[Treatment of vomiting in infants and children induced by acute infectious pathology. A comparative study of alizapride versus metopimazine].

The effectiveness and tolerance of alizapride and metopimazine used to treat vomiting induced by acute infectious diseases were evaluated in 47 infants and children seen in five hospitals. Patients were randomized to alizapride (n = 23) or metopimazine (n = 24). Medications were given orally (drops) for 3 to 5 days. All the patients were monitored until the end of the study period. Effectiveness was excellent or good in both groups with no statistically significant difference. Clinical tolerance was outstanding in both groups; one patient in the alizapride group exhibited transient, mild drowsiness after the doses. This study confirms the good risk/benefit ratio of alizapride in the treatment of emesis in infants and children.

Postinfectious encephalitis with multifocal white matter lesions.

Two cases of multifocal white matter lesions occurring after viral illness are reported. Evoked potentials study and cranial magnetic resonance imaging (T2-weighted image) showed early abnormalities while CT scan was initially normal. Patients improved dramatically with steroid therapy. It would seem that because of a considerable responsiveness to steroids this affection should be differentiated from other types of encephalitis. Relations with multiple sclerosis are discussed.

Risk of recurrence after a single, unprovoked, generalized tonic-clonic seizure.

A cohort of 119 two- to 16-year-old patients was followed to assess the risk of recurrence after a single, unprovoked, generalized tonic-clonic seizure. Only children with a normal neurological examination and intellectual development, and an EEG showing no focal abnormality, were studied. The risk of recurrence at six months was 22 per cent, at one year 28.5 per cent, at three years 32.6 per cent and at eight years 37.7 per cent. 58 per cent of recurrences occurred within the first six months and 87 per cent within the first two years. Initial EEGs showing generalized, unprovoked spikes and waves, generalized paroxysmal discharges precipitated by intermittent photic stimulation, or isolated slowing, were associated with risks of recurrence at two years of 40, 50 and 26 per cent, respectively. These results do not differ significantly from those obtained when the initial EEG was normal (29 per cent). Age, seizure duration and history of epilepsy in the family were not associated with a significantly higher risk of recurrence. The risk of recurrence was similar for treated and untreated patients.

[Congenital aortic valve stenosis treated by dilatation. Apropos of a case with hemodynamic control 12 months later].

Intraluminal dilatation of congenital aortic valve stenosis was attempted in a 14-year old boy. Significant improvement was obtained, with a fall in transaortic gradient from 80 to 30 mmHg. A control haemodynamic examination performed 12 months later confirmed that the result was stable; there was no aortic leakage, and myocardial hypertrophy had begun to regress at echocardiography. This case is of interest in that dilatation is less costly than surgical commissurotomy. However, this technique cannot be widely used until satisfactory long-term results have been demonstrated in a large population of children.

Dystonic reactions with metoclopramide: is there a risk population?

The authors examine 18 cases of early dystonic syndromes and review all the pediatric cases published previously in order to determine the evolutive and epidemiological characters of these intolerance reactions. These unrecognized manifestations normally occur within the first 24 hours of treatment and disappear within a few hours when the treatment is discontinued. The use of diazepam and trihexyphenidyl accelerates the regression of the disorders. The doses of metoclopramide were mostly above the recommended doses, 60% of the cases correspond to doses in excess of 0.5 mg/kg per day or 0.2 mg/kg in a single dose. In the absence of overdosage, dystonic reactions occur essentially in female subjects aged over 9 years. For this part of the population, the possible occurrence of such a reaction should be mentioned to the family when the drug is prescribed.

The response to initial chemotherapy as a prognostic factor in localized Ewing's sarcoma.

Ninety-five children with localized Ewing's sarcoma were included in a prospective cooperative study. All patients received initial chemotherapy with the purpose of early prevention of metastases and improvement of the conditions of the subsequent local therapy, radiotherapy in all cases, surgical resection in selected cases. Clinical response to initial chemotherapy was evaluated in 67 patients who had measurable soft tissue mass or functional symptoms. This response appeared highly correlated with outcome as the disease-free survival was 57.3% for the 41 good responders and 9% for the 26 bad responders (P less than 0.00001), though 23 of these bad responders reached complete remission with radiotherapy. This study also confirms the prognostic significance for survival of the site of the primary tumor on axial skeleton or on limbs. Nevertheless, this factor had no predictive value for response to chemotherapy, which thus appears to be an independent factor.

Platelet aggregation occurs in congenital afibrinogenaemia despite the absence of fibrinogen or its fragments in plasma and platelets, as demonstrated by immunoenzymology.

Platelet aggregation of an afibrinogenaemic patient's platelet rich plasma (PRP) was greatly decreased when ADP was used for stimulation. In the presence of collagen or arachidonic acid the changes in light transmission recorded during platelet aggregation of patient's PRP were similar to those observed with normal PRP but the size of aggregates appeared to be smaller in comparison with those observed with normal platelets. In addition, thrombin-induced aggregation of washed platelets was similar to normal platelets. The interpretation was made possible because the fibrinogen level in plasma and in platelets was found to be almost nil as demonstrated by both an Elisa procedure described here and the determination of fibrinopeptide A (fpA). Furthermore, fibrinogen fragments, which could result from abnormal synthesis and therefore replace fibrinogen in platelet aggregation, were undetectable by immunological analysis using specific antibodies against A alpha, B beta and gamma chains and 10 different monoclonal antibodies against fibrin degradation products.

Simultaneous occurrence of a T-cell lymphoma and a chronic myelogenous leukemia with an unusual karyotype.

The simultaneous occurrence of malignant T-cell lymphoma and chronic myelogenous leukemia is reported. The lymph nodes contained E rosette forming cells. Blood and bone marrow cell morphology were consistent with the diagnosis of chronic myelogenous leukemia. Lymph nodes, bone marrow and blood mitosis showed a t(6;8) (6pter----6q27 ::8p12----8pter;6qter----6q27 ::8p12----8qter) translocation. So far a number of recent reports have shown simultaneous B lymphoid and myeloid proliferations in some malignancies, this is apparently the first reported case of simultaneous T lymphoid and myeloid proliferations.

[Different congenital forms of Steinert's myopathy: contribution of EMG].

The study of two families with myotonic dystrophy enables us to recall the existence of two congenital forms: one severe form with important neonatal hypotonia and one mild form with talipes equinovarus and intellectual deficiency. In both forms EMG detection is very often of the myogenic type. It confirms the diagnosis of myopathic disorder.

[Caffey's disease in a mother and her 2 children].

This case of familial infantile cortical hyperostosis is reported because of 2 special features: abnormal x-rays were available in the mother and one son; the mother presented with late manifestations. Literature data show the fairly high incidence of familial cases of Caffey disease; however, both particular features reported here were rarely mentioned.
