[Experiences with the metroplasty].

Report on 34 metroplasties between 1955 and 1980 at the women's hospital in Halle. 20 patients postoperatively delivered 24 children. One patient is pregnant at present. There were 6 abortions. A spontaneous delivery is possible if there are no additional indications for caesarean section. In 26 cases infertility was the indication to perform the metroplasty. In case of coexisting sterility past experience has shown to insist on an exact diagnostic procedure of sterility.

Influence of prolactin on metabolism and energy production in perfused corpus luteum bearing bovine ovaries.

Eight bovine ovaries with a corpus luteum were perfused for 4 h in a haemoglobin-free semi-synthetic perfusion medium in a closed circuit. After an initial prolactin (PRL)-free perfusion phase, 4.7 ng/ml ovine PRL was added in the 1st h, followed by 47 ng/ml in the 2nd h and 470 ng/ml in the 3rd h. Glucose and oxygen consumption and the production of lactate, pyruvate and CO2 were measured, while perfusion pressure and pH-value were recorded continuously. Under the influence of PRL anaerobic glucose metabolism was stimulated by 40.5% and oxidative phosphorylation was inhibited. Energy production from aerobic glucose metabolism rose by only 0.25%. Unlike PRL, Human Menopausal Gonadotropin (hMG) and Human Chorionic Gonadotropin (hCG) stimulated aerobic metabolism. This may indicate that PRL is the "older" hormone in phylleogenetic terms.

The effect of indomethacin on spontaneous and human menopausal gonadotropin induced pressure changes in the tissue and arterial vascular system from human ovaries perfused in vitro.

Eight human ovaries were perfused in a closed recirculating system with a semisynthetic, hemoglobin-free medium. Arterial and intraovarian pressures were recorded simultaneously. In three experiments spontaneous pressure changes were noticed. In five experiments pressure changes were induced with human menopausal gonadotropin (hMG) (approximately 70mIU/ml follicle stimulating hormone (FSH) and 40mIU/ml luteotrophic hormone (LH) II. IRP-hMG). Frequency and amplitude of pressure changes in the arterial system and fluctuations in tissue pressure were reduced with indomethacin (20 micrograms/ml). In one experiment 40 micrograms/ml indomethacin had been used. In spite of the high dose of indomethacin arterial and tissue pressure changes were not abolished in all experiments. Therefore it is assumed that vascular and tissue activities are not mediated by prostaglandins only.

[Ultrasound mammography: development, current status and limits].

Starting with a survey of the historical development of ultrasound-mammography the current status of this method within other diagnostic procedures of the breast is defined. Basing on over 1000 examinations since 1975 we report the sonographic possibilities in demonstrating different normal and pathologic breast structures. Pathologic structures are: Dudectasy, Cysts, Mastitis, Fibroadenoma, Carcinoma and Gynaecomasty. Own results are reported and the future of ultrasound mammography is discussed.

[Ultrasound mammography:special applications (author's transl)].

Clinical observations on more than 700 patients between September 1979 and October 1980 are reported. If cysts are suspected clinically, ultrasound mammography provides the most reliable diagnosis and is superior to all other methods for localization, fine-needle biopsy and follow-up. Since it is difficult to assess (due to summation effects), the radiologically dense breast (DY) is a further indication for sonography. In case of excentrically located palpatory findings, pregnant or immobile patients, ultrasound mammography can provide valuable diagnostic information.

Early primary diagnosis of ovarian cancer and detection of recurrence by serum cystine aminopeptidase assay.

The aim of this study was to correlate the relationship between serum cystine aminopeptidase (CAP) activity in normal patients and that in patients with primary ovarian cancer of recurrence. Sera obtained from 63 patients and evaluated for CAP activity by the method of Babuna and Yenen were grouped as follows: in 20 normal, nonpregnant patients, the mean activity was 25; in 14 patients with fibromas of the uterus, it was 38; in two patients with ovarian sarcoma, the activities were "normal" (24 to 27); and in 27 patients with primary ovarian adenocarcinoma or recurrence, the activity corresponded to the size of the tumors. Assay of serum CAP activity is a simple procedure which can be performed in any laboratory. It is a quick method for qualitative, and even quantitative, assessment of an ovarian adenocarcinoma in the nonpregnant patient. It may prove to be especially valuable in the follow-up of patients with ovarian cancer.

[The treatment of female stress incontinence of urine combined with uterine and vaginal prolapse by vaginal hysterectomy, anterior and posterior colporrhaphy in 1,003 cases (author's transl)].

The type and intensity of pre-operative symptoms and the long term follow-up in 1,003 vaginal hysterectomies with anterior and posterior colporrhaphies are reported. The operative technique corresponds to that of the school of Vienna with emphasis on plication of the urethra with joining of the ischio-carverrosus muscles below the urethra. The pre-operative symptoms disappeared in almost all cases. Constipation and backache improved. Early complications were treated and remained without sequelae. Our method of treatment showed good results and cures the social handicap of stress incontinence. In addition a good prophylaxis against unterine cancer and excellent permanent contraception is provided.

[New aspects in ultrasound mammography (author's transl)].

We report about 239 ultrasound examinations of the breast using a new water-tank scanner (U.I.Octoson, Ausonics, Sydney, Australia). A very good tissue differentiation of the brease is caused by a special technique and a high resolution. Beside benign results, typical findings in 49 malignancies are demonstrated ("shadowing", "christmastree-sign") and its value discussed. All ultrasound examinations are correlated to X-ray mammography, thermography, needle-biopsy and patho-histology.

[Measurements of spontaneous pressure changes in the arterial vessels and in the tissue of bovine ovaries perfused in vitro (author's transl)].

12 bovine ovaries were perfused in a closed circuit system using a semisynthetic hemoglobinfree perfusate. The pressure in the ovarian tissue and the arterial pressure were recorded simultaneously. Arterial and tissue pressure changes were noticed in bovine ovaries, as have been described in other species. It could be shown that pressure in the vascular system and the arterial pressure may vary independently. It appears as if spontaneous pressure changes in the in vitro perfused ovary are more pronounced in frequency and amplitude during the phase of follicle maturation.
