[Vitiligo from p-tert. butylphenol; a contribution to the problem of the internal manifestations of this occupational disease].

10 worker suffering from occupational vitiligo due to p-tert. butyl phenol have been observed in an Austrian resin factory. The dust concentration and its content of p-tert. butyl phenol in the working area were measured. On the basis of these results preventive measures were taken to reduce the exposure of the workers to p-test butyl phenol. In the presented cases an involvement of internal organs--as described by Rodermund et al. (1975)--could not be observed.

[Ferrokinetic studies in haemoglobin Wien haemolytic anaemia (author's transl)].

Ferrokinetic studies in two patients with haemolytic anaemia due to haemoglobin Wien revealed increased haemoglobin synthesis and an increase in effective erythropoiesis. Haemolysis is due to an instability of the haemoglobin itself. The spleen does not appear to play any appreciable role in the aetiology of this condition. There was no extramedullary erythropoiesis in our patients.

[Pathogenesis of hyperthyroidism].

The present concept for the etiology of thyroid disease permits the definition of three separate entities occuring with hyperthyroidism: Graves' Disease, Toxic Nodular Goitre, Autonomous Adenoma. Physiopathological properties of these entities lead to a rational therapeutic strategy for each one of them.

Adenoma

A case-report of idiopathic juvenile osteoporosis with particular reference to 47-calcium absorption.

Calcium metabolism was studied in a 12-year-old girl preseting with idiopathic juvenile osteoporosis. Absorption of orally administered 47-Ca was high. Serum calcium and phosphorus, serum immunoreactive PTH and CT and tubular phosphate reabsorption were found to be within normal limits. The data suggest that calcium malabsorption, nutritional calcium deficiency, hyperparathyroidism, a dysfunction related to sex hormones, and Cushing's syndrome cannot be implicated in the aetiology of the osteoporosis in this case who recovered spontaneously with sexual maturation.

Lack of association between Hashimoto's thyroiditis and SD-locus HLA antigens.

All at present serological detectable HLA antigens and humoral antithyroid immunity have been studied in 26 patients with Hashimoto's thyroiditis and 450 controls. No statistical significant difference in frequencies of the 36 HLA antigens tested could be found. These findings suggest that serological defined (SD) genes of the histocompatibility complex are not the major factors involved in the predisposition to Hashimoto's thyroiditis. By analysing prevalence of thyroid antibodies in HLA-B8 positive and HLA-B8 negative groups of Hashimoto's thyroiditis patients an association between B8 and thyroglobulin and thyroid microsomal antibody formation was found. Although frequency of HLA-B8 was not increased in the patients, B8 positivity may reflect an increased susceptibility for humoral antithyroid immunity.

[Investigations of the final settling of implanted hip-joints by use of 85Sr (author's transl)].

Up to 8 years after replacement of hip-joints in 26 patients the strontium-85-uptake over both thighs was followed scintimetrically. In contrary to normal healing of fractures, this uptake became normal on the side of the operation not earlier than the end of the 2nd year after operation. One cause of this delay could be the continuing liberation of monomeres of polymethyl-methacrylates at the bed of the implant. It is important to consider the clinical signs on the opposite side, since there a raised strontium-85-uptake could produce a "pseudo"-diminished ratio between the two sides.

Aged

Genetic heterogeneity in thyrotoxicosis patients with and without endocrine ophthalmopathy.

HLA antigens and humoral antithyroid immunity have been studied in 97 thyrotoxic patients [48 without and 49 with endocrine ophthalmopathy (EOP)]. A statistical significant increase of HLA-B8 frequency could be observed only in the thyrotoxic patients with EOP, whereas in patients without EOP B8 frequency was similar as to the one in the controls. The C locus antigen Cw3 was significantly different between both groups of patients: increased in those with EOP and statistically significantly decreased in those without EOP. Thyroid microsomal antibodies were found to be associated with HLA-B8 positivity and were noted with highest incidence in B8 positive EOP patients. No such association was observed for thyroglobulin antibodies. These data show a genetic heterogeneity in thyrotoxicosis patients with and without EOP. HLA-Cw3 positivity represents and increased risk to develop EOP in thyrotoxicosis, as frequency of this antigen was significantly decreased in thyrotoxicosis patients without any signs of EOP. The increased frequency for microsomal antibodies in thyrotoxic patients with EOP, especially in B8 positives underlines the genetically influenced autoimmune reactivity in that disorder.

Antibody Formation

[Screening for hypothyroidism in the newborn with a total T 4-RIA method (author's transl)].

A screening method for detection of congenital hypothyroidism is presented in detail in cooperation with the "Austrian Program for Inborn Errors of Metabolism". Screening is performed by measuring the total T4-content of 1/8 inch filter paper dots filled with dried blood of newborns. The strategy for recall of newborns with borderline or pathological T4-values used, results in a definite diagnosis on day 55 of life. The advantages of additional TSH determination in the filter paper dots are discussed. So far (Sept. 1976) 8645 newborns have been investigated, covering the regions of Vienna and Carinthia (Austria). Preliminary studies reveal a slight dependency of the measured T4-values on the day of sampling. Two congenitally hypothyroid children have been diagnosed so far, corresponding fairly well with the reported frequency in the literature (1 : 6000).

Humans

HLA CW3 in thyrotoxicosis patients with and without endocrine ophthalmopathy.

Fifty-four thyrotoxic patients (23 with and 31 without endocrine ophthalmopathy EOP) were HLA typed for A, B, and C locus antigens. The frequency of this third locus antigen CW3 was found to be significantly increased in patients with EOP compared with those thyrotoxic patients without EOP. These findings point to a possible genetic heterogeneity in thyrotoxic patients with or without EOP. HLA B8 was observed in only 10 of the 54 patients studied, which is not in accordance with the previously reported statistically significant increased frequency of this B locus antigen in thyrotoxic patients.

[Microfluorometric demonstration of thyroglobulin antibodies with the defined antigen substrate spheres (DASS) system].

Previous investigations showed that quantitative immunofluorescence using antigens covalently bound to agarose particles represents a reproducible and sensitive assay for antibodies in experimental antisera. In this paper data are presented which show that also thyroglobulin antibodies in patients sera can be demonstrated using the defined antigen substrate spheres (DASS) system with thyroglobulin as antigen. Sera of 45 patients with various thyroid diseases were investigated for the presence of thyroglobulin antibodies using passive haemagglutination and the quantitative immunofluorescence technique. Comparable results were obtained with both techniques. Advantages and disadvantages of both methods are discussed.

Autoantibodies

[Study of age and sex dependance in renal clearances with radioisotopes (author's transl)].

In 85 patients between 15 and 95 years of age (40 male patients and 45 females) without any signs of renal disease (concentration of serum creatinine below 1,4 mg/100ml, normal diastolic blood pressure and normal urine analysis) a highly significant negative correlation was found between glomerular filtration rate (GFR, determined by 51Cr-EDTA - slope-clearance) and renal plasma flow (RPF, determined by 131J-oJHS-iodinehippuricacid-totalclearance) on one side and age on the other. Beginning with a basic value of 160 ml in males and 144 ml in females for GFR, the average decrease per year was 1,2 ml/1,0 ml respectively. The basis value of RPF was 854 ml for males and 673 ml for females, the decrease per year was 5,4 ml in men and 2,9 ml in women. The importance of physiological diminution of clearance values in elderly patients and its consequences for therapy are discussed. The more exact and methodically simpler 51Cr-EDTA-slope-clearance without urine collection should be used instead of the somewhat inaccurate determination of endogenous creatinine clearance especially in older patients.
