[Intra-arterial chemotherapy in oral cancers. A current therapy?].

Reviewing arguments based on the pharmacokinetics of arterial infusions, on literature study results, and on their personal experience with 16 patients, the authors present a case for the administration of antitumoral agents by intra-arterial infusion as a method of dispensing induction chemotherapy to stomatological cancer patients. Utilization of implanted sites and discriminating selection of indications will improve the performance of intra-arterial chemotherapy.

[Serious cervicofacial infections of dental origin].

A review of 14 cases of serious cervico-facial infections of dental origin. The authors report a current mortality rate for acute diffuse cellulitis, of close to 50%, regardless of the publication. They stress the therapeutic errors which are the most frequent causes for delay in effective therapy which should obligatorily include: methodic surgical drainage, suppression of dental causes and appropriate antibiotic therapy.

[Papillary cystadenocarcinoma of the submaxillary gland. A rare diagnosis].

The authors describe a case of a "separate" type of salivary adenocarcinoma-like tumor. The growth is described as a papillary cystadenocarcinoma of the submaxillary gland, which should be distinguished from degenerate adenolymphoma. This malignant tumor, which remains "enclosed" until late, grows slowly and should be checked regularly over a protracted period of time following surgical excision performed in accordance with cancer therapy specifications.

[Intra-arterial regional chemotherapy in stomatological cancers using implantable chambers. 15-month experience].

The authors report their experience after 15 months of use of an implantable chamber for regional intra-arterial chemotherapy in stomatological cancer. This technique rekindles interest in intra-arterial chemotherapy via a retrograde temporal route, by simplifying the technique. The remarkable local tolerance of implantable chambers makes them preferable to external catheters and suitable for long term intra-arterial chemotherapy.

[The value of comparison of echography and sialo-tomography in the preoperative evaluation of parotid tumors].

Studying information provided by sialo-tomography and parotid ultrasonography in the pre-operative investigation of parotid tumors, the authors compare results provided by the two types of investigation. Whilst ultrasonography, a non-invasive technique, provides more detail concerning the topography and appearance of the tumor than sialography, it cannot yet be used as a substitute for this radiological investigation. The two would appear to be complementary, leading the last word to histopathology.

[Extra-osseous mandibular ameloblastoma].

Gingival ameloblastoma is a rare odontogenic tumor which shares the same histology as endo-osseous ameloblastoma but from which it differs in terms of its course by the absence of "local malignancy". There would appear to be two probable origins concerning this lesion: Serre debris and the epithelium of the gingival mucosa. However, the etiopathogenesis remains controversial, raising problems of pathological distinction from gingival basal cell carcinoma.

[Cryopreserved allografts in orthognathic surgery].

After more than two years' use of cryo-preserved allografts in orthognathic surgery, the authors show the value of the use in a General Hospital of material from a cryo-preserved bone bank supplied with femoral heads by the Orthopedic Traumatology Dept.

[The Temporomandibular Joint Diseases Unit of the Villeneuve-Saint-Georges Hospital Center].

A disease as complex as that of temporomandibular joint dysfunction can only be managed by a multidisciplinary team. The authors describe the temporomandibular joint diseases unit of the Centre Hospitalier de Villeneuve-Saint-Georges which consists of Stomatologists, Radiologists, Psychiatrists and Psychologists and a Rehabilitation Medicine doctor specialised in electrodiagnosis. The hospital's Department of Stomatology and Maxillofacial Surgery (Doctor Pierre Scheffer) coordinates this unit which holds committee meetings one a month to evaluate each case. The therapeutic decisions reflect the multiplicity of the aetiologies, none of which are mutually exclusive, which explains the necessity for a multidisciplinary approach.

[Opaque arthrotomography: a routine examination in temporomandibular dysfunction].

Tomographic examination of the temporo-mandibular joint after opacification of the meniscotemporal and meniscocondylian stages is an effective method of exploration of intra-articular lesions of the meniscal dynamic complex. Normal arthrotomographic appearances are described and four evolutory stages of the lesions proposed; by order of increasing seriousness: anterior position of meniscus, reducible anterior luxation, irreducible anterior luxation, meniscal perforation. The value of this examination is important during discussion of selective surgical treatment in painful dysfunctional syndromes of the temporomandibular joint.

[Reducible meniscal luxation of the temporomandibular joint].

Normal and pathologic functioning of the temporomandibular joint are summarized and clinical and arthrographic signs of reducible meniscal luxation described. The latter is distinguished clinically and radiologically from irreducible meniscal luxation and the myofacial syndrome. Etiological features are discussed and therapeutic solutions proposed in relation to three clinical cases reported.

[A plea for better compensation for dental lesions in adults under common law].

Compensation for dental damage in the adult (common law) must follow the physiological deficit and its consequences as closely as possible. Any dental loss must be compensated, but also any relative loss when dental trauma requires therapeutic mortification. Loss of the canines, of functional dental groups, unilateral posterior prostheses with prolongation, temporomandibular dysfunction and prosthetic difficulties must be taken into account in order to ensure just compensation for damage sustained. Prostheses and their replacement must be compensated separately from injury compensation. Accessory consequences linked to dental damage must not be neglected.

[Computerized data collection and processing of graphic cephalometric data in dentomaxillofacial orthodontics].

The authors present a graphic program adapted to cartography for the acquisition and processing of graphic data from x-rays and graphics used in dento-maxillo-facial orthopaedic cephalometry. They demonstrate the performance obtained in the automation of cephalometry, in the real-time edition of the results, in the processing of graphic images and the resulting alpha-numerical results.

[Anterior open bite: the role of the infra-alveolae in its diagnosis and treatment].

Anterior open-bites may be skeletal, alveolar or mixed. The infra-alveolae is always due to a postural or functional muscular cause and the clinical examination plays an important part in the diagnosis. Delaire's cephalometric analysis can define the type of infra-alveolae. In view of their aetiology, the infra-alveolae require functional re-education, although glossectomy and multiple bands therapy may also be useful.

[Prognosis and procedure in the surgical and orthodontic treatment of maxillomandibular dysmorphism].

The treatment of maxillo-mandibular dysmorphies, which involves both surgery and orthodontics, depends on a precise diagnosis and tactical strategies based on simulations of treatment using teleradiography, telephotography and impressions. These measures are essential in order to obtain a stable, good quality result in terms of occlusion and the dental, pariodontal and skeletal aspects.

[Anomalies of the path of closure].

Authors show some examples of shuting-way abnormalities. They show that only alveolar inclination can, by changing these abnormalities, modify the maxillo-mandibular occlusion.
