[Acute neurological deficit in treated malignant hemopathies].

Neurological complications are frequent in the course of malignant hematological diseases. Five cases occurring in children are reported here: 4 presented with acute lymphoblastic leukemia and 1 with lymphoma. The clinical pattern was remarkably acute, with fever, seizures and a neurological deficit. Brain CT scan showed a limited hypodensity sometimes with a contrast uptake in the involved area in the first days or weeks. Seroconversion for measles virus occurred in 3 cases, associated with the presence of helix-like bodies in the cerebrospinal fluid and/or cerebral tissue. These case reports are discussed with respect to the neurological complications already reported in patients with hematologic malignancies. The hypothesis of measles-virus related encephalitis seems likely.

Myelofibrosis and acute megakaryoblastic leukemia in a child: topographic relationship between fibroblasts and megakaryocytes with an alpha-granule defect.

In a child with acute megakaryoblastic leukemia--severe thrombocytopenia and myelofibrosis, EM studies on bone marrow showed a strict topographic relationship between the presence of clusters of abnormal megakaryocytes and the increased number of fibroblasts and extracellular fibers. Megakaryocytes and platelets lacked alpha-granules while the plasma thromboglobulin level was three times the normal level. This suggested that the alpha-granular proteins were synthesized but not retained in alpha-granules. If this occurs, the increased marrow levels of platelet-derived growth factor and factor 4 would favor the proliferation of fibroblasts and the synthesis of collagen, and thereby promote myelofibrosis. After therapy-induced remission, the number of marrow megakaryocytes decreased, the alpha-granules were normally produced, the plasma beta-thromboglobulin level was normal and the myelofibrosis disappeared. These observations suggest that during acute megakaryoblastic leukemia, an acquired gray-platelet syndrome occurs and that the local excretion of alpha-granule proteins triggers the myelofibrosis.

A case of hemoglobin M Boston. New data about valency hybrids brought by isoelectrofocusing study.

In Hemoglobins M, as the result of a mutation theheme iron of the abnormal chain is atabilized in an oxidized form unable to carry oxygen. In this paper, we discuss the case of hemoglobin M Boston characterizing this abnormal hemoglobin as valency hybrid by spectral studies and utilising this mutant as a marker, in isoelectricfocusing, we bring the arguments that the two intermediary bands of oxidation correspond to alpha2+beta2+2 and alpha2+2beta2+3 We also indicate that the abnormal spectrum of Hb M interferes with the estimation of methemoglobin, giving erroneous values. We have shown that isoelectric focusing and subsequent scanning give a definite idea about the amount of abnormal chain present.

[Hyperparathyroidism in deficiency rickets. Changes after vitamin therapy].

The study concerned 16 cases of deficiency rickets observed over 3 years. The level of serum parathyroid (IPTH) hormone was always increased in the late stages of rickets, but was normal in 3 cases of early rickets with hypocalcaemia and monophosphataemia. There was no statistical correlation between the level of IPTH and monophosphataemia. There was no statistical correlation between the level of IPTH and calcaemia. After vitamin D-therapy, the levels if IPTH returned to normal in 5 to 21 days in most cases. No obvious difference was noted in this evolution between children treated by vitamin D2 and 25 OH D. The excretion of urinary cyclic adenosine monophosphate decreases in parallel with blood IPTH.
