[In-vitro motility studies on the strips of the canine gallbladder (author's transl)].

In-vitro motility of the gallbladder was investigated in the circularly cut full thickness strips obtained from 19 mongrel dogs. The present study was undertaken to determine an appropriate size of the strips for observation and to know any difference, if exists, between motilities of the strips prepared from the neck, body and fundus. Recordings were made on a pen chart recorder using an isometric tension system. The results obtained were as follows. 1) The most appropriate size of the specimens to examine the changes of contractile dynamics was 5 mm in width and 15 mm in length. 2) The strips from three parts of the gallbladder showed similar rhythmic and tonic spontaneous contraction patterns.

[In-vitro motility studies on the strips of the human gallbladder (author's transl)].

The influence of inflammation on the motility of the human gallbladder was investigated through an in-vitro experiment. Motility of longitudinally cut strips from 50 patients were examined by the same method as described in our previous articles. We pathohistologically classified the severity of the inflammation of the specimens into four grades: grade 0 is normal, grade I, slightly inflamed; grade II, moderately inflamed; grade III, highly inflamed. Relationships between the grade of inflammation and the motility of the strips were summarized as follows. 1) In grade 0 cases, the strongest tonic contractions were observed in the strips from the fundus. This tendency was also found in cases of grade I and II but III inflammation. 2) The tonic contractions of the strips from the fundus or body decreased along with the grade of the inflammation. 3) In any strips, the frequency of the rhythmic contractions decreased progressively with the grade of the inflammation. 4) The amplitudes of rhythmic contractions of the strips with grade I and II inflammation were larger than those of grade 0 inflammation.

rho-Shaped anastomosis: a reconstruction of the alimentary tract after total gastrectomy.

One hundred sixty-four patients underwent total gastrectomy with a rho-shaped jejunal food pouch (rho loop) and roux-en-Y jejunojejunostomy. The rate of postoperative complications was very low (9.1 per cent), and the most important problem of anastomotic failure was never encountered. Only 4 of 164 patients (2.4 per cent) died within one month of surgery, and the causes of death were pneumonia (2 patients), cerebral hemorrhage (1), and ileus (1). The function of the newly constructed passage was estimated through an elaborate barium meal examination. The test revealed that the rho-shaped jejunal food pouch has adequate reservoir and mixing functions for ingested foodstuff. No reflux of barium meal into the esophagus was observed during the examination. In a follow-up study of the patients, only a few complained of symptoms of reflux esophagitis or dumping, but none of them needed any treatment.

[In-vitro motility studies on the strips of the human gallbladder (author's transl)].

Studies in-vitro motility of the human gallbladder was investigated in the longitudinally cut strips obtained from 24 patients. Strips of the wall of the organ, containing mucosa, muscle and serosa, 5 mm in width and 15 mm in length, were cut from the fundus, body and neck. These specimens were suspended in Tyrode solution at 36 degrees C equilibrated with the 100% O2. Recordings were made on a pen chart recorder which were transduced using a isometric system by the changes of contractions. The results obtained were as follows: 1. The strips from three parts of the gallbladder showed rhythmic and tonic spontaneous contractions. 2. The tonic contractions from fundus were strongest in those three. 3. The rhythmic contractions of neck exhibited greater amplitude than those of fundus and body.

A hydraulic behavior of the mucosal structure of the intramural common bile duct. An experimental study.

In the intramural portion of the common bile duct in the dog, the lumen is occluded by the mucosal folds which are pointed to the direction of the ductal orifice. Luminal occlusion by the folds becomes more marked from the halfway of the portion as judged from the duodenal muscle layers. A pressure study using the pressure ramp apparatus suggested that the mucosal structure plays some role in generating the passage resistance to bile floow and in the prevention of regurgitation of duodenal content.

Indirect cholangiojejunostomy using a Latex T tube for palliation of malignant jaundice.

Indirect cholangiojejunostomy was carried out using a Latex T tube for palliation of non-curative malignant jaundice and its technique is described. The surgical procedures reported here can separate cholangiojejunostomy which otherwise involves some difficulty into three simple steps: 1) insertion of a Latex T tube into the common bile duct or hepatic duct, 2) Witzel type internal fistulization, and 3) Roux-en-Y jejunostomy. The reported technique can be one of the useful arms for control of obstructive jaundice due to malignancy of the biliary-pancreatic region and may have a special appeal to general practitioners.

Acute renal failure in a tertiary care center in Nepal.

From July 1998 to July 1999, 45 cases of acute renal failure were treated at Bir Hospital, Kathmandu. Out of which 24 were male and 21 were female. Age ranged from 11 months to 84 years with mean age being 35 years and 9 cases were below 10 years. Four cases with pre-renal azotaemia and twenty five cases of acute tubular necrosis (ATN) accounted for 64% of all cases. These were due to gastroenteritis 10, sepsis 6, post surgical 1, trauma 1 and obstretical complications 5. Multiple hornet stings were responsible for acute renal failure in 3 cases, acute urate nephropathy in 1 case and miscellaneous causes in 2 cases. Glomerulonephritis / vasculitis accounted for 17.7%, acute interstitial nephritis 4.4%, haemotytic uraemic syndrome (HUS) 6.6%, and post renal azotaemia in 6.6% of all cases. Mean serum creatinine was 8 mg/dl, mean blood urea 190 mg/dl. Eight cases were treated only conservatively, eighteen received haemodialysis, fourteen received peritoneal dialysis, three received both and two refused for dialysis. Average duration of hospital stay was 13.6 days. Out of the forty-five cases twenty-nine recovered normal renal function, ten expired, two recovered partially, two progressed to chronic renal failure and two left against medical advice. Overall mortality was 22.2%. Common causes of acute renal failure in our setting were gastroenteritis (22%) and sepsis (20%). HUS was exclusively seen in children following bacillary dysentery. Multiple hornet stings is an important cause of acute renal failure in our country.
