Sensitivity and specificity of pathergy test results in Israeli patients with Behçet's disease.

Hyperreactivity of the skin to intracutaneous injection or needle prick (the pathergy test) is a unique feature of Behçet's disease. This phenomenon has been demonstrated to be highly sensitive and even specific in patients with Behçet's disease from Turkey, Japan, and Israel. A lack of positive reaction to this test has been reported in British and American patients. To evaluate the specificity of the pathergy test's results in Israeli patients with Behçet's disease, we compared their prevalence in these patients to that in a group of patients with other diseases associated with vasculitis. Forty-six Jewish and Arab Israeli patients with Behçet's disease, during a period of peak activity of their disease, and forty-six patients with other diseases associated with vasculitis, were studied for the hyperreactivity phenomenon. A positive reaction was observed in forty-five of forty-six patients with Behçet's disease, while the test gave negative results in all those with other diseases. Our study indicates, therefore, that the results of the pathergy test are highly sensitive and specific for Behçet's disease in Israeli patients.

[Brown spider bite].

The diagnosis of bite by the brown recluse spider, Loxosceles reclusus, is rarely based on absolute identification of the insect because the victims are usually bitten while sleeping or dressing. More often, the history, clinical findings and course of the bite lead to the diagnosis. For early confirmation up to 24 hours after the bite, the passive hemagglutination test can be used. For older lesions, the in-vitro lymphocyte transformation test is useful, but is available in only a few medical centers. Treatment of the bite of the brown recluse spider varies from conservative to more active approaches. Resting, local cooling, systemic antibiotics to prevent infection and mild anti-inflammatory drugs may be given. In the more active approach oral corticosteroids are added in the first 72 hours to the antibiotics, especially in massive bites with necrotic centers greater than 2 cm in diameter, or when there is systemic loxoscelism. Recently, good results have been reported with Avlosulfon (dapsone), which is claimed to cure necrotic cutaneous ulcerations, presumably by reducing the activity of polymorphonuclear leukocytes. Other treatments include specific antivenin, (of limited use because it must be administered shortly after the bite), and surgery to prevent spreading of the venom. We describe 3 cases of brown spider bite with typical clinical presentations in adults aged 20-40 years. 2 were treated with corticosteroids and antibiotics and 1 with Avlosulfon and prednisone, all within 72 hours of the bite. 2 recovered completely within a few days, but the third treated with prednisone and antibiotics, developed an ulcer which healed only after several months of treatment.(ABSTRACT TRUNCATED AT 250 WORDS)

A comparative histopathologic study of generalized and localized granuloma annulare.

We have reviewed the morphologic findings in 41 histologic slides of granuloma annulare (GA), 15 from localized (LGA) and 26 from generalized granuloma annulare (GGA). The most common pattern was the histiocytic infiltrative type, more so in cases with LGA than with GGA. The palisading granuloma pattern was present in 21.9% of patients and its prevalence was almost equal in both clinical types. The least prevalent histologic type in all patients taken together was the epithelioid nodule type. However, the prevalence of this type in LGA was equal to that of the palisading type, in contrast to that in GGA where it accounted for only 11.5%. In addition, a mixed histologic pattern was found in greater than 25% of sections from GGA and in only one section from LGA. Periodic acid-Schiff stain revealed hyalinized capillary basement membrane in most of GGA and only in approximately 50% of LGA sections. The variation in prevalence of the different histologic patterns between sections from LGA and GGA, and the difference between our findings and those observed in other series, are discussed.

Postirradiation extensive cicatricial scalp alopecia treated by tissue expansion.

The scalp of a 20-year-old woman with extensive cicatricial alopecia caused by x-ray irradiation for tinea capitis has been reconstructed by the use of tissue expansion. Two stages were required to reconstruct the large defect. The hair-bearing posterior scalp has been expanded to cover the whole irradiated area, creating a new anterior hairline and a pleasing cosmetic result.

[Treatment of pemphigus with plasmapheresis].

Therapeutic plasmapheresis was successfully used in a 29-year-old man with corticosteroid-resistant pemphigus vulgaris who had severe side-effects from the medication. 4 plasmaphereses were performed over a period of 3 weeks with removal of a total of 7900 ml of plasma. Concurrently he received large doses of adrenal steroids and Imuran. There was clinical improvement soon after the first plasmapheresis and 8 weeks after the treatment was started he was free of lesions. IC-Ab titers markedly decreased during the course of treatment and continued to decrease after it was completed. Side-effects were few and relatively mild. Plasmapheresis may be a useful adjunct in the treatment of pemphigus vulgaris resistant to conventional therapy. It may be used initially together with corticosteroids, giving more rapid improvement, as well as a corticosteroid-sparing effect.

[Erythema ab igne].

Erythema ab igne (EAI) has not been common since coal stoves have no longer been in general use for heating. Nevertheless, each winter several patients with macular-reticular skin lesions considered to be EAI are seen in our clinic. Correct diagnosis depends on awareness of the condition. 3 women, aged 17, 34 and 50, respectively, in whom EAI was induced by prolonged sitting close to an electrical heating device are presented. When this was discontinued the skin lesions disappeared. In the majority of reported cases of EAI the course was benign, similar to that in our patients in whom there were no additional related lesions. However, isolated cases of squamous cell carcinoma developing in patients with EAI have been described. In other cases the local heating which caused the EAI was applied for relief of pain and the appearance of EAI was the first clue to an underlying internal disease.

Elastic tissue changes in generalized granuloma annulare.

The histological picture in granuloma annulare (GA) is well defined. The types of infiltrate and the changes in collagen and elastic tissue have been repeatedly described. This is a preliminary study focused on the elastic tissue changes in lesions of generalized GA. Biopsies from four patients were studied. Specimens of involved and uninvolved skin from both exposed and unexposed parts of the body were compared. Changes of actinic elastosis in the papillary dermis in each patient in different stages were observed only in the exposed skin. The typical GA infiltrate was located beneath, in the upper mid-dermis. In the zone of the infiltrate in all specimens--exposed and unexposed--with no regard to the severity of actinic elastotic changes, absence or a marked reduction of elastic material was noted with engulfment of elastic remnants in giant cells. Giant cells with phagocytosis of elastotic material were mostly located on the periphery of the granuloma in the upper as well as in the deeper portion, where elastic fibers were still present. This might support the suggestion that degenerating elastic fibers promote the granulomatous reaction that leads to the formation of granuloma annulare.

Papillon-Lefèvre syndrome: a study of the long-term clinical course of recurrent pyogenic infections and the effects of etretinate treatment.

A family with a clinical variant of Papillon-Lefèvre syndrome (PLS), associated with recurrent pyogenic infections, has been followed up for more than 20 years. Of the five living siblings, four were treated with etretinate for a period of at least 21 consecutive months. Clinical follow-up showed that the course of recurrent infections in susceptible PLS patients, although usually more severe in childhood, can be variable and unpredictable. The etretinate therapy resulted in marked improvement of the keratodermas, and was associated with complete remission of the pyodermas on both keratotic and non-keratotic skin. It is, therefore, suggested that etretinate may have a primary role in the prevention of recurrent pyogenic infections in susceptible PLS patients.

Comparative ultrastructural study of generalized and localized granuloma annulare.

The dermal changes in four cases of generalized granuloma annulare (GGA) and in four of localized granuloma annulare (LGA) were studied ultrastructurally, and the findings in the two groups were compared. The basic alterations were similar in both types, and showed varied stages of development. The cellular infiltrate was composed mostly of histiocytes. Cell debris was found in all lesions. The degenerative changes affected the collagen fibers, the elastic fibers, and the cellular infiltrate. Of special interest are the following findings in GGA: First, masses of intercellular fibrin material were seen only in the lesions of GGA. Second, thick and multilayered basal lamina around capillaries was apparently more common in the generalized form. These changes may suggest that a more pronounced immune reaction is responsible for the development of the generalized form of the disease.

A study of HLA antigen association in localized and generalized granuloma annulare.

HLA-A and -B antigen frequencies were studied in 78 patients, 35 with localized granuloma annulare and 43 with generalized granuloma annulare (GA). Twenty-eight patients in each group were also typed for HLA-DR antigens. A group of 200 healthy age-matched subjects served as controls. HLA-A31 and B35 were increased significantly in patients with generalized GA, but not in the localized form. HLA-DR antigen distribution showed no significant variation.

The interaction of Kaposi's sarcoma with monoclonal antibodies to human sarcoma and connective tissue differentiation antigens.

Four monoclonal antibodies (McAbs) previously generated against human soft tissue sarcomas and reacting with connective tissue differentiation antigens were evaluated for their interaction with tissues obtained from patients with classic Kaposi's sarcoma. Biopsy was performed on active neoplastic lesions from the skin of 26 patients, frozen sections were prepared, and the binding of the McAbs was tested using the indirect immunofluorescence assay. Clinically uninvolved skin from the same patients as well as skin and muscle from eight non-cancer patients were treated similarly and served as controls. McAbs IXG11, 23H7, IIIE5, and 15G5 interacted strongly with the Kaposi's sarcoma lesions and weakly with the uninvolved skin in 22 of 26 (84%), 23 of 26 (88%), 12 of 14 (85%), and 1 of 6 (16%) of the patients, respectively. IXG11, 23H7, and IIIE5 interacted weakly with the skin of seven of eight non-cancer patients. McAb 15G5 was found to bind strongly to tumor lesions, to the respective uninvolved skin in four of five Kaposi's sarcoma patients, and also to skin and connective tissues of muscle from non-cancer patients. The mode of interaction was morphologically different for each McAb. It is suggested that McAbs IXG11, 23H7 and IIIE5 identify markers whose expression is markedly increased in Kaposi's sarcoma lesions as compared with uninvolved skin of the same patients. These markers may serve as immunologic probes for the investigation of this neoplastic process.
