Class III dento-skeletal anomalies: rotational growth and treatment timing.

AIM: The interception of a Class III malocclusion requires a long-term growth prediction in order to estimate the subject's evolution from the prepubertal phase to adulthood. The aim of this retrospective longitudinal study was to highlight the differences in facial morphology in relation to the direction of mandibular growth in a sample of subjects with Class III skeletal anomalies divided on the basis of their Petrovic's auxological categories and rotational types. MATERIALS AND METHODS: The study involved 20 patients (11 females and 9 males) who started therapy before reaching their pubertal peak and were followed up for a mean of 4.3 years (range: 3.9-5.5 years). RESULTS: Despite the small sample size, the definition of the rotational type of growth was the main diagnostic element for setting the correct individualised therapy. CONCLUSION: We therefore believe that the observation of a larger sample would reinforce the diagnostic-therapeutic validity of Petrovic's auxological categories, allow an evaluation off all rotational types, and improve the statistical significance of the results obtained.

Age Factors↗

[Height growth velocity and orthodontic treatment timing].

The auxologic evaluation, by periodic measurements of the height of children and the comparison of the values obtained with standard curves of growth, is a common practice for the pediatrician providing a precise indication on the general course of the growth and on the presence of possible growth pathologies. In fact, decelerations or pathological accelerations of growth can be a suspicious sign of endocrine, metabolic, genetic, gastric or tumoral diseases. The use of some growth variables and particularly the evaluation of the kinetics of statural height growth, should enter the daily practice of the orthodontist. In fact, the correspondence among peaks of statural and mandibular growth allows the orthodontist to choose the suitable time to intervene with the therapy aimed to stimulate the growth of the jaws. In this paper the authors report 2 orthodontic cases characterized by a malocclusion due to mandibular deficiency, in which the integration of the orthodontic and pediatric competences, through the use of the curves of the speed of growth and other growth variables, has allowed to reach optimal results, having achieved normal skeletal and occlusal relationships in the shorter time. The periodic measurement of height growth velocity represents a simple and safe method that allows to evaluate the kinetics of growth in children, with the remarkable advantage, unlike radiological examinations, to be repeated without any risk for the patient.

[Neuromuscular occlusion in the orthodontic treatment of craniomandibular disorders. A clinical case].

In the last few years, it has been observed a considerable increase of cranio-mandibular disorders which presently represent one of the most common commitments for dentists and, in particular, for orthodontists. The aim of this paper is to evaluate the efficacy of Jankelson's therapeutic protocol, followed by an orthodontic treatment, in a patient with cranio-mandibular disorders associated with occlusal pathologies. The cranio-mandibular electromyography and kinesiography promoted by Jankelson provide objective diagnostic measurements, while tens, relaxing masticatory muscles, allows a correct recording of myocentric occlusion. In this case, the authors utilise these devices, according to a well established diagnostic and therapeutic protocol. The first step is purely gnatologic, and consists of the application of an orthotic to temporarily treat pain and dysfunction. The following phase is a simple orthodontic treatment representing the final therapy. Orthodontic and/or prosthodontic rehabilitation of dentition, in fact, is the ultimate step of the therapeutic scheme which allows long-lasting RESULTS.

Relationship between chronic nasal obstruction and craniofacial growth: an experimental model.

The aim of this paper was to verify if the growth of the nasomaxillary complex can be influenced by a purely functional alteration such as nasal obstruction, which was induced experimentally in a genetically controlled animal model. Sixty albino rats were employed. Twenty of them had the right nostril occluded by a synthetic resin; another twenty had both nostrils occluded; the other 20 were taken as control group. When the growth was completed, the rats were sacrificed and cephalometric analysis was carried out. Both treated groups showed a statistically significant reduction in overall weight and height, in the vertical development of the nasomaxillary complex and in the skullbase longitudinal axis. After discussing the literature on the subject, the authors conclude that normal craniofacial growth in the rat must somehow depend on physiological nasal breathing, which should therefore be considered of crucial importance.

[Craniofacial morphogenesis after unilateral extraction experiments on rabbits].

BACKGROUND: Many authors have observed that reduction of the posterior vertical dimension after the loss of teeth during cranio-facial growth can lead to alterations not only of occlusion but also of cranio-facial skull structure. The alteration differs according to whether the absence of teeth is mono- or bilateral. In this paper, the authors present the skeletal alterations after monolateral tooth loss. METHODS: An experiment was performed on 25 rabbits, all having undergone monolateral tooth extraction. The experiment followed two rules of procedure: in a group of rabbits we extrasted the upper teeth only (experiment A), in another group of rabbits the extractions were performed both in the upper and in the lower jaw (experiment B), the third group of rabbits was used as a control group. At regular intervals, the authors examined skeletal alterations by means of Engstrom's cephalometric analysis (the asymmetries, the vertical posterior dimension). RESULTS: It was thus possible to observe some alterations of the cranio-facial bones and to compare the data with those of the control group.

[Oxytalan fibers in the condylar cartilage. Methodologic and experimental contribution of histologic studies conducted on dogs, rabbits and rats].

BACKGROUND: Oxytalan fibers were identified by Fullmer and Lillie in 1958 for the first time, using the same coloring methods utilized for the elastic fibers. However, the istologic preparation had been previously treated with strong oxidation. Oxytalan fibers were found in many organs and tissues of different animal species and seem to be a type of "immature" elastic fiber in a precocious state of development. Anyway, the functional role of oxytalan fibers is still not clear. The oxytalan fibers, located in particular mechanical stress anatomic sides, seem to be connected to a more compact connective tissue in this sides. The purpose of our study is to analyze the anatomic arrangement of the oxytalan fibers in the temporomandibUlar joint of three different animal species: the rat [correction of mouse], the rabbit and the dog. MATERIALS AND METHODS: Up to now, a coloring method specific for the oxytalan fibers has not been developed. Therefore, a correct evaluation of their presence requires a systematic approach in which two contiguous sections of a sample, one heavily oxidized and the other non oxidized, are compared. RESULTS: Our results show the presence of the oxytalan fibers differently located depending of the animal species and the age of the animals.

Nasal obstruction and skull base development: experimental study in the rat.

The influence of nasal obstruction on the development of the anterior skull base, of the tympanic bulla, and of the Eustachian tube was investigated in three groups of 20 albino Wistar rats in which one (group B) or both nostrils (group C) had been experimentally obstructed by means of synthetic resin mixed with radiopaque material. After two months, a cephalometric investigation demonstrated that the angle formed by the median line passing through the superior interincisive point and the most posterior median point of the occipital bone and by the line connecting the tympanic bulla and the superior interincisive point was significantly increased when the homolateral nostril had been obstructed if compared to controls (group A). On the basis of these preliminary cephalometric observations we suggest that nasal obstruction is able to determine anatomical changes of the superior maxilla, the skull base, and the jaw, with abnormal skeletal growth and consequent possible Eustachian tube dysfunction.

[Immunochemical and immunohistochemical study of calpastatin, an endogenous calpain inhibitor, in the masseter muscle of the rabbit].

The calpains-calpastatin system (calcium-activated neutral proteases and endogenous inhibitor) seems to be, in the skeletal muscle, a fine enzymatic system of myofibrillar turnover regulation, in normal as well as pathological conditions (for ex., dystrophic muscle). The purpose of the research is to establish in qualitative and quantitative terms whether the level of calpastatin can evidence differences between a muscle in normal activity conditions and one having dysfunctional alterations experimentally induced. So the masseter muscle of rabbit in normal conditions and with experimentally modified occlusal plane has been used. Our results confirm the presence of the 68 KDa calpastatin in the masseter muscle. The presence of the inhibitor in the same subcellular structures in which the calpains have been detected (myofibrillars, sarcolemma, endomysial connective) has been confirmed. Finally, variations in calpastatin amount in the muscle of animals experimentally treated with respect to the controls have been found. Thus, calpastatin seems to act as a marker of muscle dysfunctions connected to occlusal plane alteration and to loss of vertical dimention.

Analysis of chewing movement using elliptic Fourier descriptors.

Mandibular kinesiography measures and records the plot of jaw movements in the three spatial planes; this technique not only offers to investigators information about the dynamics of chewing, but also opens important prospects of quantifying dysfunction. At present, chewing patterns are compared on a qualitative basis only, and their morphologic analysis is not accurate. It is therefore necessary to use suitable mathematical algorithms that can manage the data supplied by modern image analyzers without losing the large amount of information they contain. A mathematical model that, by using a Fourier description of the plots of jaw movements, analyzes and characterizes different chewing classes has been developed. As an example of the possible applications, the variability of the exclusively morphologic aspect of the frontal plane plots of the chewing cycle of some subjects under standard conditions was studied and quantified. A morphologic distance between the Fourier coefficients that characterize each plot and the coefficients of a reference plot was also calculated. On these bases it will be possible to define and measure a "normal" chewing cycle under standard conditions and to make an eventual diagnosis of dysfunctional chewing based on a true morphoquantitative evaluation. Moreover, by deepening the analysis of the more occlusal part of the cycle, the model could aid in the identification of unapparent malocclusions.

The effect on bone repair of aspirin cones placed in extraction sockets in dogs: a histopathologic study.

Four canines of eight 2-year-old dogs were extracted surgically, a mucoperiosteal flap being raised. A standardized pair of the post-extraction cavities were filled with three aspirin cones, the other pair being used as controls. The flaps were sutured with surgical gut. The dogs were killed at intervals of 2, 4, 6 and 8 weeks; bone blocks including the cavities were resected and decalcified for histologic examination; serial sections were stained with hematoxylin and eosisin. Microscopic observation on 2 week samples showed an inflammatory response with granulation tissue in both experimental and control cavities with delayed bone apposition in cavities filled with aspirin cones. The 4, 6 and 8 week samples did not show any major difference between the experimental and control cavities. We observed a decrease in infiltrating of cells followed in both by a similarly increasing amount of fibrous tissue and bone remodeling. These results were due perhaps to the details of the experimental method as the suture allowed aspirin to stay in the cavities for only a short time (2--3 days), as occurs in clinical practice.

Clinical-morphological correlation of nasal obstruction with skull base development and otitis media. An experimental study.

Morphological changes of the middle ear were investigated by means of microotoscopy and scanning electron microscopy in 40 developing Albino rats. In 20 one nostril (group B) and in 20 both nostrils (group C) had been experimentally obstructed. 20 rats were used as controls (group A). Microotoscopy and scanning electron microscopy on the middle ear mucosa were performed when the animals were 2 months old, i.e. at 90% of their development. At microotoscopy the tympanic membranes, homolateral to the obstructed nostril, proved to be abnormal in 81% of group B rats and in almost all rats belonging to group C. Scanning electron microscopy, performed after the bullae had been removed, showed the following epithelial surface changes of the middle ear homolaterally to the obstructed nostril: (a) a significant increase of nonciliated elements with a secretory behavior as compared to controls, even of the floor where normally a large number of ciliated cells is present; (b) several ciliated cells were destroyed and their cilia were irregular and untidy; (c) thick mucous secretions covered the floor of the tympanic bulla; and (d) squamous metaplasia of the hypotympanic epithelium. Extrarotation of the eustachian tubes and developmental abnormalities of the skull base in growing rats, caused by nasal obstruction, seem to be able to induce pathological events of the middle ear.

Morphological distribution of middle-ear epithelium in the Wistar rat: a functional hypothesis.

The authors studied the overall microscopical anatomy of the middle ear and of the eustachian tube in 20 tympanic bullae of 10 Wistar rats. Large hexagonal epithelial flat cells covered the roof of the bulla, the two upper thirds of both lateral walls, the upper half of the eustachian tube, and the upper third of the tympanic orifice; ciliated and secretory cells lined the inferior thirds of both walls of the bulla while a ciliated epithelium with strong cilia, all directed towards the tubal orifice, located on the floor of the tympanic cavity together with groups of non-ciliated cells was detectable near the tubal and antral areas and on the floor of the eustachian tube. Non-ciliated cells covered by microvilli were found near the pharyngeal orifice. These observations seem to demonstrate the following: (1) the ciliated elements present a well-defined topographic distribution; (2) a preferential pathway for mucociliary clearance made up of strong ciliated cells located on the floor of the bulla is clearly detectable, and (3) the roof of the bulla seems to be mainly involved in the ventilatory function.
