Antibody regulation of Tcell immunity: implications for vaccine strategies against intracellular pathogens.

Intracellular microbial pathogens cause a plethora of diseases that pose a huge public health challenge. Efficacious prophylactic vaccines are needed to protect the population from this myriad of infectious diseases. Contemporary approaches to vaccine design are guided by the immunobiological paradigm that extracellular pathogens are controlled principally by humoral immunity, involving specific antibodies, whereas host protection against intracellular pathogens requires effectors of cell-mediated immunity. However, this distinct T-helper (Th) type 1 and 2 paradigm of host defense has encountered a major challenge due to the reality that most antigens or vaccines induce mixed immune responses comprising of both humoral and CMI effectors. Besides, the true functional independence of antibodies and T-cells under in vivo physiologic conditions is uncertain. Recent findings have revealed that antibodies exert a significant immunoregulatory effect on T-cell immunity. Thus, a robust and protective T-cell memory response against microbial pathogens such as Chlamydia and Mycobacteria require an effective primary humoral immune response characterized by specific antibody isotypes whose role is to modulate Th1 activation via Fc receptors (FcR) by facilitating a rapid uptake, processing and presentation of pathogen-derived antigens for an enhanced T-cell response. These findings have crystallized into a paradigm shift in host defense wherein different components of the apparently disparate mixed immune responses elicited against a microbial pathogen function concertedly to maximize the principal effector mechanism. This review focuses on the essential role of both arms of the immune system in controlling intracellular microbial pathogens, especially the regulatory role of FcR-mediated antibody function in optimizing the induction of a protective Th1 response. The immunobiological implications are discussed in the context of vaccine design, delivery and evaluation against intracellular microbial pathogens of bacteria, fungi and parasitic origin.

[Study of infusion of oxygen-enriched liquid to correct severe hypoxemia in infectious diseases: a report of pilot clinical study].

OBJECTIVE: To investigate a new therapy for effectively correcting severe hypoxemia in patients with infectious diseases by infusion of oxygen-enriched liquid, in order to raise the partial pressure of blood oxygen without passing through pathologically damaged alveoli of such patients. METHODS: Intravenous drip with oxygen-enriched liquids was given to 6 cases suffering from severe acute respiratory syndrome (SARS), and 3 cases of acquired immune deficiency syndrome (AIDS) in the course of treatment for 1 to 5 days, 500-700 ml per day. RESULTS: For all the 9 SARS cases, their hypoxemia was gradually corrected to normal in 20 minutes' or 4 hours' intravenous drip with oxygen-enriched liquid. Respiratory rate decreased from 29-49 breath/min to 18-22 breath/min, heart rate decreased from 89-145 beats/min to 60-79 beats/min, two faint patients regained consciousness, hypoxemia was redressed, partial pressure of oxygen in artery increased from 56 mm Hg (1 mm Hg=0.133 kPa) to 87 mm Hg, saturation of oxygen increased from 0.89 to 0.96. CONCLUSION: Intravenous drip of the oxygen-enriched liquid effectively helped correct the hypoxemia of SARS and other infectious diseases cases by bypassing the diseased alveoli through which oxygen would not pass into the blood by conventional oxygen inhalation. This therapy of oxygen-enriched liquid infusion could be quite life-saving in the combined treatment for SARS and other infectious diseases.

Comparison of (99)mTc-methoxyisobutylisonitrile myocardial single-photon emission computed tomography and electron beam computed tomography for detecting coronary artery disease in patients with no myocardial infarction.

BACKGROUND: Previous studies have compared single-photon emission computed tomography (SPECT) and electron beam computed tomography (EBCT) in detection of coronary artery disease (CAD) in patients with myocardial infarction (MI). The purpose of this study was to compare SPECT with EBCT in detection of CAD in patients with no MI. METHODS: One hundred and forty-seven patients with suspected CAD underwent stress-rest (99)mTc-methoxyisobutylisonitrile ((99)mTc-MIBI) myocardial SPECT, cardiac EBCT and coronary angiography (CAG) within one month. Of them, 73 patients (aged 52.6 +/- 10.6 years old) with no history of MI were included in this study. Coronary artery calcium (CAC) was defined as a CT value >or= 130 HU within the boundary of coronary artery on EBCT. RESULTS: There were 35 and 38 patients with or without CAD according to CAG. Ninety-six percent of the patients with abnormal SPECT and CAC had a coronary arteries stenosis >or= 50%, and 90.9% patients with normal SPECT and EBCT showed no CAD. The sensitivity of SPECT and EBCT in detection of CAD was comparable, and the specificity of SPECT (92.1%) was significantly higher than that of EBCT (55.3%) (P < 0.005). For the detection of individual coronary artery stenosis, both sensitivity and specificity of SPECT (75.0% and 93.7%) were significantly higher than those of EBCT (53.3% and 76.7%) (P < 0.025 and P < 0.005, respectively). In patients without chest pain, the sensitivity and specificity of SPECT (76.9% and 91.4%) were significantly higher than those of EBCT (23.1% and 69.0%) in detection of a coronary artery stenosis of >or= 50% (P < 0.01 and P < 0.005, respectively). However, in patients with chest pain, both sensitivity and specificity of SPECT were comparable to those of EBCT. In patients or= 50% (P < 0.005), and the specificity of SPECT was comparable to that of EBCT. In patients > 45 years old, the specificity of SPECT (94.3%) was significantly higher than that of EBCT (70.5%) (P < 0.005), and the sensitivity of SPECT was comparable to that of EBCT. CONCLUSION: (99)mTc-MIBI myocardial perfusion SPECT has higher accuracy than that of EBCT in detection of CAD in patients without MI.

Developing effective delivery systems for Chlamydia vaccines.

Members of the genus Chlamydia cause a plethora of ocular, genital and respiratory diseases, with severe complications, such as blinding trachoma, pelvic inflammatory disease, ectopic pregnancy and tubal factor infertility, interstitial pneumonia, and chronic diseases that may include atherosclerosis, multiple sclerosis, adult-onset asthma and Alzheimer's disease. The current medical opinion is that an effective prophylactic vaccine would constitute the best approach to protect the human population from the most severe consequences of these infections. There are three essential and mutually inclusive areas of challenge confronting researchers developing Chlamydia vaccines. These are to define the elements of protective immunity and the basis of vaccine evaluation, the judicious selection of an immunogenic and safe antigen(s) to form the basis of a subunit vaccine, and to develop effective delivery systems that boost the immune response to achieve long-lasting protective immunity. The development of delivery vehicles and adjuvants to boost protective long-term immunity against chlamydiae currently poses the greatest challenge in vaccine research. However, enormous progress is being made in the construction of novel delivery systems, such as DNA and plasmid expression systems, viral vectors, and living and non-living bacterial delivery systems, and the use of chemical adjuvants. In addition, there is increasing effort being made in designing delivery strategies involving specific immunomodulatory procedures that modify the cytokine and chemokine environment, upregulate co-stimulatory molecules and target vaccines to specific mucosal sites. These efforts will likely culminate in an efficacious chlamydial vaccine in the near future.

[Dynamics of ecological landscape pattern and its affecting factors in desert-oasis in Fukang, Xinjiang].

The spatial pattern of ecological landscape during land utilization in Fukang is heavily influenced by natural difference and the scale of water and land resource development. Analyses on the spatial pattern based on different zones and indexes showed that from 1987 to 1998, the change of the spatial pattern of ecological landscape during land utilization in Fukang was mainly the increase of plantation area in pluvial fan and the decrease in alluvial plain. The case was on the contrary about badlands. The acreage of woodland decreased in lower mountains, uplands and alluvial plain, but no variety in alluvial plain. The acreage of grassland increased in lower mountains and uplands, while decreased in other fields. The acreage of town increased in each sample field, while that of water area remained uncharged. The landscape diversity and evenness was descending, the dominance was ascending in lower mountains and in pluvial fan, while it was reverse in alluvial plain. Accessorial fragmentation showed the increasing influence of human beings. The change of the spatial pattern of ecological landscape in Fukang focused on the acreage alteration of plantation and badlands in pluvial fan and alluvial plain. The key factor was the dynamic variation of water-salt in water and soil resource utilization. Terrain and land utilization were the key factors affecting water table, and the continuous changes of the water table worked on the spatial distribution of soil water-salt.

The distributions of particulate heavy metals and its indication to the transfer of sediments in the Changjiang Estuary and Hangzhou Bay, China.

Particulate heavy metals (Cu, Pb, Cd) were measured following intensive sampling in June and October 1994 at 70 stations in the Changjiang Estuary and Hangzhou Bay, China. In the study area, particulate Cu and Pb have a similar level that is higher than the concentration of particulate Cd. Cu, Pb and Cd concentrations in suspended sediments are higher than those in bed load. In the middle of Hangzhou Bay, heavy metal concentrations in suspended sediments and bed load are similar. This may be the result of the frequent exchange between them, which is due to the strong dynamic condition. A negative relationship was observed between concentrations of heavy metals in bed load and bulk density. Fine-grained sediments were the main carriers of heavy metals. Sedimentary dynamics dominate the fate of heavy metals in these sites. The distributions of metals (especially Cu) in suspended sediments can indicate the transfer of sediments in this area.

Detection of segmental internal fat by bioelectrical impedance analysis in a biological phantom.

OBJECTIVE: Quantification of internal adipose tissue such as visceral adipose tissue currently relies on expensive, cross-sectional imaging modalities. The purpose of this study was to test the hypothesis that surface impedance, determined by bioimpedance analysis, might be used to predict regional internal fat content change in a phantom model. METHODS: Fresh hollowed-out cucumbers were used as cylindrical biological phantoms to test this hypothesis. After removal of the seeds, the cucumbers were filled with normal saline, mixture of saline and corn oil, or porcine adipose tissue bathed in saline. Surface resistance and reactance were measured with a bioimpedance analyzer accurate to 0.1 Omega (Quantum 10X, RJL Systems), and impedance was calculated. A linear regression model was used to interpret the association between composition and impedance. RESULTS: Surface impedance varied linearly with changes in the relative internal corn oil portions (r- = 0.98). A similar relation was noted with porcine adipose tissue bathed in saline (r(2) = 0.95) regardless of the specific position of adipose tissue within the cucumber. CONCLUSION: Surface impedance measured by bioimpedance analysis can detect variations in fat content in the interior of a cylindrical phantom.

Studies of adipose tissue metabolism in human immunodeficiency virus-associated lipodystrophy.

We studied aspects of metabolism in subcutaneous adipose tissue (SAT) in 40 human immunodeficiency virus (HIV)-infected subjects with and without lipodystrophy and in healthy control subjects. HIV-infected subjects without lipodystrophy had less SAT and visceral adipose tissue (VAT). Glycerol release was higher in both HIV-infected groups, especially those without fat redistribution. Tumor necrosis factor (TNF) release from SAT and serum soluble TNF receptor 2 concentrations were significantly higher in HIV-infected individuals with lipodystrophy. The absolute production of acylation-stimulating protein (ASP) and the percentage conversion of the complement protein to ASP were significantly lower in HIV-infected subjects with lipodystrophy. Further studies are needed to dissect the factors that mediate lipoatrophy in HIV infection.

Total body potassium differs by sex and race across the adult age span.

BACKGROUND: Total body potassium (TBK) is an index of fat-free mass. Data describing changes in TBK in African American, Asian, or Hispanic populations have not been reported. OBJECTIVE: The aim was to investigate possible sex and racial differences in TBK in adults over an age range of 70 y. DESIGN: The study used longitudinal and cross-sectional data collected in a body-composition unit from 973 men and 1368 women of African American, Asian, white, and Hispanic race-ethnicity. Random coefficient models in which baseline weight and height were taken into account were applied to estimate sex-specific changes in TBK among the 4 racial-ethnic groups. RESULTS: The ages of 30 and 31 y were identified for women and men, respectively, as the cutoffs after which TBK began to decline. Both sexes had similar racial-ethnic patterns for expected mean TBK at the age cutoffs: African Americans had the highest value, followed by whites, Hispanics, and Asians. After the age cutoffs, the decline in TBK differed by race and sex. In women, African Americans showed the most rapid decline, whereas Asians had the lowest. In men, Hispanics had the most rapid decline in TBK, followed by African Americans, whites, and Asians. CONCLUSION: Significant sex and racial differences exist in the rate of change in TBK with age. Further studies are needed to explore the associations of declining TBK with health risks.

Embryonic stem cells: new possible therapy for degenerative diseases that affect elderly people.

The capacity of embryonic stem (ES) cells for virtually unlimited self renewal and differentiation has opened up the prospect of widespread applications in biomedical research and regenerative medicine. The use of these cells would overcome the problems of donor tissue shortage and implant rejection, if the cells are made immunocompatible with the recipient. Since the derivation in 1998 of human ES cell lines from preimplantation embryos, considerable research is centered on their biology, on how differentiation can be encouraged toward particular cell lineages, and also on the means to enrich and purify derivative cell types. In addition, ES cells may be used as an in vitro system not only to study cell differentiation but also to evaluate the effects of new drugs and the identification of genes as potential therapeutic targets. This review will summarize what is known about animal and human ES cells with particular emphasis on their application in four animal models of human diseases. Present studies of mouse ES cell transplantation reveal encouraging results but also technical barriers that have to be overcome before clinical trials can be considered.

Growth in early life and its relation to pubertal growth.

BACKGROUND: The timing and magnitude of pubertal growth in relation to fetal, infancy and childhood growth have not been explored. METHODS: We used data from a longitudinal growth study of 3650 full-term healthy children who were born in Gothenburg, Sweden in 1973-1975. This analysis included 2738 children with height and weight measurements available both in early life and during adolescence. RESULTS: We found that faster linear growth during infancy and childhood was associated with earlier peak height velocity during adolescence. In contrast, greater height and body mass index (BMI) at birth were associated with later peak height velocity in adolescence. Children with faster linear growth and greater BMI in infancy and childhood had less height gain between ages 8 and 18. However, greater height and BMI at birth were associated with more height gain between ages 8 and 18 after adjusting for height and BMI in childhood. CONCLUSIONS: Both length and BMI (at birth, in infancy and during childhood) are associated with the timing and magnitude of pubertal growth. Being small at birth is associated with early puberty and a reduced height gain during adolescence.

Preferential loss of omental-mesenteric fat during growth hormone therapy of HIV-associated lipodystrophy.

Lipodystrophy with increased intra-abdominal fat in human immunodeficiency virus (HIV) infection is common in the era of highly active antiretroviral therapy. It contributes to the metabolic derangements, as it does in non-HIV-related conditions. Growth hormone administration reduces intra-abdominal fat content. This study compared the relative changes in omental-mesenteric (OMAT) and retroperitoneal adipose tissues (RPAT) during therapy with recombinant human growth hormone (rhGH) in HIV-associated lipodystrophy. Of 30 subjects who began rhGH therapy (6 mg/day), 25 completed 12 wk and 19 completed 24 wk. Fourteen subjects were followed for an additional 12 wk. Volumes of OMAT and RPAT were calculated from total body MRI scans and compared by paired t-tests. Both OMAT and RPAT significantly decreased after 12 and 24 wk of rhGH treatment (P < 0.001), but the reduction was more pronounced in OMAT than in RPAT (P < 0.001). Both OMAT and RPAT increased significantly (P < 0.001) after therapy was discontinued, but OMAT increased significantly more than did RPAT (122 vs. 37%, P < 0.001). There is preferential loss and regain of OMAT, compared with RPAT, in subjects with HIV-associated lipodystrophy undergoing growth hormone treatment.

[Early indexes to predict the therapeutic effect of interferon on chronic hepatitis B].

OBJECTIVE: To summarize the clinical changing characters of the clinical markers after interferon treatment in chronic hepatitis B (CHB) and make out practical indexes to predict the effect. METHODS: 150 CHB patients were randomly divided into two groups: therapeutic group (90) and control group (60) in the prospective controlled trial. The levels of endogenous interferon before treatment, interferon antibody at the end of the second month and fourth month after treatment, alanine aminotransferase (ALT) and HBV DNA in the serum were detected. Then the data was analysed to find out indexes for predicting the effect. RESULTS: (1) The clearance rate of HBeAg had no significant difference in age except for 20 - 30 and 30 - 40 (t > 2.331 2, P < 0.01). (2) It was more effective if ALT level was higher than 400 U/L before treatment and it decreased more than 50% two months after treatment. (3) The patients whose HBV DNA was negative (dot hybridization) or less than 10(6) copies/ml before treatment had higher rate of HBeAg clearance. (4) There was no effect on patients whose interferon antibody turned positive at the end of the second month. (5)A predictive method of comprehensive factors was made out, whose sensitivity, specificity, and accuracy were 80%, 100% and 90%, respectively. CONCLUSION: The clinical characters of these Chinese patients are different from those of the westerners and the effects of interferon have close relation to the levels of ALT, HBV DNA and interferon antibody.

[Random control trial of the efficacy of cardio pump on pre-hospital cardiac arrest].

OBJECTIVE: To compare the efficacy of early resuscitation of active compression-decompression cardiopulmonary resuscitation (ACD-CPR) with standard CPR in pre-hospital cardiac arrest. METHODS: Ninety-two patients with cardiac arrest occurring out of hospital were allocated to ACD-CPR (cardio pump) or standard CPR randomly, then determine systolic blood pressure (SBP) of both groups at the 1st, 3rd, 5th, 10th, 15th, 30th minute after the beginning of resuscitation, and the proportion of resumption of spontaneous circulation (ROSC), as well as admission rate. RESULTS: The SBP and ROSC at 3rd, 5th, 10th, 15th minute were significantly higher in the group receiving ACD-CPR than in the group receiving standard CPR. On arriving the hospital, the survival rate was higher in the ACD-CPR group than in the control group, but of no statistical significance. CONCLUSION: ACD-CPR may improve the efficacy of early resuscitation in patients with pre-hospital cardiac arrest.

Trunk fat and blood pressure in children through puberty.

BACKGROUND: Fat distribution is well recognized as a cardiovascular risk factor in adults. The association between android fat distribution and cardiovascular risk factors, such as blood pressure (BP), was previously reported in an African-American and Caucasian pediatric population. The aim of the present study was to investigate the relationship between BP and body fat distribution in a large cross-sectional pediatric sample. The effects of race, sex, and puberty on this relationship were assessed. METHODS AND RESULTS: BP was measured in 920 healthy children and adolescents (African-American, Asian, and Caucasian, ages 5 to 18 years). Fat distribution was determined by skinfold thickness and dual-energy X-ray absorptiometry (DXA). Pubertal status was assessed by the criteria of Tanner. Regression analysis was used to explore the association between BP and fat distribution. Significant positive relationships between systolic and diastolic BP and trunk fat adjusted for total fat were seen in boys at all pubertal stages in all 3 races by both DXA and skinfold measurements. In girls, trunk fat was not a significant predictor of BP. CONCLUSIONS: Our results demonstrate a sex difference in the relationship between BP and trunk fat in that a significant positive relationship was present in boys only. These findings, based on 2 independent measures of fat distribution, may help identify the specific features of individuals at risk, allow earlier intervention, and suggest sex-specific determinants for BP.

Probability of adult overweight and risk change during the BMI rebound period.

OBJECTIVE: To develop a probability chart of adult overweight based on childhood body mass index (BMI) values and to evaluate the BMI change during the BMI rebound period during childhood, in different populations, with the use of risk function curves. RESEARCH METHODS AND PROCEDURES: A longitudinal growth study of 3650 full-term healthy Swedish children followed from birth to 18 years of age. Weight and height values of our subjects were obtained. RESULTS: A probability chart for reaching a BMI > 23 kg/m(2) at 18 years of age was constructed for boys and girls. For example, a BMI of 18 kg/m(2) at 4 years of age is associated with 0.70 probability of attaining a BMI > 23 kg/m(2) at 18 years of age in boys; a BMI of 16 kg/m(2) at 4 years of age leads to 0.40 probability of having a BMI > 23 kg/m(2) at 18 years of age in girls. Children with an obvious BMI rebound before 8 years of age have a high risk of being overweight at 18 years of age. There is a clear trend of BMI increase from the 1970s to the 1990s in U.S. children from a parallel dataset, and Hispanic children are at the highest risk of adult overweight. DISCUSSION: The probability chart for adult overweight developed here provides a functional method of defining childhood obesity that is based on the risk of long-term ill health rather than on a certain statistical cut-off point. It will help pediatricians or healthcare workers identify those children who are at a high risk of becoming overweight in adulthood, which will allow clinical intervention at younger ages.

Biodegradable calcium phosphate nanoparticles as a new vehicle for delivery of a potential ocular hypotensive agent.

The purpose of this study was to determine the efficacy of a newly prepared formulation containing biodegradable calcium phosphate nanoparticles (CAP) and 7-hydroxy-2-dipropyl-aminotetralin (7-OH-DPAT) in pigmented and non-pigmented rabbits using the surrogate end points of intraocular pressure (IOP) and aqueous flow rate. IOP (mmHg) was measured by utilizing a manometrically calibrated Mentor pneumatonometer. Rates of aqueous humor flow were measured with a Fluorotron Master by estimating the dilution rate of fluorescein. In non-pigmented rabbits, the ocular hypotension induced by topical administration of 7-OH-DPAT (75 microg) with CAP (115 microg) was more pronounced and sustained than that of 7-OH-DPAT without CAP. Furthermore, IOP-lowering effects of topically administered 7-OH-DPAT (125 microg) alone were markedly diminished in pigmented rabbits compared to non-pigmented rabbits. However, topical application of 7-OH-DPAT formulated with CAP produced significant dose-related (37.5, 75, 125 microg) reductions of IOP accompanied by suppression of aqueous humor flow rates in pigmented rabbits. It is postulated that 7-OH-DPAT in vehicle without CAP binds to pigments in the anterior segment of the pigmented rabbit's eyes, and this binding limits the 7-OH-DPAT's action. Pretreatment with raclopride, a dopamine D2/D3 receptor antagonist, reduced the ocular hypotensive effect induced by 7-OH-DPAT in vehicle containing CAP thereby supporting the role for dopamine D2/D3 receptors in modulating IOP. It is concluded that CAP, as a delivery system, enhances activity by 7-OH-DPAT in pigmented rabbit eyes suggesting that CAP is potentially useful for achieving controlled and targeted drug delivery for treatment of ocular diseases.
