Synthesis of a new series of 4-aryl-1,4-dihydropyridines with calcium channel blocking and vasodilatory activity.

Synthesis of a new series of 4-aryl-1,4-dihydropyridines possessing potential calcium channel blocking activity along with good vasodilatory profile is reported. The compounds were synthesized using modified Hantzsch condensation of various aldehydes with methyl 3-aminocrotonate in the presence of a catalytic amount of trifluoroacetic acid and subsequent alkylation with various hydrochlorides of dialkylaminoalkyl chlorides. In vitro calcium channel blocking activity has been evaluated in cultures of neonatal rat cortical neurons by measuring the inhibitory response at L-type calcium channels activated by veratridine. Many compounds exhibited moderate to significant calcium channel blockade around 1 microM. The vasodilatory activity was assessed on isolated rat thoracic aortic rings precontracted by phenylephrine/KCl (30 mM). Most of the compounds produced a concentration-dependent inhibition of the contractile response.

[Chronic backache and occupational rehabilitation: prognostic factors].

The professional outcome in 65 patients with chronic low back pain who stayed in the Invalidity Insurance Medical Observation Center from 1982 to 1987 was studied retrospectively. The group studied was composed of 51 males and 14 females aged 28 to 61 years. Thirty-one out of 65 patients (48%) returned to work, 4 to full time employment (active group), whereas 34 did not return to work (inactive group). Data established during our initial observation served to compare the 2 groups by means of a logistic regression. No significant difference was found between the 2 groups of height, weight, age, family situation, severity of the X-ray findings and the existence of an associated psychiatric diagnosis were compared. On the other hand, the absence of professional training, female gender and the presence of a radicular syndrome significantly decreased the proportion of patients returning to work. If these 3 risk factors were absent, the chance of resuming work was 72%; if one was present it was 50%, and if there were 2 or 3 risk factors it was 28%.

Antihistaminic activity of pulegone on the guinea-pig ileum.

Pulegone, a natural monoterpene compound, has an antihistamine effect on guinea-pig ileum. Its antagonism is of the competitive type (PA2 = 6.35) like that of mepyramine and dexchlorpheniramine, two H1-antagonists, with PA2 values of 10.15 and 8.74, respectively.

[A simple test for the detection of respiratory allergy to current pneumallergens].

Phadiatop is a new in vitro test used for screening patients with allergic respiratory diseases. This test, based on the RAST procedure, is performed with a paper disc to which a balanced mixture of relevant inhalant allergens has been coupled. One hundred and one adults ranging in age from 18 years to 73 years and 38 children (4 months to 15 years) were studied. We found a good correlation between this new test and total IgE (adults: 74.3 p. 100, children: 68.4 p. 100), specific IgE (adults: 91.1 p. 100, children: 100 p. 100) and skin test (adults: 79.2 p. 100, children: 97.1 p. 100). There was a high correlation between clinical atopy assessment and the result of Phadiatop, its efficiency was 83 p. 100 in adults and 97 p. 100 in children.

[Ophthalmological side effects of lithium (author's transl)].

Among the side-effects due to the psychotropic drugs, one knows the ocular lesions, especially those due to major tranquillizers (f.e. opacities in the cornea and the lens). Ocular examination of 73 patients suffering of affective disorders, and receiving lithium salts during several years showed no significant ocular lesions due to the medication.

[Chronicity problems of the ambulatory patient].

This report is comprised of three parts: the first two parts give a very brief summary of two research projects carried out on groups of chronic, psychiatric out-patients within the framework of the University Psychiatric Policlinic of Lausanne. The third part is an attempt at discerning the fundamental, psychological aspects common to almost all chronic patients as well as the features typical of the relationship between the doctor and his chronic patient. The first piece of research consists of an epidemiological, clinical and catamnestic study of a group of 309 patients. Ten years after the original sample group was selected, 82 patients are still being treated. Various signs help to bring into focus the picture of the so-called chronic patient. They also reveal that the doctor-patient relationship deviated rather rapidly starting right from the first consultation towards a relationship described under the title maintenance service relationship. Furthermore it can be seen that in those groups studied, it was not possible to discern which patients would become seriously chronic at the time when they were first taken in charge. The object of the second study was the medical prescriptions given to a group of 65 chronic patients in treatment from 1965 through 1977. As can be expected in a public service with its frequent change of physicians, the patients were followed by several, not to say numerous, physicians. Regarding the different reasons causing a change of prescription, it is interesting to note that the psychic state of the patient played a much more important role here than the change of the treating physician. Thus the observation of chronic, somatic patients is very useful for the psychological understanding of chronicity. There is, in fact, no concordance between the condition of a person suffering from a chronic, physical illness and a person who is psychologically chronic. A description is given of those factors related to personality structure whose interrelation--defined as a basic psychodynamic triad--is necessary for psychological chronicity to set in. Furthermore, the reflections underline the fact that a better understanding psychological chronicity is also very closely subordinated to increasing our knowledge of the doctor-patient relationship.

A comparison of interhemispheric transmission times as measured by verbal and manual reaction time.

Interhemispheric transmission times (IHTTs) were calculated for 33 right-handed subjects. Verbal and manual response times (RTs) were recorded to visual stimuli exposed in either the left or right visual fields (LF, RF) for 50 msec. Each subject received 15 stimuli (dots) in the LF (1.5 degrees to the left of fixation), 15 stimuli in the RF, and 15 blank trials in both the verbal and manual response condition. IHTTs were calculated for each subject by subtracting the mean RF RT (I.E., uncrossed path) from the means LF RT (i.e., crossed path). Results indicated a nonsignificant IHTT obtained under the manual response condition of 5 msec and a highly significant IHTT obtained under the verbal response condition of 20 msec. While no significant correlation was found between IHTTs obtained under both response conditions, high correlations were found between component RT measures (i.e., manual RF RT, manual LF RT, verbal RF RT, and verbal LF RT). Thus, IHTT is independent of simple RT.
