[Benign paroxysmal positional vertigo of the horizontal canal: A multicenter study].

OBJECTIVE: BPPV localized in the horizontal semicircular canal is an infrequent entity. Nowadays there are controversies about the different treatments available. The objective of this study is to present our results. MATERIAL AND METHODS: A multicenter and retrospective study was performed in 31 patients diagnosed of BPPV-HSC between January 1996 and May 2004. RESULTS: cupulolithiasis was diagnosed in 48% on the patients. Symptoms disappeared before signs (p<0.05). Global cure rate was 85%, while relapses were 16% at one year. No relations were found between cure rate and relapses and age, gender, duration of symptoms, canalithiasis and cupulolithiasis. CONCLUSION: Our results support that there are not differences between the treatments performed in the BPPV-HSC. Symptoms disappeared before signs when canalith repositioning particles (CRP) maneuver was performed.

[Infantile facial paralysis: diagnostic and therapeutic features].

This paper deals with a series of 11 cases of peripheral unilateral facial paralyses affecting children under 15 years. Following parameters are reviewed: age, sex, side immobilized, origin, morbid antecedents, clinical and neurophysiological explorations (electroneurography through magnetic stimulation) and the evolutive course of the cases. These items are assembled in 3 sketches in the article. Clinical assessment of face movility is more difficult as the patient is younger, nevertheless electroneurography was possible in the whole group. Clinical restoration was complete, excepting one complicated cholesteatomatous patient. Some aspects concerning the etiology, diagnostic explorations and management of each pediatric case are discussed.

[Progression of profound perceptive deafness in children].

We have studied 340 cases of deep neurisensorial deafness in children and their evolution along a 7.2 years term. Conventional audiometry has been used as commonest method for assessing the auditive function. In a considerable number of cases (15.9%) hearing loss was progressive and some relation referring to sex, the bearing of hearing aids and the etiology is statistically contemplated.

[The blink reflex in the electrophysiologic exploration of Bell's palsy and its prognostic value].

Conventional electrophysiology of the facial nerve gives only information about the functional condition of its extracranial segment. The blink-reflex triggered for the electric stimulation of supraorbital nerve allows the study of the whole nerve path, even the intraosteal, where the great majority of its pathology settles. We have realized this tests in 43 Bell's palsy cases (at the beginning of the paralysis and three weeks later) and correlated the results with the clinic course, in order to set up the pronostic value of the procedure. The cases with shorter tendency of blink reflex along the three first week have a better outcome.

[Tobacco addiction and current treatment methods].

Smoking currently is considered to be an addiction with severe medical and social effects. It has been implicated in some respiratory and extra-respiratory diseases. In spite of knowing its dangerous effects, many patients cannot stop smoking without outside help. This help usually is provided by health-care personnel. It is necessary to be familiar with disintoxication methods in order to be able to advise patients on the use of any one or a combination of them. The most efficient method is individual medical help. Nicotine substitutes have demonstrated their effectiveness. Other complementary methods are not considered effective unless combined.

[Secretory middle ear otitis with severe sensorineural deafness].

Secretory middle ear otitis is of difficult assessment in children with severe hearing impairment. This otitis, very frequent in infants and children, influences negatively the auditive capacity and apprenticeship as well. Our study deals with the prevalence and severity of the secretive middle ear otitis in an scholar population handicapped by a heavy hypoacusia. Demographically, etiologic and seasonal correlations are considered in the paper. The outcome shows a high incidence of the condition, an inversely relation with the age, an evident seasonal distribution and the absence of correlation between etiology of middle ear disease and sensorineural deafness.

Hypoglossal nerve conduction study by transcranial magnetic stimulation in normal subjects.

The introduction of transcranial magnetic stimulation has allowed the study of conduction in the proximal portions and central pathways of the cranial nerves. A study is made of cranial nerve XII with transcranial magnetic stimulation at two levels, cortical and cisternal, registering the motor evoked potential by means of surface electrodes in contact with the upper face of the tongue. Motor evoked potentials were constantly observed on cortical stimulation, in a painless, easy, and reproducible way, with mean values of 10.84 +/- 1.14 milliseconds (latency) and 7.81 +/- 1.14 mV (amplitude). Motor evoked potentials were unconstant and showed reduced amplitudes on cisternal stimulation, with mean values of 4.72 +/- 0.62 milliseconds and 0.83 +/- 1.26 mV. The magnetic stimulation technique allows the study of the entire motor pathway of cranial nerve XII (motor cortex-medulla, motoneuron-muscle). The method is efficient, noninvasive, painless, and easily reproduced, and it comes close to being an ideal clinical conduction study technique for this cranial nerve.

[Oncogenic activity of head and neck tumors].

Some types of malignant neoplasms are consequence of genetic changes. The starting of oncogenesis begins when some genetic alterations arises, or the genome acquires some peculiar and pathological traits owing to the action of physical, chemical or viral agents. These alterations set in motion an activatory phenomenon upon the proto-oncogene which is turned into an oncogene. The oncogene is responsible for fixed alterations of the genome inducing to an anomalous cellular growth. In squamous carcinomata of the head and neck has been observed mutations of the p-53 tumour-suppressor gene, alike amplifications of the ras-gene, of the C-myc proto-oncogene and also of the epidermal growth factor gene (EGFG). The viruses induce mutations on the genome. That would be the explanatory reason for the association carcinoma of the rhinopharynx-Epstein-Barr virus.

Superior laryngeal nerve evoked potentials: an experimental study in the rabbit.

An experimental model is described for recording laryngeal evoked potentials at the brainstem level through electrical stimulation of the superior laryngeal nerve. Rabbits were divided into two groups according to age: Group 1 consisted of 20 adults (over 6 months old), while group 2 was comprised of 1-month-old rabbits. Under general anesthesia, the superior laryngeal nerve was electrically stimulated and the corresponding evoked potentials were recorded by needle electrodes placed close to the dorsal and ventral aspects of the brain stem. Clear age-related differences were observed for latency, amplitude, and morphology of the potentials. This fact suggests postnatal maturation of the nerve structures, both peripheral and central. Sectioning the superior laryngeal nerve abolished all responses. Adduction of both vocal cords was observed in conjunction with the use of stimulation.

Immotile cilia syndrome: nasal mucociliary function and nasal ciliary abnormalities.

We present 17 patients with a typical symptomatology of immotile cilia syndrome, seven of them with complete situs inversus. Firstly, a study of the nasal mucociliary transport was made by means of the radioisotopic technique with serum albumin-Tc99m. In all cases there was absence of transport. Secondly, we studied the ultrastructure of the nasal cilia. Defects in the dynein arms were frequently found (65%). In two cases (11%) there were no cilia; in two other cases the cilia were normal, and in another two cases alterations of the central pair of microtubules were seen. Ciliary complexes were detected in all cases. We conclude that in patients with chronic or recurrent infections of the airways without known cause we must initiate a study of the nasal mucociliary transport. If this is absent or decreased, study of ciliary ultrastructure should be carried out. If mucociliary transport is normal, immotile cilia syndrome is ruled out and ultrastructural study of the cilia is not required.

[The protocol for the vestibulospinal system study by means of a dynamometric platform. Patterns of normalcy].

The Posturography, the recording of the postural balance in standing position, allows the assessment of the vestibulospinal function, through the shifts of the pressure center. In this paper are expressed the results gained with this static posturographic protocol followed at our Department, using a dynamometric platform DINASCAN (Institute of Biomechanics, Valencia, IBv) in 63 normal people. Four parameters (i.e. maximal extent of antero-posterior displacement: lateral wards; full length of the shifting; and the surfaced limited for the changes of the pressure center) in 6 several body positions (Romberg with open and closed eyes; closed eyes and overextended head; closed eyes and right and left cervical turn and standing on only one foot with open eyes) were evaluated. The outcome of the proofs become gradually worsened in accordance with the difficulties of the tests, although there were no differences when considering the results of the proofs with turning or hyperextending the head. Diagnostic possibilities trying dynamometric platforms are considered.

Laryngeal mucociliary transport: normal values and under the influence of anesthetics. An experimental study in rabbits.

The larynx is the point of confluence of all tracheobronchial secretions and of the particles trapped within them. A study is made of laryngeal mucociliary transport using charcoal particles in 8 anesthetized rabbits and in 8 more during the immediate postmortem period. Most secretions were found to reach the posterior commissure, and were then swallowed. In the immediate postmortem rabbits, mucociliary transport velocity was 3.59 +/- 1.23 mm/min., with a transport time of 4.62 +/- 1.5 min. In the 8 rabbits anesthetized with droperidol, diazepam and thiobarbital, transport was found to be significantly slowed (p = 0.001).

[The influence of age, sex and circadian rhythms on the nasal mucosal in the mucociliary clearance].

We have studied the nasal mucociliary transport in 115 normal persons between 5 and 85-years-old. Using vegetable charcoal powder method the transport time varies between 5 and 18 minutes. The mean transport time was 8.35 +/- 2.6 min. Transport time is not variable with age until sixty years, when transport slow down. The mucociliary clearance is faster in female (p = 0.018). In these individuals the transport time is less in the evening-hours (p = 0.01). In men no differences has been noted (p = 0.57).

Accessory nerve electroneurography by magnetic stimulation in normal subjects.

An electroneurographic study was made of the accessory nerve in 15 healthy subjects by conventional peripheral electrical stimulation and transcranial magnetic stimulation at three different levels: cortical contralateral (frontoparietal), homolateral occipital and at the base of the skull. The motor evoked potential (MEP) obtained by magnetic stimulation at the skull base exhibited greater latency and amplitude than that recorded by conventional peripheral electrical stimulation. The MEP obtained by stimulating at occipital level again exhibited a greater latency than that corresponding to skull base stimulation, but the amplitude was smaller. Finally, the MEP resulting from cortical stimulation failed to appear uniformly in all of the individuals studied. Its amplitude was smaller than in the previous cases, with a mean latency of 11.76 ms and a central conduction time of 7.5 ms. The magnetic stimulation technique allowed the efficient, painless, non-invasive and easily reproducible study of the motor pathway of the spinal nerve over its entire trajectory (motor cortex-motor neuron-muscle).

[Kniest syndrome. An audiological study].

In this syndrome there is a disproportionate dwarfism with kyphoscoliosis, stiffness of the joints, minor facial dysmorphia, myopia and both conductive and sensorineural deafness in 50 percent of patients. It is a dominant autosomic heritable malady of unknown incidence, but both sexes seems equally affected. The patient's condition is diagnosed at birth, because of the short limbs and the enlargement of the joints. In the presented case, diagnosed at birth, the deafness was suppose to be transmissive, owing to the coexistence of a cleft palate, and consequently a grommet was inserted in the tympanum. Only afterwards, when the girl was 12 years old, the cochlear component of the hypoacusis fell in consideration.

[Granular cell tumor (Abrikossoff's tumor). A review and our experience].

The low incidence of these tumors compelled the AA., after diagnosis of one of laryngeal sitting, to a review of the theme and its literature as well, particularly under the viewpoint of its histogenesis, which has been debatable up to lately. The report is complemented with the study of immunohistochemical tests for NHK, S-100 and specific neural enolase of the specimen.
