Lethal brain abscess due to the fungus Scedosporium apiospermum (teleomorph Pseudallescheria boydii) after a near-drowning incident: case report and review of the literature.

A 39-year-old healthy man developed a brain abscess weeks after a near-drowning incident. Scedosporium apiospermum, the anamorph of Pseudallescheria boydii, was isolated from the abscess. The patient died 153 days after the accident despite antifungal therapy. We discuss the role of antifungals and review the literature for comparable cases.

Finding of a skeleton in the Altaussee Lake--a forensic odyssey.

Divers found several human bones in a lake in central Austria. As signs of blunt force trauma to the skull were detectable and one of the possible perpetrators still was alive, exact medico-legal examination was necessary. Age of the person and time since death were estimated. The residues of possible brain tissue underwent histological examination. Two of the three missing persons could be excluded, one of them using mitochondrial DNA analysis. Any pending legal proceedings could be avoided as a result of our examinations.

Rare fatal vascular complication of transsphenoidal surgery.

We report the case of a 61-year-old man, who underwent transsphenoidal surgery for a pituitary macroadenoma. The presence of tough fibrous septa dividing the tumour permitted only a partial resection. Progressive loss of consciousness soon after surgery occurred, an emergency CT scan showed no evidence of haemorrhage. Twenty hours later, MRI revealed compression of both internal carotid arteries with arrest of arterial flow resulting in stroke by an enlarged haemorrhagic mass consistent with a pituitary apoplexy. On the second postoperative day, the patient died as a result of this extensive stroke. The mechanisms of this rare complication after transsphenoidal surgery are theorized and the sensitivity of imaging methods is discussed.

Fatal truck-bicycle accident involving dragging for 45 km.

Vehicle-bicycle accidents with subsequent dragging of the rider over long distances are extremely rare. The case reported here is that of a 16-year-old mentally retarded bike rider who was run over by a truck whose driver failed to notice the accident. The legs of the victim became trapped by the rear axle of the trailer and the body was dragged over 45 km before being discovered under the parked truck. The autopsy revealed that the boy had died from the initial impact and not from the dragging injuries which had caused extensive mutilation. The reports of the technical expert and the forensic pathologist led the prosecutor to drop the case against the truck driver for manslaughter.

Cytogenetic study of six cases of radiation-induced meningiomas.

It is known that, following radiotherapy, secondary cancer may occur after a long latent period. Few cytogenetic studies have been reported on tumors of the central nervous system occurring after radiotherapy. We report the cytogenetic study of six cases of radiation-induced meningiomas. In all cases, we observed the same chromosome abnormality, der(1)(1qter-->1p11::22q12-->22pter). SKY and CGH techniques allowed us to identify the chromosomal abnormalities. We suggest that a gene localized on 1p13 is involved in radiation-induced meningiomas.

Sudden death from myocardial contusion following an isolated blunt force trauma to the chest.

Cardiac contusion is a common concomitant injury in blunt, non-penetrating chest trauma and is mostly a benign disorder which remains undiagnosed. In the case presented, a young man sustained a single blunt trauma to the chest from falling pieces of a wooden wheel and died at the scene. Among other findings, the autopsy revealed a circumscribed detachment of the coronary arteries on the anterior side of the heart. The most unusual findings were lacerations of the vessel wall layers in these areas which could already be seen at the autopsy and were proven by histological examination.

Colchicine poisoning by accidental ingestion of meadow saffron (Colchicum autumnale): pathological and medicolegal aspects.

Although intoxications with colchicine, the alkaloid of Colchicum autumnale (meadow saffron), are well known, in most cases the intoxications are evoked by oral or parenteral preparations traditionally used as medication against gout. The accidental ingestion of Colchicum autumnale, on the other hand, is a rare event and has to our knowledge only twice been described in detail. We report a further case in which two persons confused this highly poisonous plant with wild garlic (Allium ursinum), a popular spice in the Central European cuisine. While one person merely complained about a 3-day episode of nausea, vomiting and watery diarrhea, the second person died of multi-organ system derangements 48 h after the ingestion of the colchicum leaves. At autopsy hemorrhagic lung oedema, hypocellular bonemarrow, centrilobular fatty necrosis of the liver and necrosis of the proximal convoluted tubuli of the kidneys were observed. A colchicine concentration of 7.5 micrograms/ml was found in the bile whereas no substance was detected in the postmortem blood.

Fatal missile injury from the rotating knife of an agricultural mower.

The first case of a lethal injury inflicted by a projectile-wise shot piece of the rotating knife of an agricultural mower (gyro mower) is presented. The knife had travelled approximately 100 m through the air before hitting the victim's body. It transected most organs of the ventral neck including both carotids, the trachea and the esophagus and led to death from exsanguination. The type of the knife which was recovered from the body led to the identification of the mower whose operator had not noticed the accident. Legal action against the user or against the producer of the mower were considered but dismissed as no safety regulations had been violated.

[Sudden infant death (SIDS) in Austria. How reliable is the diagnosis?].

The sudden infant death syndrome (SIDS) is the most frequent cause of death during the postneonatal period (2-12 months) in Austria. A retrospective analysis of the SIDS cases registered between 1988 und 1992, however, shows marked differences between the provinces with regard to the relative proportion of SIDS against total infant mortality figures. The use of different definitions for SIDS seems to be responsible for local differences. Thus, a standardized investigation for all sudden unexpected deaths is required (autopsy, histological investigations, death scene investigation). Furthermore, a uniform classification for sudden unexpected infant death should be applied.

Injuries by chicken bills: characteristic wound morphology.

A 90-year-old woman, hampered in walking, was found dangerously wounded in a farm next to a chicken yard. On the occasion of forensic clinical investigation in hospital we found hematomas and many peculiar injuries of the epidermis and some defects even reaching the extremities of the subcutis and fascia. After experimental investigations we could show that these injuries were caused by the bills of chickens.

[The Austrian penal code and the Codex Ur-nammu--a comparison from the forensic medicine viewpoint].

A comparison between the Mesopotamian Law (Codex Ur-nammu) and the Austrian Penal Code reveals the long-sightedness of the founder of the 3rd dynasty from Ur, called Ur-nammu. It seems extremely remarkable that at those remote times (3rd Millenium B. C.) body injuries were satisfied by simple money-penalties. The Codex Ur-nammu therefore not only represents on exceedingly piece of work from the historical point of view but also from the point of view from Legal Medicine.

[Light and electron microscopy studies of actinomycosis of the lacrimal duct].

For the first time we present electron-microscopic results of an actinomycosis of the lacrimal canaliculus. The interior of the actinomycotic conglomerate showed no evidence of a cellular defence reaction but in the loosly woven outer network of hyphae we observed a massive granulocytic reaction. Also, after phagocytosis the structure of the actinomycotic micro-organisms within the granulocytes is not significantly damaged. Within the tissue of the lacrimal canaliculus adjacent to the actinomycotic conglomerate we observed an increased number of plasmacells but no pathogenic organisms.

[Electron microscopy studies of the trabecular meshwork in pigmentary glaucoma].

Trabeculectomy specimens from three patients with pigmentary glaucoma were investigated electron-microscopically. The pigment accumulations were highest in the uveal and inner corneoscleral trabecular meshwork. The outer lamellae and the cribriform region were hypochromic, while the endothelium of Schlemm's canal was unpigmented. The aqueous pathways of the inner trabecular meshwork were obstructed by hyperpigmented cells, cellular fragments and released pigment granules, those of the outer trabecular meshwork by depositions of extracellular material and collagen, by degenerative cells and by collapse of the trabecular lamellae.
