[Magnetic resonance tomography with fast spin-echo sequences--the results in the ENT area].

UNLABELLED: The use of T2-weighted spin-echo sequences is mandatory for identifying tumours in the ENT region and for differential diagnosis of pathological findings. A highly promising alternative to time-consuming conventional SE sequences is now available in the fast-spin-echo sequences (FSE). FSE was compared with the conventional SE (CSE) basing on 100 assessments of examinations of 20 patients. RESULTS: Conventional SE = TR/TE = 1800/90 ms with one average (NEX), FSE = TR/TE = 3200/120 ms with 4 averages (NEX). Measurement time CSE 6:59 min, FSE 3:12 min. Analysis was performed in analogy to on ROC analysis by 5 radiologists as a blind study. The anatomic differentiation and contrast performance were compared as well as the differentiation and definition of lesion and lymphatic node and a subjective overall assessment. Differentiation of pathological findings was the same with both sequences. FSE was superior to CSE in the visualisation of normal tissue, lymph nodes and also in the subjective overall assessment. The following criteria were employed in the subjective overall valuation: sum total of impressions gathered from contrast behaviour or performance, susceptibility to artifacts, and signal-to-noise ratio. Due to the definitely improved parameters of image quality and the markedly reduced scanning time--which is reduced to 50%--FSE will replace the conventional spin-echo sequence in routine diagnostics.

[Value of magnetic resonance tomography for diagnosis of bone tumors].

Primary bone tumors are well investigated with magnetic resonance imaging (MRI). Depending on the tumor site and localization the examination technique has to be tailored to the particular situation. Using MRI, the delineation of extraosseous tumor spread and the intramedullary extension of malignant bone tumors is superior as compared to other imaging modalities.

["Metastasizing" Echinococcus alveolar of the liver].

We report a case of metastatic alveolar echinococcosis of the liver. There were calcifications of the liver parenchyma, and a large tumour adjacent to the porta hepatis infiltrated neighbouring structures. In addition, haematogenous metastases to the spine and foot were found. This extensive haematogenous "metastasis" is rare and may complicate diagnosis, but the typical findings in the liver should permit the correct conclusion.

Paragangliomas of the jugular bulb and carotid body: MR imaging with short sequences and Gd-DTPA enhancement.

Twenty-six patients with glomus jugulare (16), glomus tympanicum (three), or carotid glomus (seven) tumors were examined with contrast-enhanced CT scans and MR scans without and with Gd-DTPA. MR and CT scans had similar sensitivities, but the enhanced MR scans were diagnostically more specific than either CT or nonenhanced MR. Dynamic MR scanning permitted measurement of the degree of Gd-DTPA enhancement over time. We recommend contrast-enhanced MR with short sequences and a dynamic approach in patients with suspected carotid, tympanic, and jugular paragangliomas.

[Magnetic resonance tomographic studies of paragangliomas of the glomus caroticum and glomus jugulare using Gd-DTPA].

The value of magnetic resonance tomography in the diagnosis of paragangliomas in the head and neck has been studied and compared with CT and angiography. Magnetic resonance tomography on its own equals the accuracy of CT, but the use of Gd-DTPA improves diagnostic accuracy. In 19 patients with a glomus jugulare tumour, MR tomography with Gd-DTPA accurately diagnosed all tumours larger than 5 mm. In seven patients with a carotid body tumour, it was possible to arrive at an exact differential diagnosis. Sensitivity was better than that of CT or of sonography. Where there is clinical suspicion of a glomus tumour, magnetic resonance tomography with Gd-DTPA should be the first investigation; if there is a positive finding, angiography should be carried out to demonstrate the circle of Willis.

[Magnetic resonance tomography in suspected acoustic neurinoma: technic and differential diagnosis].

Sixty-five patients with suspected acoustic neuromas were examined by CT and MR; the optimal diagnostic procedures are discussed. Extrameatal acoustic neuromas (11 cases) were all diagnosed by CT and MR, but only 82% of intrameatal tumours could be diagnosed by CT combined with air cisternography. Using special surface coils and the paramagnetic contrast medium Gd-DTPA, the accuracy of MR was 100%. All tumours greater than 4 mm were demonstrated. Following clinical and neurological examination, MR is the primary diagnostic method in the investigation of acoustic neuromas.

MR imaging of the oropharynx and tongue: comparison of plain and Gd-DTPA studies.

The effect of intravenously administered gadolinium-diethylene-triamine pentaacetic acid (Gd-DTPA) on signal intensity in comparison to plain noncontrast imaging was analyzed. Sixty-one patients with diseases of the oropharynx and tongue base were examined using multiplanar magnetic resonance (MR) imaging. The contrast medium Gd-DTPA was administered in 26 cases. Thirty-eight patients also underwent CT. Forty-one patients had surgery and pathological verification. Plain MR imaging was equal or superior to CT in all except one patient. Marked contrast enhancement produced by Gd-DTPA was observed in carcinomas, sarcomas, inflamed salivary glands, and in normal pharyngeal mucosa. The usefulness of Gd-DTPA may be increased by measuring the enhancement in signal intensity versus time to allow a better differentiation of histological features.
