[Analysis of the first 1000 implants of intraocular lenses].

The authors give an account of the development of implantology and extracapsular extraction in their department. In 1987 they performed 342 operations of cataract, an intraocular lens was implanted in 13.2%, extracapsular extraction was performed in 12.9%. In the first half of 1991 354 operations of cataract were performed, an intraocular lens was implanted in 82.5%, and extracapsular extraction was performed in 96.9%. The most frequent peroperative complication was rupture of the posterior capsule--from 14.0% to 20.1%. The most frequent postoperative complication (306 implantations in the first half of 1991)--keratopathia striata 179, fibrinous exudate--79, elevated intraocular pressure--50. The time spent in hospital in the first half of 1991 was on average 2 days in 156 operated patients who did not develop any or only one postoperative complication. The most frequent postoperative complication was posterior capsule opacification--11%. The most serious late complication during the entire follow-up period was endo-ophthalmitis in 8 eyes which in 5 patients after an interval up to 12 months following operation ended by evisceration of the bulbus--in all instances Russian lenses which were sterilized by ourselves were involved. The visual acuity on admission was below 6/60 in 72.3%, on discharge 6/12 or better in 32.8% and 6 months after operation 6/12 or better in 73.9%, incl. 651 eyes (88%) without supplementary correction. Despite the difficult conditions extracapsular extraction with implantation of an intraocular lens has become a routine method in our department, with a short period of hospitalization and minimal contraindications.

Cataract Extraction

[Use of 5-fluorouracil in complicated cases of glaucoma--personal experience].

The authors give an account of the results of trabeculectomy with the use of 5-fluorouracil in a group of 8 patients with complicated glaucoma. Treatment was successful in 5 patients, in three patients it failed. The use of 5-fluorouracil in specific cases of glaucoma gives greater hope of successful surgery.

[Results of 52 operations for traumatic cataracts].

The authors are presenting the results of the traumatic cataract surgery in 52 patients (45 male, 7 female). The cataract was caused in 7 cases by blunt trauma, in 43 cases by penetrating trauma (in 16 cases with intraocular foreign body). The mean age of patients was 29 years, ranging from 4 to 73 years. In 2 cases was indicated the intracapsular extraction for the subluxation of the lens, in 2 eyes the discision was done and in 2 eyes the primary evacuation. In all other cases the extracapsular extraction was performed (19 with IOL implantation). Before the operation the visual acuity was 0.1 and worse in 90.4%, 0.16-0.33 in 7.7% and 0.5 and better in 1.9% of the patients. The visual acuity at the time of the last follow-up visit (3 months to 3.5 years after the operation-average 15.6 months) was 0.1 and worse in 16.7% of cases, 0.16-0.33 in 19% and 0.5 and better in 64.3%. The visual acuity 0.5 and better was in 85.7% of patients with IOL implantation. The most serious complication was the hypotony of the eye (in 3 cases) followed by the atrophy of the eye (1 eye was enucleated). Although we could expect more complications of the surgery of traumatic cataract it is possible to gain good visual acuity results. The IOL implantation is indicated in cases where it is not excluded by the condition of the eye.

[Cataract operations and implantation of intraocular lenses in children and adolescents].

The authors are presenting the results of 60 cataract operations performed in 54 patients younger then 18 years. Traumatic cataract was operated in 33 patient (29 boys, 4 girls), congenital cataract in 19 eyes and complicated cataract in 8 eyes. The visual acuity 0.5 and better was gained in 40% of the eyes in which it was possible to find out the visual acuity. This visual acuity was achieved in 50% of 19 patients with IOL implantation (the youngest was 4 years old). The visual acuity was 0.5 and better in 75% of 12 patients regularly wearing contact lens. The worse results were in patients with congenital cataract-just one eye from 10, in which the visual acuity was possible to evaluate, had the visual acuity 0.5. In one case it was necessary to replace the luxated posterior chamber IOL. The IOL implantation in children, especially in those with traumatic cataract, is becoming more frequent.

[Steroid glaucoma in severe atopic keratoconjunctivitis. A reminder for possible therapy with plasmapheresis].

In 1989 in the authors' hospital four plasmaphereses were performed in a patient treated with steroids on account of severe atopic keratoconjunctivitis with suspicion of hyperimmunoglobulinaemia IgE. As a result of treatment of the basic disease the patient developed steroid glaucoma. The authors recommended as alternative treatment plasmapheresis in order to discontinue corticoid therapy. Three months after the fourth plasmapheresis the intraocular tension improved and is still within a normal range.

[Clinical index for prediction of visual acuity after cataract surgery--personal experience].

The authors are giving an account of their own experience with the clinical index for prediction of the visual acuity after cataract surgery. The index uses the following data - age, visual acuity before operation, number of regularly taken drugs and frequency of reading before operation. In 171 eyes of patients operated on account of cataract by different techniques (the patients were not specially selected and some suffered from other eye diseases), the authors found a close agreement between the vision predicted by means of the clinical index and the final visual acuity after operation. Agreement was recorded in 68%. The authors recommend the clinical index for more extensive clinical tests.

[Scleroplasty surgery. I. Results in children].

The authors give an account on the results of scleroplastic operations (according Pivovarov) in 159 children (298 eyes). They describe in detail the surgical procedure, the mechanism of action of the operation, the fate of the implanted material. The most suitable material for scleroplastic operations in children is the irradiated human sclera which caused the minimal incidence of complications. Most frequently eyes with the diagnosis myopia progressiva 57.7% were operated, another large group were eyes with the diagnosis of myopia gravis -35.2%. The mean age at the time of operation was 11.3 +/- 3.3 years. In myopia progressiva the correction was three years after operation the same or lower in 53.3% of the operated patients, the vision was equal or better in 80% of the operated patients. In children with myopia progressiva who were operated at the age of 10-15 years, after three years no difference in vision was recorded as compared with the finding before operation; the correction changed on average only by 0.27 dioptres. This result is very satisfactory, as indication for operation was progression of myopia by at least 1 dioptre per year before operation. In myopia gravis the correction was three years after operation equal or lower in 68.4% of operated eyes, the vision was equal or better in 81.6% of the operated eyes. Reinforcement of the sclera--scleroplasty--is at the moment the only rational therapeutic method of progressing and severe myopia.

[Scleroplasty surgery. II. Results in adults].

The authors give an account of the results assembled in 181 patients (350 eyes) where scleroplasty in Pivovarov's modification was performed. 81.4% of the operations were indicated in myopia gravis, 91% in myopia progressiva and 5.1% in combination of myopia gravis and cataract. Of four implantation materials used the irradiated human sclera proved best. Scleroplastic operations had a favourable effect on the subsequent course of myopia gravis and myopia progressiva (followed up to four years). In both these diagnoses during the postoperative period a statistically significant improvement of visual acuity was observed, while the correction changed only slightly. Scleroplasties are a suitable surgical procedure in adults who suffer from progressing or severe myopia and give them real hope of preservation of visual functions.

[Results of keratoplasty performed at the Eye Clinic of the ILF in Ustí nad Labem].

The authors give an account of their experience and results with transplantations of the cornea in 79 eyes during a period of 44 months. They investigated the condition of the optic disk and visual acuity with optimal postoperative correction in patients after keratoplasty. In 61% of the eyes vision improved as compared with the condition prior to operation, in 61% of the eyes the disk of the donor remained clear throughout the observation period. The observation period was at least one month, the maximum being 44 months.

[Analysis of patients with retinal detachment surgery performed at the Eye Clinic of the ILF of the Regional Hospital in Ustí nad Labem 1987-1989].

In a group of 104 patients operated at the Ophthalmological Clinic in Ustí nad Labem on account of retinal detachment in 1987-1989 the authors investigated the anatomical and functional results of the operation during an average period of 15 months. Anatomical attachment of the retina was achieved in 82%; 52% of the patients improved from the functional aspect. In addition to vision on admission, discharge and during the last check-up the authors evaluated the extent of retinal detachment, the site of fissures, the number of myopes and aphakic subjects in the group and the period of retinal detachment before operation. There were 37.5% myopes and 27.9% aphakic patients and artephakia was recorded in 4.8%. The duration of detachment of the retina before operation was 7.4 weeks (3 days to 2 years). An association between the development of retinal detachment and injury was proved in six patients (5.8%).

[The effectiveness of taking measures against infection in eye surgery. Microbiological study].

The authors investigated in a group of 100 eyes the incidence of micro-organisms in the conjunctival sac and on the borders of the eyelids and their sensitivity to antibiotics before planned operations of the eyes. In all instances cultivation at three time intervals was used--on admission, on the morning before operation after administration of antibiotic drops for at least one day, and in the operation theater after careful disinfection of the surgical field. The assembled results indicate the small effectiveness of short-term preoperative administration of disinfectant and antibiotic drops and confirm the high effectiveness of antiseptic preparation of the surgical field.

[Initial experience with surgery for retinal detachment using a balloonprobe].

The authors present an account of their initial experience with balloon probe according to Lincoff-Kreissig. The surgical procedure was that described by the authors without modifications. The balloon probe was used in five patients, twice because of fatigue of material an episcleral implant had to be used. The retina was put in position in all patients. Temporary buckling by means of a balloon probe is a simple and safe therapeutic method in suitable types of detachment of the retina--fissures to the extent of 1 hour of the circumference of the retina, without traction of the vitreous body. After training at a department with sufficient experience with this method, it can be used even in departments where the number of operations of detachment of the retina is not high.

[Combination therapy of the retina in diabetic retinopathy].

Authors refer experiences with the complex therapy of diabetic retinopathy combining laser photocoagulation with cryocoagulation. Various indications and techniques of treatment were described. Equatorial cryocoagulation is advantageous with respect for its rapid performance without excessive loading of the patient and has similar effects as photocoagulation. Cryocoagulation is recommended as the operation of first choice in cases of the partial intravitreal hemorrhage. Cryocoagulation was performed in 51 eyes, 43 of them was followed for the mean period 7.2 months, with additional photocoagulation. After the cryocoagulation, which was preceeded or followed by photocoagulation, the visual acuity was measured one month after the operation and in 18 patients (42%) was improved, in 6 patients (14%) worsened. After one year in 12 followed patients 3 of them were improved, 7 patients worsened. Partial intravitreal hemorrhage with vision 6/60 and better was during one month sufficiently absorbed as to perform the further photocoagulation.

Combined Modality Therapy

[Use of soft contact lenses after scleroplasty surgery for myopia].

Authors refer their favourable experiences with the application of soft contact lenses used after the performed scleroplastic operation described by Pivovarov. Soft contact lenses were applied in 54 patients where myopia was operated. In 90% of patients the daily use of contact lenses was more than 8 hours. These patients tolerated the contact lenses even better than the patients with myopia but without operation. Compared with the correction with spectacles, the visual acuity was significantly increased in the group using the contact lenses. The visual acuity 6/6-6/9 was achieved in the group of contact lenses wearers in 63% and in the group of patients using spectacles was present only in 29%. In all patients wearing the contact lenses their specific working activity was distinctly enhanced. The modest scarring of the conjunctive as well as presence of implants does not interfere with the contact lens wear, when the myopic operation was carefully performed.

[CT localization of foreign intra-ocular bodies].

In the course of three years from the total number of 124 CT examinations indicated by the Ophthalmological Department of the Regional Hospital in Ustí nad Labem ten examinations of the orbit with an intraocular foreign body were made. Based on a group of 10 eyes the authors present a report of their hitherto assembled experience with the use of computed tomography in the diagnosis of intraocular foreign bodies. They draw attention to the advantages but also certain limitations of the location of foreign bodies by CT. In two of 10 cases the foreign body was located incorrectly by CT in the bulbar wall--although it was retrobulbar. In the remaining eight cases the CT examination was a great asset and contributed greatly to successful operation. 50% of the patients have after operation a vision of 6/6.
