Omega-pyridiniumalkylethers of steroidal phenols: new compounds with potent antibacterial and antiproliferative activities.

Novel omega-pyridiniumalkylethers of two steroidal phenols were synthesized as compounds with potential antimicrobial activity. 3-Hydroxy-estra-1,3,5(10)-triene-17-one and 1-hydroxy-4-methyl-estra-1,3,5(10)-triene-17-one were reacted with omega,omega'-dibromoalkanes to omega-bromoalkoxy-estra-1,3,5(10)-trienes followed by reaction with pyridine to obtain the desired steroidal omega-pyridiniumalkoxy compounds as bromides. Their antimicrobial activity against strains of multiresistant Staphylococcus aureus (MRSA), a vancomycin resistant Enterococcus faecalis and fast growing mycobacteria depends clearly on the length of the alkyl chain. A strong broadband activity has been found for the compounds with eight or 10 C-atoms; in some cases better than ciprofloxacin or cetylpyridinium salts. In addition, the antiproliferative and cytotoxic activity depends on the chain length, too. The differentiation between antibacterial and cytotoxic activity is better for the steroid hybrid molecules than the cetylpyridinium salts. These new compounds can serve as lead compounds for further optimization.

Possible hemodynamic pathways of intraurethral prostaglandin-E1 (MUSE).

Intraurethral application of prostaglandin-E1 (MUSE) is a well-tolerated pharmacotherapy for erectile dysfunction. However, the physiological mechanisms of drug transfer into the cavernous bodies are not completely clear. Using spongiosography in 35 patients, our study tried to elucidate existing shunt mechanisms. The X-rays show venous drainage through the deep dorsal vein up to the plexus Santorini. The circumflex veins are also contrasted and the cavernous bodies show opacification in their distal portion only. Structures shunting directly between the corpus spongiosum and the cavernous bodies were not demonstrable. Retrograde filling of the cavernous bodies through the deep dorsal vein and its circumflex braches seems to be the most relevant way of drug transfer after intraurethral application of prostaglandin-E1. Diffusion into the cavernous bodies or a systemic mechanism of action does not seem probable.

[Intravesical migration of a polypropylene mesh implant 3 years after laparoscopic transperitoneal hernioplasty].

The repair of an inguinal hernia is the surgical procedure most often performed. Complication rates after laparoscopic hernioplasty amount up to 19%, with hematoma/seroma, neuralgia, urinary retention, and chronic pain most frequently reported. Significant complications such as trocar site bleeding or bowel injury occur in 0.4-5.6%, and sporadic intraoperative lesions of the bladder have been mentioned. We present a 48-year-old patient with recurrent dysuria 3 years after transabdominal preperitoneal hernioplasty (TAPP). The preoperative diagnostic evaluation led to the assumption of an intravesical mesh dislocation. In spite of extensive adhesions between the mesh and the bladder wall, the mesh including five fixation coils could be removed via a suprapubic access. The postoperative period was without complications, and the patient has no complaints. The incidence of complications after laparoscopic hernioplasty is low. Still, severe problems such as mesh rejection, spermatic granuloma, or mesh migration into the small and large intestine do occur. Migration of a mesh into the urinary bladder has only been described twice.

Antimycobacterial activity of some 2,3-dianilinoquinoxaline derivatives.

The antimycobacterial activity of 2,3-dianilinoquinoxaline derivatives was studied and the substances were found to be mostly wide-spectrum antimycobacterial agents. Replacement of one nitrogen atom in the heterocycle for oxygen or sulfur was usually accompanied by loss of activity. The model structure for the study was the wide-spectrum antimycobacterial drug clofazimine.

[Modified retroperitoneal lymphadenectomy in non-seminomatous stage I, IIa and IIb testicular tumors].

In 60 patients with non-seminomatous testicular germ cell tumours in clinical stages I, IIa and IIb inguinal orchiectomy was performed as primary treatment, followed by modified retroperitoneal lymphadenectomy (RLA). In stage II disease chemotherapy was given in addition. After a follow-up period of at least 24 months only 5% of the patients showed progression of disease. Normal ejaculatory function was preserved in 88% of our patients. Modified RLA seems to be a safe procedure with an acceptably low rate of complications not only in stage I disease, but also in metastatic stages IIa and IIb.

Phosphorylation of the L-type calcium channel beta subunit is involved in beta-adrenergic signal transduction in canine myocardium.

Cyclic AMP-mediated phosphorylation of calcium channel subunits was studied in vitro and in vivo in preparations from dog heart. Calcium channels in native cardiac membranes were phosphorylated by cAMP-dependent protein kinase (PKA) solubilized with digitonin and subsequently immunoprecipitated using a polyclonal antibody generated against the deduced carboxy-terminal sequence of the cardiac beta subunit. A 62 kDa protein was identified as the major PKA-substrate in the immunoprecipitates. In the intact myocardium, this putative beta subunit was found to be phosphorylated in response to cAMP elevating agents. In contrast, no phosphorylation of a protein with an electrophoretic mobility similar to the alpha 1 subunit was detected, although 1,4-dihydropyridine receptor sites were recovered in the immunoprecipitates. Thus, we suggest that PKA-mediated phosphorylation of the beta subunit is the major mechanism for beta-adrenergic regulation of cardiac L-type calcium channel activity.

Monoclonal antibodies that recognize trypsin binding domain of human urinary trypsin inhibitor.

Three MAb M2, B6, P1 (IgG1 type) against human urinary trypsin inhibitor (UTI), a glycoprotein with antiinflammatory properties, have been produced by hybridization of mouse myeloma cells P3o1 with spleen cells of immunized mice BALB/c. Competitive ELISA-examination of peroxidase conjugates of M2, B6, and P1 MAb in the presence of the trypsin binding domain shows the highest affinity of M2 antibody for this domain. On the basis of MAb M2 competitive ELISA of UTI concentration in urine is proposed. ELISA detectable changes in the UTI content of urine from patients with nephritis without renal failure can be considered as an early index of renal parenchyma damage.

Venous surgery in erectile dysfunction. The role of dorsal-penile-vein ligation and spongiosolysis for impotence.

We report here on our surgical experience with venous leakage of the cavernous bodies. Out of 159 patients operated on, 134 were available for long-term follow-up. Depending on the cavernosographic findings, one of three different surgical procedures was carried out: ligation of the deep dorsal vein of the penis, spongiosolysis, or ligation of the crura. 18% of the patients undergoing ligation of the deep dorsal vein, postoperatively attained spontaneous erections, while 35% needed adjuvant corpus cavernosum autoinjection therapy. Spongiosolysis gave a more favourable result: spontaneous erections in 30% and vasoactive drug-dependent erections in 35%. Crural ligation did not prove successful. No serious complications were encountered postoperatively. Our data suggest that venous surgery should only be offered to a selected group of patients comprising young impotent men with venous leakage, maybe in combination with arterial disease, and patients suffering from distal venous leakage. Old age, neurogenic disorders causing erectile dysfunction, and diabetes mellitus should represent exclusion criteria for venous surgery.

Spongiosography: a valuable adjunct to the diagnosis of urethral strictures.

To obtain better information on fibrotic tissue damage in urethral strictures we performed spongiosography, that is opacification of the spongy body by injection of contrast medium, in 32 patients in whom the diagnosis had already been established by uroflowmetry, urethrography and urethroscopy. Another 18 patients with erectile dysfunction served as the control group. The investigation was well tolerated by all patients and no side effects or complications were encountered. Attenuation or even interruption of the contrast medium within the corpus spongiosum on spongiosography clearly revealed the fibrosis in urethral strictures. The extent of scarring was completely independent of the clinical symptoms and urethrographic findings. Accordingly, our decision on the appropriate surgical treatment was based mainly on the results of spongiosography. If used as an additional routine investigation in cases of urethral stricture spongiosography may help to decrease the recurrence rate in the future.

Nd:YAG coagulation of the verumontanum: an endoscopic attempt at sterilization.

In order to develop a new and less invasive method of sterilization in men we performed photocoagulation of the verumontanum in 10 rams, using a Nd:YAG laser, wavelength 1,060 nm. Under general anesthesia a perineal urethro-cutaneous fistula was achieved, and, by means of a 16F laser-endoscope, transurethral coagulation of the ejaculatory ducts, located in the verumontanum, was carried out, using pulses of 1.5 sec in duration and 20-30 W in power. The animals were killed at different postoperative time intervals, ranging from 1 day to 8 weeks. Injection of methylene-blue into the testicular ducts of the resected prostate-bladder specimen and histologic studies of the coagulated tissue revealed that complete bilateral occlusion of the ejaculatory ducts had been achieved in 4 rams. Failures were due to over-extended irradiation of the verumontanum. Except for urinary tract infections occurring in 4 animals, no serious complications were encountered. Further investigations are still necessary to determine the role of lasers in male sterilization.

Combination therapy for penile giant Buschke-Löwenstein condyloma.

The case of a 25-year-old patient who underwent circumcision and Neodymium-YAG laser treatment for giant condylomata acuminata of the penis is presented. Two cycles of interferon therapy were administered postoperatively, each comprising beta-interferons and recombinant gamma-interferons. This combination therapy provided an excellent cosmetic and functional result and the patient remains disease-free 15 months following surgery.

Laser-assisted vasovasostomy.

To find a less time-consuming alternative to conventional microsurgical vasovasostomy, we performed vasal anastomosis in four sterilized men by means of a Nd:YAG laser (wavelength 1,060 nm) under macroscopic conditions. After dissection of the vasal ends, the ends were approximated using 6-0 prolene sutures as intervasal stents. The apposed tissue edges were welded by laser pulses of 0.5 sec in duration and 10 W in power. Postoperative sperm counts proved normal in two patients; one of them reported a successful pregnancy in his wife 12 months after vasovasostomy. One patient had cryptospermia and one patient oligoasthenospermia. Laser-assisted vasovasostomy is a simple procedure and shortens the operative time. More patients still need to be studied to allow a favorable comparison with conventional suture techniques.

Effects of intracavernosal pharmacotherapy on self-esteem, performance anxiety and partnership in patients with chronic erectile dysfunction.

156 patients undergoing intracavernosal autoinjection pharmacotherapy for periods of 2-36 months were studied for the effect of treatment on self-esteem, performance anxiety, partnership and acceptance using a semi-structured questionnaire. Replies were received from 86.3% of the patients. 93% gave a very good, good or satisfactory rating of intracavernosal pharmacotherapy. 78.2% indicated that their self-esteem had grown. 79.4% said that performance anxiety diminished in the course of therapy (25.6%) or disappeared entirely (53.8%). 71.8% of the replies confirm the positive effect of treatment on marital relations or partnership. 61.9% of the patients used intracavernosal pharmacotherapy permanently. 28.3% needed therapy intermittently or at large intervals only, and 4.5% (7 patients) were cured of their chronic sexual dysfunction. When preceded by thorough multidisciplinary diagnostics and careful patient selection, intracavernosal pharmacotherapy constitutes an efficient mode of treatment for chronic erectile dysfunction.

[The application of peroxyoxalate-chemiluminescence in biochemical analysis: determination of oxalate].

A new sensitive method for the determination of oxalate is presented. By using the chemiluminescence decay of monoperoxyoxalic acid very low concentrations of oxalate can be determined (up to 100 nmol/l). Oxalate determinations in urine require the precipitation of calcium oxalate, in order to avoid oxalate-unspecific side emissions. This could be avoided by combination with a specific enzymatic partial reaction, which would also permit the determination of low concentrations of other organic species (e.g. citrate, pyruvate, malate).

[Diagnosis of enuresis--results of urodynamic studies].

The results of clinical evaluation of 71 patients with primary or secondary enuresis are presented. In 66 patients a complete urodynamic assessment was possible. 35 percent of these patients showed a normal urodynamic pattern, in 45 percent detrusor hyperreflexia and in 18 percent a combination of detrusor hyperreflexia and a functional infravesical obstruction was demonstrated.

[Spongious autoinjection therapy following implantation of a penis prosthesis--case report].

Due to relative shortness of his penis a 67-year-old patient suffering from erectile dysfunction could not perform sexual intercourse, despite the implantation of a penile prosthesis. After intraspongious injection of 3 ml of a vasoactive substance mixture (papaverine 15 mg/ml and phentolamine 0,5 mg/ml) an increase in glandular circumference of 1.5 cm and in penile length of 1.8 cm took place, lasting for 1 1/2 h. By this means the patient regained the ability of a normal sexual life.
