[Human calcitonin in the treatment of symptomatic Paget's disease].

We evaluated the effect of a 6 months trial with synthetic human calcitonin in 10 symptomatic patients with polyosseous Paget's disease of bone. All patients reported a distinct clinical improvement which correlated with a significant decrease of alkaline phosphatase in the plasma and hydroxyproline and calcium excretion in the urine. Though no changes in skeletal x-rays were observed, the bone lesions showed a significant decrease of activity in tho bone scintigrams. Histologically there was a marked decrease of bone turn-over calcitonin therapy. No patient revealed the formation of autoantibodies against exogenous calcitonin after treatment.

[Platelet half-life in patients with primary hyperlipoproteinemia type IIa, IIb, and IV according to Fredrickson with and without clinical signs of atherosclerosis].

It is generally accepted that platelet half-life is shortened in atherosclerotic vascular disease. Concerning changes due to hyperlipoproteinemia (HLP), however, there exist only few data. Therefore, we examined the platelet-half life in 60 patients with recently discovered HLP type IIa, IIb and IV according to Fredrickson before treatment in comparison to 60 controls. 33 of the HLP-patients had no clinical symptoms of angiopathy, 27 patients suffered from peripheral vascular disease or from coronary heart disease as verified by angiography. The labelling of autologous platelets was performed with 100 microCi of 111Indium-oxine-sulfate at 37 degrees C for 5 minutes. The mean labelling efficiency was 90%, the recovery after 2 hours about 70%. Serum lipoproteins were estimated by means of ultracentrifugation and polyanionprecipitation according to Lipid Research Clinic Methods. In the patients with HLP platelet half-life was significantly shortened in comparison to the control group (p less than 0.01). These changes were most pronounced in patients with HLP-type IIa and with atherosclerotic lesions, respectively. In patients with HLP-type IIa a very close correlation could be demonstrated between platelet half-life and LDL-cholesterol (r = -0.72; p less than 0.001) as well as total cholesterol (r = -0.73; p less than 0.001). These data prove that in HLP in-vivo platelet function as measured by platelet survival is significantly influenced even before the occurrence of clinically relevant symptoms of atherosclerosis.

[Age dependency of platelet half life].

In the last few years many reports have been published concerning shortened platelet half-life in different diseases such as atherosclerosis and hematological disorders. Up to the present moment, however, there is no information available concerning age and sex dependency in healthy people. Therefore, we evaluated platelet half-life in 106 patients, who had been examined because of a suspected atherosclerotic or hematological disorder having been excluded in the sequel. In all the patients we found negative bicycle ergometry and negative sonography of the carotid arteries and the legs. Platelets were labelled with 100 muCi of 111Indium-oxine-sulfate at 37 degrees C for 5 minutes. The mean labelling efficiency was 90%, the recovery about 70% after two hours. We found a statistically significant correlation (r = -0,5395; p less than 0,001 between age and platelet half-life. In male patients we observed a trend towards shortened platelet half-life, however, the differences were not significant.

[The use of labeled thrombocytes in the monitoring of patients after kidney transplantation].

Indium 111 (111In) labelled autologous platelets were used in the diagnostic observation of patients after renal transplantation. 75 patients were collected in 3 groups of 25 cases each. The first group consisted of patients who were examined during the first 4 weeks after transplantation. Group 2 were patients suffering from histologically proved chronic graft rejection. The 3rd group included cases with good and stable transplant function tested at least 9 months after transplantation. By means of a computerized static study the transplants were examined under the gamma-camera concerning a platelet trapping. Thereby a platelet-uptake-index (PUI) was calculated and compared with the platelet half-life-time (t1/2). This diagnostic method turned out to be suitable for early diagnosis of acute graft rejection. Hereby the PUI increased from 1.13 +/- 0.11 to 1.74 +/- 0.17. The further course of rejection episode manifested itself in corresponding changes of PUI. In irreversible rejections the PUI stayed in the high range or increased further. On the other hand reversible graft rejections showed a reduction of PUI, however, in the most cases not to the original basic level. The platelet t1/2 exhibited a strong negative correlation with PUI. Cases suffering from chronic transplant rejection revealed significantly higher platelet trapping by the graft than patients with good and stable transplant function.

[Vitiligo from p-tert. butylphenol; a contribution to the problem of the internal manifestations of this occupational disease].

10 worker suffering from occupational vitiligo due to p-tert. butyl phenol have been observed in an Austrian resin factory. The dust concentration and its content of p-tert. butyl phenol in the working area were measured. On the basis of these results preventive measures were taken to reduce the exposure of the workers to p-test butyl phenol. In the presented cases an involvement of internal organs--as described by Rodermund et al. (1975)--could not be observed.

[Ferrokinetic studies in haemoglobin Wien haemolytic anaemia (author's transl)].

Ferrokinetic studies in two patients with haemolytic anaemia due to haemoglobin Wien revealed increased haemoglobin synthesis and an increase in effective erythropoiesis. Haemolysis is due to an instability of the haemoglobin itself. The spleen does not appear to play any appreciable role in the aetiology of this condition. There was no extramedullary erythropoiesis in our patients.

[Pathogenesis of hyperthyroidism].

The present concept for the etiology of thyroid disease permits the definition of three separate entities occuring with hyperthyroidism: Graves' Disease, Toxic Nodular Goitre, Autonomous Adenoma. Physiopathological properties of these entities lead to a rational therapeutic strategy for each one of them.

A case-report of idiopathic juvenile osteoporosis with particular reference to 47-calcium absorption.

Calcium metabolism was studied in a 12-year-old girl preseting with idiopathic juvenile osteoporosis. Absorption of orally administered 47-Ca was high. Serum calcium and phosphorus, serum immunoreactive PTH and CT and tubular phosphate reabsorption were found to be within normal limits. The data suggest that calcium malabsorption, nutritional calcium deficiency, hyperparathyroidism, a dysfunction related to sex hormones, and Cushing's syndrome cannot be implicated in the aetiology of the osteoporosis in this case who recovered spontaneously with sexual maturation.
