TNF-alpha, 5 ng ml(-1)) exhibited greater relaxation to BK (E(max)-50.0+/-5.1%) than polymyxin B-treated controls (E(max)-33.1+/-4.0%), but did not differ in their response to PE. 4. Prolonged incubation (16 h) with endotoxin (3 microg ml(-1)) did not alter the response to PE, however incubation with IFN-gamma (10 ng ml(-1)), TNF-alpha (5 ng ml(-1)) and interleukin-1beta (20 ng ml(-1)) for 16 h increased PE log EC(50) from -6.44+/-0.09 to -6. 10+/-0.11. 5. Nitric oxide is an important mediator of endothelium-dependent relaxation in ovine digital arteries but does not modulate PE-induced vasoconstriction. Incubation with cytokines decreased the sensitivity of digital arteries to PE.

Patients with ocular hypertension have abnormal point scotopic thresholds in the superior hemifield.

PURPOSE: To compare light- and fully dark-adapted thresholds at loci within the central visual field in patients with ocular hypertension and glaucoma. METHODS: Eighteen patients with chronic open angle glaucoma, 13 patients with ocular hypertension, and 24 age-matched normals were studied. The Humphrey automated perimeter with the standard background illumination of 31.5 apostilbs was used to determine photopic thresholds at 18 loci within 20 degrees of fixation. Fully dark-adapted thresholds were measured at the same loci after 30 minutes of dark adaptation by automatic, static campimetry. RESULTS: The glaucoma group showed elevated scotopic thresholds. Scotopic defects also were found in a significantly higher proportion of patients with ocular hypertension than in normals. These scotopic defects were predominantly in the superior hemifield. CONCLUSIONS: Scotopic threshold campimetry may identify the subgroup of patients with ocular hypertension who progress to develop glaucomatous field loss identifiable by standard photopic and mesopic perimetry.

Vertical optic disk diameter: discrepancy between planimetric and SLO measurements.

PURPOSE: To compare measurements of the vertical diameter of the optic disk using the Heidelberg Retinal Tomograph (HRT) and planimetric methods. METHODS: On the HRT, three measurements of the vertical disk diameter were made on 32 eyes from a composite image using the circle draw facility and were compared with three measurements of the central image of the 32 tomographic cuts. These were repeated by a second observer. Measurements were compared with those obtained from photographs using the three corrections described by Bengtsson and Krakau. RESULTS: For the HRT, there was minimal intraobserver variation, coefficient of variation (1.515% to 1.882% for the two observers). Interobserver variation was also small (CoV 1.969%). The authors found the closest agreement with the HRT measurements using regression analysis was obtained using correction 3 (r = 0.8258). HRT measurements were significantly smaller (P < 0.0001) by a constant amount of 0.13 mm across the range of optic disk size. There was a significant difference between the HRT measurement of the composite image and the central cut (P = 0.0003 for observer 1). CONCLUSIONS: HRT measurements are significantly smaller than those from photographs and, therefore, are not interchangeable.
