Osteoma of the sphenoid sinus.

OBJECTIVE: Only twenty cases of osteomas of the sphenoid sinus have been reported. This tumour causes progressively worsening headaches and visual disturbance and should be resected when symptomatic or fast-growing. In selected cases, endoscopic sinus surgery offers an effective alternative to open procedures. CASE REPORT: The authors report a case of sphenoid osteoma in a 19-year old woman. Computed tomography performed because of complaints of progressively worsening headaches identified a large osteoma of the sphenoid sinus. The clinical features and radiological assessment of the disease are presented together with a review of the literature. RESULTS: The endoscopic technique used for resection of the tumour gave a very good result. CONCLUSION: Sphenoid osteoma is an extremely rare lesion which can be approached endoscopically in selected cases.

[Video-roentgen cinematographic evaluation of pharyngeal phase disorders during swallowing in patients after partial laryngectomy due to localized supraglottic cancer].

Videorentgenocinematographic examinations of swallowing were conducted on 81 patients after partial laryngectomy and on 35 subjects being a control group. Resection of piriform recess, a part of base of the tongue, the hyoid bone or its part is the factor that causes intensified difficulty during swallowing and increase in the frequency of the occurrence of aspiration. The results of videorentgenocinematographic examinations indicate that the shape and mobility of the tongue and the mobility of remaining after the surgery parts of the larynx have the greatest influence on the efficient swallowing in patients who have undergone partial laryngectomy due to cancer initially located in the supraglottic area. The larynx mobility is closely related to the remaining of the hyoid bone. The importance of remaining the possibly non-deformed structure and mobility of the tongue during partial laryngectomy involves the issue of reconstruction of defects occurred during the surgery. Videorentgenocinematographic examinations confirm the effectiveness of the method involving reconstruction of defects in a part of the base of the tongue with a vascular pedicle flap of the submandibular gland.

[Objective evaluation of the influence of the tongue base reconstruction on the swallowing disorders].

Laryngographic, manometric and videorentgenocinematographic examinations of swallowing were conducted on 54 patients after partial laryngectomy and on 35 subjects being a control group. Resection of a part of the base of the tongue is the factor that causes intensified difficulty during swallowing and increase in the frequency of the occurrence of aspiration. The results of manometric and videorentgenocinematographic examinations indicate that the shape and mobility of the tongue has the greatest influence on the efficient swallowing in patients who have undergone partial laryngectomy due to cancer initially located in the supraglottic area. The importance of remaining the possibly non-deformed structure and mobility of the tongue during partial laryngectomy involves the issue of reconstruction of defects occurred during the surgery. Both manometric and videorentgenocinematographic examinations confirm the effectiveness of the method involving reconstruction of defects in a part of the base of the tongue with a graft of angio-pedunculated submandibular gland. The study of effectiveness of glottis or neoglottis performed with the use of laryngograph showed no relation between glottis/neoglottis occlusion effectiveness index and intensified swallowing disorders. This shows the lack of compensating option in swallowing disorders with the help of a mechanism based on an efficient glottis or neoglottis occlusion alone.

[Objective evaluation of the influence of the hyoid bone resection on the disorders in the act of swallowing].

Laryngographic, manometric and videorentgenocinematographic examinations of swallowing were conducted on 49 patients after partial laryngectomy and on 35 subjects being a control group. The results of examinations indicate that the mobility of remaining after the surgery parts of the larynx have the great influence on the efficient swallowing. The larynx mobility is closely related to the remaining of the hyoid bone. Resection of the hyoid bone or its part is the factor that causes intensified difficulty during swallowing and increase in the frequency of the occurrence of aspiration. The study of effectiveness of glottis or neoglottis performed with the use of laryngograph showed no relation between glottis/neoglottis occlusion effectiveness index and intensified swallowing disorders. [Videorentgenocinematographic analysis of pharyngeal phase in the act of swallowing].

The course of the pharyngeal phase was analysed in physiological conditions. The mobility of anatomical structures taking part in the phase was evaluated, taking into consideration the time sequence in which the elements of the phase followed. To objectify the evaluation of the analysed phenomena, authors of this paper proposed and used linear and time parameters also used by other authors involved with these issues.

[Effectiveness coefficient for laryngographic closure of the glottis as a way of evaluating airway protection against aspiration of gastric contents].

Laryngographic examination was conducted on 35 men aged 45 to 65 with good hearing and proper pronunciation. Based on the examination, the proposed by the authors of this paper laryngographic closure effectiveness coefficient of the glottis (LCEC) was calculated as a way for evaluating protection of airways against the aspiration of food. Glottis occlusion serves an essential function as a mechanism protecting airways, especially in patients who have undergone horizontal supraglottic laryngectomy. The examination was conducted with the purpose of obtaining numerical values of LCEC, which will be treated as results of the controlled group during the examination of swallowing disorders in patients with partial laryngectomy.

[Evaluation of airway protection against aspiration of gastric contents in patients after partial laryngectomy using the laryngographic closure effectiveness coefficient of the glottis/neoglottis].

Laryngographic examinations were conducted on 97 patients who had undergone partial laryngectomy and on 35 subjects being a control group.

Acoustic assessment of voice signal deformation after partial surgery of the larynx.

OBJECTIVE: The main objective of the present study was to assess the degree of voice signal impairment among patients who had undergone partial surgery of the larynx due to cancer of this organ. Such on evaluation may be helpful in the selection of the optimal surgical technique for the treatment of tumors displaying a varying degree of local advancement. METHODS: A prospective examination was carried out among 128 patients. Additionally a comparative study of the control group consisting of 36 healthy males was carried out. Acoustic tests were carried out in an echo-free chamber. The temporal changes in the value of acoustic pressure of the uttered text were registered. The 'distance' between the normal speech signal and the pathological voice has been established. RESULTS: The values of the fundamental frequency increase together with an increase of the range of resection of anatomical structures. The biggest differences in the value of results describing the distance from the standard were observed after hemilaryngectomy. The shortest distance from the acoustic standard was observed after chordectomy. No significant differences in the degree of voice signal impairment among patients who had undergone extended chordectomy and hemilaryngectomy were observed. CONCLUSION: The above findings can be of help in arriving at an optimum solution in cases of partial surgery of the larynx. The problem is particularly important in situations where there is the choice between different types of surgery.

Studies on swallowing in patients operated by subtotal laryngectomy.

A series of 39 patients with problems of deglutition after total laryngectomy was studied by X-ray screening and manometry. The importance of the retropulsive movement of the tongue on the quality of deglutition has been demonstrated, as has the function of the superior oesophageal constrictor, the effect of a reduced relaxation time, and of the force of suction in reducing its quality.

A possible correlation between vertebral artery insufficiency and degenerative changes in the cervical spine.

We studied 130 patients, aged 20 to 81 years, with symptoms of tinnitus, vertigo or dizziness. Radiological examinations revealed degenerative changes in the cervical spines of all patients such as discopathy or osteophytes. Head and neck and neurological examinations ruled out other symptoms apart from vertebrobasilar artery flow insufficiency. The vertebrobasilar arteries were examined by means of a color Doppler ultrasonograph using duplex scanning. The correlation coefficient (CC) defining the relationship between the number of patients with abnormal blood flow and the total number of patients with radiologically confirmed changes in the cervical spine was 41.5%. When patients were separated by age, the value of the CC coefficient increased proportionally according to age, changing from 0 to 79.1%. Use of the Doppler ultrasonograph was found to be a safe and non-invasive diagnostic method that enabled us to assess the influence of degenerative changes in the cervical spine on hemodynamic disturbances in the inner ear and brain stem. Our findings demonstrated a pathological decrease of vertebral artery flow velocity in relationship to degenerative changes in the cervical spine.

Effectiveness of classical chordectomy in the treatment of cancer of the glottis.

From 1980 to 1992, 85 patients diagnosed with tumour of the glottis, localized exclusively in the vocal cord area, had undergone surgery. A 90% 5-year survival rate without recurrences had been obtained. Taking into account the cases of life-saving surgery, the percentage of the 5-year survivals amounted to 94%. Considerably better results were obtained by the excision of the entire vocal cord and not just one of its sections (1/2, 2/3 etc.). The degree of deformation of the voice following chordectomy was assessed by means of acoustic and laryngographic methods (Laryngograph Processor PCLX). A 13% rate of irregularity in the functioning of the neoglottis, following surgery, was observed. The Jitter-Shimmer co-efficients were established. The results of the deformation of the voice following chordectomy were presented on the J-S scale and compared with other partial surgeries performed on patients with tumours of the glottis. All of the acoustic and laryngographic findings were presented in the from of mean values most representative of chordectomy.

[The correlation of Doppler's blood flow distempers in vertebral arteries with degenerative cervical spine changes of patients undergoing treatment for tinnitus].

120 patients aged 20-81 years were treated at the Otolaryngology Clinic of the CM UJ in years 1994-1996 for tinnitus. The radiological method showed degenerative changes of the cervical spine in 100 of them. The otolaryngological and neurological methods excluded any reason of tinnitus other than distempers of vertebrobasilar arterial system. The ACUSON 128 XP/10 (Duplex Scan + Color Doppler) was used during examination of blood flows in vertebrobasilar arterial system. The correlation index was calculated as a proportion between the number of patients with pathological vertebral artery blood flow and the total number of patients. For all patients aged 20-81 it was 45.0%. The correlation index for the subsequent age groups grew proportionally to the age of patients, reaching values from 0% to 73.7%.

[Analysis of causes of the parotid pleomorphic adenoma recurrence].

Out of the total number of 173 patients who had undergone surgery for pleomorphic adenoma of the parotid gland, 19 had to be treated owing to the recurrence of the tumor, following earlier surgery 12 patients were reoperated on by means of the enucleation technique; 5 underwent superficial parotidectomy; in 2 cases the recurrence took on the form of tumor of the parapharyngeal space, which was removed through the cervical-parotid approach. The causes of the recurrences were analyzed taking into consideration the operation technique applied in the first surgery. A serious consequence of pleomorphic adenoma recurrence is the possibility of its becoming malignant, which took place in 2 cases (10.5%), as well as a permanent paralysis of facial nerve during the reoperation, which occurred in 4 cases (21%).
