Leprosy in Louisiana, 1855-1970. An epidemiologic study of long-term trends.

Study of a well known focus of leprosy in Louisiana in 1855-1970 showed a decrease in incidence which began before the use of chemotherapy. Many of the cases had onset in a limited area of Louisiana, French Louisiana, where there was a high incidence of multiple family cases. Overall incidence rates were found to be decreasing for each cohort of birth, and within cohorts after 1920, the highest incidence rates were in the age group 10-19. A high incidence rate was found for cases with onset in the years 1900-1929; rather than an epidemic, this was probably related to bias in the recording of year of onset. Although persistent for a long period, the focus remained limited in geographic extent and seems to be disappearing.

Undiagnosed bacterial meningitis in Vermont children.

Community-acquired bacterial meningitis in Vermont children under 5 years of age was recognized less frequently in 1967-1970 in those towns with low total hospitalization rates than in towns with hospitalization rates above 15 admissions per 100 population. Using the towns with high hospitalization rates as a norm, it was found that towns with fewer recognized meningitis cases than expected had significantly greater rates of death from obscure causes in children 1-59 months of age in 1967-1970. It is suggested that about 17 deaths in 1967-1970 in Vermont children 1-59 months of age were associated with the failure to recognize bacterial meningitis in children from towns with low rates of medical care utilization.

Human-to-human transmission of Pseudomonas pseudomallei.

Melioidosis, the clinical manifestation of infection with Pseudomonas pseudomallei, has occurred infrequently in American citizens; almost all reported cases have been in Vietnam veterans, usually associated with respiratory disease. A Vietnam veteran from Mississippi developed chronic prostatitis, with no other clinical manifestations, during service in Vietnam, and P. pseudomallei was isolated from prostatic secretions 2 years after his return to the United States. The patient had had sexual contact with four women including his wife since his return from Vietnam. Vaginal and cervical cultures and serum samples were obtained from the four women, and serum samples and cultures of semen were obtained from the patient. Vaginal swabs and semen cultures were negative for P. pseudomallei. The patient and his wife had hemagglutination titers (greater than 640) diagnostic of P. pseudominallei infection. This occurrence of venereal transmission is the first report of person-to-person spread of P. pseudomallei infection.

Herpes simplex virus encephalitis simulating a frontoparietal convexity neoplasm.

The unusual occurrence of a single, cell circumscribed encephalitis due to herpes simplex virus in the frontoparietal convexity is described and served to underline the importance of considering this pathology in the event of a rapidly expanding lesion presenting in any portion of the brain. The valuable, but limited, usefulness of brain scanning and cerebral angiography in making this diagnosis is discussed. Neurosurgical intervention and brain biopsy are required to make the definitive diagnosis.
