[Long-term follow-up of vena cava filters: real-time sonography and the native x-ray image].

In 16 patients with vena cava filters (12 Günther filters, 4 Kimray-Greenfield filters) a follow-up study was performed by ultrasound and abdominal plain film. Ultrasound allows to identify type and location of the filter as well as the entrapment of emboli. Abdominal radiography is useful for exact evaluation of changes in filter position. Combination of sonography and plain films is considered to be most suitable for follow-up studies of caval filters.

Natriuretic effectiveness of bumetanide in rats with immature kidney function.

The influence of bumetanide on renal excretion of water and electrolytes was measured in saline loaded rats of various ages. The relative increase in sodium excretion following bumetanide administration is more pronounced in 5 and 15 day old rats than in older animals. The lower natriuretic potency of bumetanide in adult rats is not limited by the administered sodium dose since an additional sodium load does not provoke a more pronounced sodium excretion in these animals.

Circadian rhythm of C-reactive protein in patients with rheumatoid arthritis.

Ten men with classic rheumatoid arthritis were studied for 23 days in Badgastein, Austria, in August, 1980. One man (patient 07) showed a marked increase of disease activity after a few days. C-reactive protein (CRP) concentrations increased from 8.7 mg/dl on day 2 to 13.0 mg/dl on day 16. CRP values expressed as percent mean of a day showed a significant circadian rhythm with the acrophase at -30 degrees. For the same patient we also found significant circadian rhythms in grip strength and pearl stringing with acrophases in the evening and a circadian rhythm in walking time with the acrophase in the early morning. Seven of the ten men in the study had elevated CRP concentrations during the 3 weeks of observation. Population-mean cosinor results of CRP, grip strength, pearl stringing, and walking time revealed acrophases similar to the single cosinor results of patient 07. Our results suggest that inflammation in rheumatoid arthritis is a circadian rhythmic process with lowest disease activity in the evening.

Urinary neopterin reflects clinical activity in patients with rheumatoid arthritis.

Neopterin is a marker for activation of cellular immunity. Urinary neopterin levels were measured in 106 patients with rheumatoid arthritis (RA) and in 45 patients with osteoarthritis. Levels were significantly higher in RA patients than in osteoarthritis patients and were strongly dependent on stage and activity of RA. Correlations with other laboratory parameters were weak. Multivariate analysis demonstrated that urinary neopterin levels reflected clinical activity better than did other laboratory findings. Thus, urinary neopterin determination might be useful in monitoring RA patients.

Psychomotor disturbances in psychiatric patients as a possible basis for new attempts at differential diagnosis and therapy. II. Cross validation study on schizophrenic patients: persistence of a "psychotic motor syndrome" as possible evidence of an independent biological marker syndrome for schizophrenia.

This study investigates the existence and course of psychomotor symptoms in schizophrenic patients (n = 57, both treated and untreated with antipsychotic drugs) as compared to 25 healthy controls. Previous psychometric studies had suggested the existence of a "psychotic motor syndrome" (PMS) both in (untreated) schizophrenic and endogenous depressed patients, consisting of disturbances of lip and tongue movements, fine and gross movements of the dominant right hand and impaired complex motor coordination of the extremities. We confirmed the existence of the PMS in this study. There was no correlation of the PMS with the psychopathological status of the patients, or with extrapyramidal side-effects of the drugs used, perhaps indicating an independent "basic syndrome" ("Basisstörung"). Factorial analyses revealed similar structures both in schizophrenic and healthy persons; the differences in motor performance may be due to an impairment of the first factor "general motor ability" in schizophrenic patients. The PMS did not disappear parallel to the psychopathological improvement of the patients, nor in the symptom-free remission interval. The role of the PMS as possible independent biological marker syndrome for schizophrenia can consequently be further supported, with its implications towards the differential diagnostic and therapeutical values of this syndrome.

In vitro studies on the subcellular location of glucosidase I and glucosidase II in dog pancreas.

When programmed with yeast prepro-alpha-factor mRNA, the heterologous reticulocyte/dog pancreas translation system synthesizes two pheromone related polypeptides, a cytosolically located primary translation product (pp-alpha-Fcyt, 21 kDa) and a membrane-specific and multiply glycosylated alpha-factor precursor (pp-alpha-F3, 27.5 kDa). Glycosylation of the membrane specific pp-alpha-F3 species is competitively inhibited by synthetic peptides containing the consensus sequence Asn-Xaa-Thr as indicated by a shift of its molecular mass from 27.5 kDa to about 19.5 kDa (pp-alpha-F0), whereas the primary translation product pp-alpha-Fcyt is not affected. Likewise, only the glycosylated pp-alpha-F3 structure is digested by Endo H yielding a polypeptide with a molecular mass between pp-alpha-F0 and pp-alpha-Fcyt. These observations strongly suggest that the primary translation product is proteolytically processed during/on its translocation into the lumen of the microsomal vesicles. We believe that this proteolytic processing is due to the cleavage of a signal sequence from the pp-alpha-Fcyt species, although this interpretation contradicts previous data from other groups. The distinct effect exerted by various glycosidase inhibitors (e.g. 1-deoxynojirimycin, N-methyl-dNM, 1-deoxymannojirimycin) on the electrophoretic mobility of the pp-alpha-F3 polypeptide indicates that its oligosaccharide chains are processed to presumably Man9-GlcNAc2 structures under the in vitro conditions of translation. This oligosaccharide processing is most likely to involve the action of glucosidase I and glucosidase II as follows from the specificity of the glycosidase inhibitors applied and the differences of the molecular mass observed in their presence.(ABSTRACT TRUNCATED AT 250 WORDS)

[Selective blood sampling from the inferior petrosal sinus using digital subtraction angiography].

Simultaneous bilateral venous sampling of blood from the inferior petrosal sinuses helps in the differentiation between peripheral and central ACTH hypersecretion. One can also locate the site of a hormonally active hypophyseal micro-adenoma that cannot be demonstrated by other methods. The authors have experience with fifteen patients and discuss the indications, technique and problems as well as the advantages of using digital subtraction angiography.

[Embolization of a pseudoaneurysm of the hepatic artery with hemobilia by direct percutaneous puncture].

Haemobilia is a rare cause of gastrointestinal bleeding that carries a high mortality. It occurs as a complication of percutaneous trans-hepatic biliary drainage in up to 6% cases. The cause is usually a pseudo-aneurysm of the hepatic artery, which can be treated by trans-catheter embolisation. If this fails, direct puncture and embolisation of the aneurysm is a possible alternative to surgical ligation of the hepatic artery.

[Animal experiment results with a new vena cava filter].

A new cava filter has been tried in 23 dogs weighing between 16 and 23 kg. The filter consists of a stainless steel basket which can be introduced either antegrade or retrograde through a 10F teflon catheter and which can be removed again within the first four days. The filter was effective in holding back embolising thrombus. In the early stages, even without artificial embolisation, two out of five animals showed clot formation in the basket. After three or four months, all the filters were permeable and on only two occasions was a small amount of thrombus found.

Selective blood sampling in adrenal hypertension.

Selective venous blood sampling was performed in 89 patients with hypertension (14 pheochromocytoma, 10 Conn's syndrome, 8 Cushing's disease, 57 essential hypertension). We looked for diagnostic criteria and the valuability of blood sampling from the adrenal veins in such diseases. Defining a norepinephrine concentration of more than 8,000 ng/l as pathological, we had an accuracy of 94.6%. Defining an aldosterone concentration of more than 1,400 pg/ml as pathological, we had an accuracy of 97.4%. In Cushing's disease this method was not very helpful due to overlapping results.

[Lymphogranulomatosis: diagnosis by conventional roentgen studies and computed tomography].

Thanks to computed tomography the diagnostics of Hodgkin's disease has improved for both the nodal and the extranodal affection. CT is of special importance to determine an extension along the thoracic wall and to assess a pericardial and vascular invasion. All lymph node groups are visualized by computed tomography, but the structure of the lymph nodes cannot be assessed. In a series of 68 patients suffering from Hodgkin's and non Hodgkin's lymphomas, a retroperitoneal lesion was found by CT in 77% and by lymphography in 90% of cases. Sonography is considered as a complementary method. CT is not very sensitive in case of a spleen and liver lesion. Percutaneous biopsy is recommended for the diagnosis of recurrences and for the assessment of residual tumor tissue.

[Cystine aminopeptidase and oxytocin in the plasma of pregnant patients with premature labor].

L-cystine aminopeptidase (CAP) activity in plasma was determined by spectrophotometry in 243 normal pregnant women in week 20-40 and 68 pregnant women with premature pains in week 28-37 of pregnancy. For significantly diminished CAP activity, pains and the subsequent premature birth correlated with infantile dystrophy, while in case of normal CAP activity the number of premature births was significantly less and newborns had eutrophic birth weights. In addition, radioimmunological measurements of oxytocin concentration in plasma were performed in ten pregnant women each of these groups. In women with premature pains, the oxytocin concentration was significantly higher than in normals. The present findings suggest that fetal oxytocin release might be the trigger signal for pains, thus causing premature birth. This signal might be chronically induced by a stress situation which manifests itself in hypotrophy of the newborns.

[Non-operative percutaneous cholecystostomy in acute postoperative cholecystitis].

Seriously ill patients with acute postoperative cholecystitis are at high risk from emergency cholecystectomy. A case is presented which shows that non-operative percutaneous cholecystostomy may improve the patient's condition. With continuous percutaneous drainage of the infected bile and selective antibiotic treatment, the patient's condition can be stabilized so that all the necessary preparations for elective cholecystectomy can be made.

[Percutaneous transcatheter ablation of parathyroid gland tumors by alcohol injection and contrast media infusion].

An attempt to obliterate the parathyroid glands by a catheter technique has been made in three patients with primary hyperparathyroidism. Two mediastinal tumours were defunctionalized with alcohol, and followup for 11 and 13 months showed that it had been successful. One cervical parathyroid tumour was over-injected with contrast medium, but there was a recurrence after 14 months, as indicated by rising serum calcium and parathormone levels. In selected cases the method may be an alternative to surgery, particularly in patients where previous surgery had been unsuccessful or if there is a high operative risk.

[Removal of retained bile duct stones through the T-tube drain].

Within seven years 23 patients were treated postoperatively by percutaneous bile stone extraction. This procedure was successful in 18 patients. Although an interval of 5-7 weeks between choledochotomy and percutaneous extraction is recommended, the procedure was done 11 to 31 days after surgery with no negative effects. In six cases the stones had to be dislodged into a more favorable position with a Fogarty-type balloon catheter. The papilla of Vater was dilated in two cases. With the development of newer techniques trans-T-tube extraction remains the treatment of choice in retained intra- and extrahepatic bile duct stones.
