[Rare forms of non-neoplastic tumors of the lacrimal gland].

Out of the total number of 20 patients who in years 1982-1996 underwent surgery of the lacrimal gland in the Otolaryngological Clinic of the CM UJ, in 2 cases a rare form of tumor was diagnosed. They turned out to be: amyloidosis and sarcoidosis. Case histories, diagnostic difficulties, as well as methods of surgery, taking into consideration the latest views on the subject, were presented.

[Therapeutic and diagnostic difficulties with parapharyngeal tumors].

Tumors of the parapharyngeal space are not very common. The case with big tumor of the parapharyngeal space was described, primary misdiagnosed as the tumor of the parapharyngeal tonsil and then as the tumor of the soft palate. The tumor was removed finally from the external parotido-cervical approach.

[Changes in vascularization of sternocleidomastoid myocutaneous flaps].

Studies of variation in the vascularisation of the sternocleidomastoid myocutaneous flap. An investigation of vascularisation of the sternocleidomastoid myocutaneous flaps was conducted. The research was performed on human cadavers and after injection of arteries 40 myocutaneous flaps of sternocleidomastoid muscle were isolated. The experiment was performed to investigate the origin of the arteries vascularising the muscle and the area of skin, which were used to make the myocutaneous flaps. Changes are characteristic in the lower 2/3 parts of the muscle. These variables may exert an important influence of the outcome of reconstruction treatment in the case of the flaps used from the lower attachment of the muscle.

[Chemodectoma of the nasal cavity and paranasal sinuses].

A rare case of chemodectoma of the nasal cavity and paranasal sinuses is presented. Chemodectoma are unusual tumours arising in specialized tissue probably of neural cres origin. The patient underwent surgery with total removal of the tumour. The presence of such a tumour raises questions as to the origin of such specialized tissue within the nose and paranasal sinuses.

Microvasculature of the human fetal laryngeal anterior commissure.

The vascular pattern of the injected with Mercox human fetal larynges, especially of the anterior commissure and surrounding it area, was analysed within the horizontal and frontal sections under a light microscope. The main result of our study was confirmation of existence of the avascular "plane zero" situated above the anterior commissure, which separates that structure from the supraglottic tier. That finding i.e. avascular separation between teh middle and upper laryngeal compartments, already present in the fetal period, has clinical implications as far as the spread of neoplastic process within the larynx is cancerued.

[The clinical significance of metastatic cancer of the larynx to the Delphi node].

Between years 1965-1987, 920 patients with cancer of the larynx were operated on. In this time we identified 9 cases of histologically proven metastasis to the Delphian node. In all patients except one, the neck was clinically negative but a positive Delphian node was discovered at partial or total laryngectomy. Five patients developed metastasis postoperative. Seven of 9 patients have died from their laryngeal cancer. The frequency of neck metastasis and local recurrences is very high in patients with a positive Delphian node.

[Studies of the changes in vascularization of the myocutaneous flaps of the pectoralis major. Preliminary report].

The research in the vascularisation of myocutaneous flaps of the pectoralis major muscle was conducted on the human cadavers. After the injection of the arteries 20 myocutaneous flaps were created. The main artery which is the source of vascularization of this muscle is pectoralis branch of thoraco-acromial artery. The objective of the research was to find out the direction of run of this artery. The direction variability was defined. The variability in the run of the branch of the thoraco-acromial artery refers to the place of departure from the thoraco-acromial artery, does not influence usefulness of the pectoral major muscle in creating the myocutaneous flap, considering the constant pathway of run of the distal part of this artery. The results of our research was compared with the classical description of the vascularisation given by the creator of this flap as well as with the works of authors dealing with the same issue.

[Malignant neoplasms of the middle ear in Krakow Otolaryngological Clinic in the years 1962-1994].

In the years 1962-1994, 20 patients were treated with a surgery because of malignant neoplasms of the middle ear. Pathomorphological examination showed rhabdomyosarcoma and squamous carcinoma. After surgery patients were treated supplementary Rtg-th. As a results of treatment 45% asymptomatic 5-years long survivals. A considerable advancement of the disease, late diagnosis and resulting from it nonradicality of the surgery influenced the results. Other reports and own observation advocate combined treatment: surgery and irradiation for radiosensitive malignant neoplasms of the middle ear.

[Facial nerve damage after parotid surgery in benign diseases].

One hundred fifty three patients who underwent parotid surgery for benign neoplasia in the past 22 years we reviewed with attention to postoperative facial nerve function. Immediate dysfunction was frequently encountered 28% but permanent dysfunction was uncommon 3%. The incidence of long-term dysfunction is higher in revision cases.

Angiomorphology of the retrobulbar part of the optic nerve. A scanning electron microscopy of vascular casts.

The presented study was carried out on the fetal specimens of the eye-ball, together with the retrobulbar part of the optic nerve which were obtained from 15 human fetuses aged 16-20 weeks of the intrauterine life. The vascular casts were prepared by injection of low-viscosity resin Mercox which afterwards were analyzed in a scanning electron microscope (SEM). The technique employed allowed the authors to describe both the vascular pattern of the optic nerve as well as the network of arterial vessels which formed the Zinn and Haller's circle. The own original findings are compared with results obtained by other authors on the vasculature of the optic nerve in mammals and humans in different periods of the life.

Microvasculature of the dorsal mucosa of human fetal tongue: a SEM study of corrosion casts.

The vasculature of the dorsal mucosa of the tongue was investigated in 18-21 week human fetuses by corrosion casting and scanning electron microscopy. Microvascular systems of the fungiform, foliate and circumvallate papillae, albeit less complex, showed similarity to those described in children, while the capillary networks of the filiform papillae were not yet fully developed, having either a knot-like or cone-like form instead of the corolla-like pattern typical of the postnatal period. Morphological features suggesting angiogenesis included both capillary outgrowths characteristic of vascular sprouting and tiny holes in the vascular walls regarded as evidence of intussusceptive capillary growth. The subpapillary vascular network supplying and draining the papillary vessels was composed of the more superficial capillary bed and the deeper plexus of larger arterioles and venules.

[The suprahyoid approach for carcinoma of the base of the tongue].

From 1989 till 1993 10 patients with cancer of the base of the tongue were treated surgically. The surgical approach was suprahyoid pharyngotomy in 3 cases expand to intraoral approach and lateral pharyngotomy. Three patients died 3 years after surgery. All patients underwent adjunctive irradiation. The suprahyoid pharyngotomy is an ideal surgical approach in the management of T1 or T2 squamous cell carcinoma of the base of the tongue.

Microvasculature of the human fetal extra-ocular muscles. A SEM corrosion casts.

In the light of already known differences on the vascular patterns between slow and fast contracing skeletal and extra-ocular muscles (EOM) which has been described in both mammals and man we were interested to see if same of these features are presented in the human EOM during the second half of the gestation. Therefore the microvasculature bed of EOM was studied by SEM of vascular corrosion casting technique. 15 human embryos at 16-20 week of gestation, after spontaneous abortion, were used for this study. The vasculature bed of the all EOM studied is composed of two layers i.e. the orbital and the global. The orbital layer, in all specimens examined, has shown a relatively higher number of transverse anastomoses than the global one, although the capillary vessels were arranged similarly on the both sides.

[Evaluation of elemental and mineral composition of larynx thyroid cartilage in the period of its involutional mineralization].

The research has been conducted on the material of 170 larynx thyroid cartilages of persons in the age range from 19 to 99 years, applying the radiological method, spectroscopy, electyronic scanning microskopy and rentgenological diffractometry. The distribution of certain elements has been determined in fragments of thyroid cartilage with different progression of mineralization. It has been proved that the elemental composition of the examined thyroid cartilages depends on the individual age and the progression of mineralization, resulting from the age.
