Indication for histological examination of endometrium in breast carcinoma patients receiving tamoxifen therapy.

OBJECTIVE: To investigate the effects of tamoxifen on the uterine endometrium and define the indications for histological examination of endometrium on the thickness of uterine endometrium and on the duration of tamoxifen therapy. METHODS: The endometrial thickness was measured on the transvaginal ultrasonogram in 40 postmenopausal breast carcinoma patients receiving tamoxifen (tamoxifen group), and control group. Endometrial histological examination was carried out. Receiver operating characteristic (ROC) curve analysis was carried out. RESULTS: Endometrial thickness in the tamoxifen group was 11.2 +/- 5.1 mm, and that of the control group was 3.8 +/- 2.1 mm. The incidence of endometrial abnormalities in the tamoxifen group was greater than that in control group. The cut off values derived from the ROC curve analysis were 9 mm for endometrial thickness, and 24 months for duration of tamoxifen therapy. CONCLUSION: The histological examination of endometrium should be carried out if the endometrial thickness is more than 9 mm, or the duration of tamoxifen therapy is more than 24 months even if the patients do not have any symptoms.

[Significance of full-thickness chest wall resection as a local treatment for sternum metastasis of breast cancer].

Bone metastasis of breast cancer generally tends to be multiple. However, breast cancer frequently metastasizes to the sternum as solitary metastasis. Resection of the sternum was reported to be an effective surgical technique for sternum metastasis which could not be remarkably improved by chemo-endocrine therapy and locoregional recurrent breast cancer invading the skin. A first patient was diagnosed with sternum metastasis invading the skin 44 months after radical mastectomy for breast cancer of stage II. A second patient had breast cancer of stage IV simultaneously with metastases to the sternum and the lung. Only the lung metastasis was successfully controlled by chemo-endocrine therapy, but the sternum metastasis was progressive, following radical mastectomy. Both patients manifested solitary bone metastases and underwent full-thickness chest wall resection. The defect of the chest wall was reconstructed with the mucocutaneous flap using the dorsal latissimus muscle in case 1 and with the abdominal rectus muscle in case 2. Twenty months after the resection of the sternum, the first patient suffered from supraclavicular lymph node metastasis, and the multidisciplinary treatment including radiation therapy was indicated. This therapeutic procedure was effective, and the patient survived for 63 months after resection of the sternum. The second patient has made good progress for six months in maintaining complete remission with chemo-endocrine therapy. These findings suggested that surgical resection was a useful local treatment for solitary sternum metastasis.

The argyrophilic nucleolar organizer region (Ag-NOR) score in invasive ductal carcinoma of the pancreas.

The biological malignancy of pancreatic carcinoma was evaluated by determining the score of the argyrophilic nucleolar organizer region (Ag-NOR) in resected specimens, and comparing it with the various clinicopathological factors and long-term results of 38 patients who underwent surgical resection for invasive ductal carcinoma of the pancreas between 1977 and 1992, in whom the Ag-NOR could be stained. The Ag-NOR analysis of pancreatic carcinomas from the 38 patients resulted in a mean Ag-NOR score of 3.82 +/- 0.62, which was significantly (P < 0.01) higher than the mean score of 1.72 +/- 0.28 observed in normal pancreatic tissues obtained from 20 of these patients. The mean Ag-NOR score significantly increased in patients with anterior capsular infiltration (s) (P < 0.01), posterior tissue infiltration (rp) (P < 0.01), and lymph node metastasis (n) (P < 0.05) compared to those without these factors. The rate of curability A or B was only 13.0% in patients with a high Ag-NOR score of > or = 3.80 in comparison to 66.7% in those with a low Ag-NOR score of < 3.80. The 3-year survival rate was significantly (P < 0.05) higher in the low Ag-NOR score group than in the high Ag-NOR score group. These results suggest that the Ag-NOR score can serve as an indicator of the biological malignancy of pancreatic carcinoma, and of the patients' prognosis.

[Intraoperative interstitial microwave therapy for recurrent hepatocellular carcinoma of the caudate lobe: a case report].

Microwave tissue coagulation (MTC) therapy was given patients with recurrent hepatocellular carcinoma (HCC) of the caudate lobe of the liver, in which radical surgery for deteriorated liver function was impossible. A total of 40 MTC sessions was applied to two tumors under laparotomy. MTC was administered for 20 seconds in each session. Microwave energy output was 70 watts for 15 mm needle-electrodes and 100 watts for 30 mm electrodes. Alpha-fetoprotein levels in serum had decreased after surgery. Abdominal computed tomography showed no blood flow whatsoever in tumors undergoing MTC. There are fewer limits to the sites and angles for insertion of electrodes under laparotomy. Thus, the surgical approach provides access to all parts of the liver for treatment. We conclude that intraoperative MTC is highly effective in tumor necrosis, and can be a useful local treatment for nonresectable HCC.

Effects of microwave tissue coagulation on the livers of normal rabbits: a comparison of findings of image analysis and histopathological examination.

The livers of normal rabbits were subjected to microwave tissue coagulation (MTC), and comparison was made of the subsequent time-course changes in tissue observed on MRI, CT and histopathological examination. 16 rabbits were used. MTC was performed with a 21 gauge needle electrode inserted into the liver at laparotomy. 1-2 h after thermal coagulation, a region with slightly lower attenuation than that of surrounding normal liver parenchyma was observed on CT, and no enhancement was detected. With MRI, change from high signal intensity to iso-signal intensity from the inner zone to the margin was found on T1 weighted images (T1WI), and heterogeneous high signal intensity was observed on T2 weighted images (T2WI). On Gd-DTPA enhanced MRI, no enhancement occurred. 1-4 weeks after coagulation, the cellular structure at the site of coagulation was lost on histological examination, and the tissue became necrotic. On CT, homogeneous water density was observed, and no enhancement was detected. With MRI, regions of iso- or slightly low signal intensity were observed on T1WI, and regions of heterogeneous high to low signal intensity were seen on T2WI. After 1 week, a granulation layer consisting mainly of fibrous tissue developed, and a ring-shaped enhancement was observed in the low signal intensity region on T1WI and in the high signal intensity region on T2WI. The ring-shaped enhancement was also noted on CT. MRI appears to be useful for observation of time-course changes following MTC therapy because of its sensitivity in the detection of tissue changes.

Intraoperative microwave tissue coagulation as treatment for patients with nonresectable hepatocellular carcinoma.

BACKGROUND: The microwave tissue coagulator (2450 MHz) has been used clinically in the treatment of hepatocellular carcinoma (HCC) to transection of the liver parenchyma and has proven an excellent method for hemostasis. There are, however, few reports on the application of this coagulator to the induction of tumor necrosis. METHODS: Microwave tissue coagulation (MTC) was applied at laparotomy in eight patients with nonresectable multiple HCCs. All patients were treated with a combination of resection or intrahepato-arterial chemotherapy and MTC. A total of 222 bouts of MTC were applied to 21 tumors, the largest of which was 65 mm in largest dimension. The monopolar needle electrode was inserted directly into the tumor and the procedure was repeated at approximately 5 mm intervals. RESULTS: Levels of alpha-fetoprotein in serum were found to have decreased in all patients one month after surgery with MTC. Contrast-enhanced computerized tomography (CT) showed the complete absence of blood flow in all tumors subjected to MTC. Needle biopsy one month after MTC confirmed tumor necrosis in all cases. All patients are alive at the time of this report, with the longest survival period being 24 months. In three of eight patients, new tumors were confirmed by angiographic CT at sites separate from the treated tumors. MTC resulted in fewer adverse effects on liver function and less extensive inflammatory reactions than liver resection. CONCLUSION: Intraoperative MTC appears to be an effective method for inducing local tumor necrosis, and may be of use in combination with palliative surgery for multiple HCC when radical liver resection is not feasible.

Long-term survival of 5 years following initial surgery for gastric cancer and simultaneous disseminated peritoneal metastasis: report of a case.

We report herein the rare case of a patient who survived for 5 years and 10 months after commencing treatment for gastric cancer with simultaneous disseminated peritoneal metastasis. A 45-year-old man was diagnosed as having advanced gastric cancer following the discovery of numerous nodules in the peritoneal cavity at laparotomy. The patient was treated by palliative gastrectomy and continuous hyperthermic peritoneal perfusion (CHPP) immediately after surgery on November 11, 1987. Postoperatively, he underwent radiofrequency (RF) hyperthermia with intraperitoneal cisplatin a total of seven times. He continued on a combination of uracil and tegafur (UFT) administered orally with a protein-bound beta-D-glucan extracted from the mycelia of Cariolus versicolor (PSK). Long-term survival was achieved following the initial palliative gastrectomy despite simultaneous disseminated peritoneal metastasis.

Intraperitoneal thermochemotherapy for prevention of peritoneal recurrence of gastric cancer. Final results of a randomized controlled study.

BACKGROUND: Continuous hyperthermic peritoneal perfusion (CHPP) with a solution that contained 10 micrograms/ml mitomycin C was devised initially as a method for intraperitoneal thermochemotherapy. The authors conducted a randomized clinical trial to evaluate the efficacy of CHPP as a prophylactic treatment for prevention of peritoneal recurrence of gastric cancer with serosal invasion. METHODS: Between January 1983 and October 1986, 82 patients with gross serosal invasion but no gross peritoneal metastasis were divided by random sampling into two groups before undergoing potentially curative surgery for gastric cancer: 42 patients were scheduled to receive CHPP, whereas 40 were not scheduled to receive this treatment. CHPP was administered immediately after closing the abdomen after gastric resections while the patients were still on the operating table under general anesthesia. RESULTS: The 5-year survival rate (64.2%) of patients in the CHPP group was higher than that (52.5%) of patients in the control group although the difference was not significant. Of several patterns of cancer recurrence, peritoneal recurrence was more frequent in the control group than in the CHPP group. The mortality rate from peritoneal recurrence in the case of patients in the CHPP group was much lower than that of patients in the control group (P = 0.0854). CHPP did not induce anastomotic breakdown or chemical peritonitis after surgery. CONCLUSIONS: The results indicate that CHPP is effective in preventing peritoneal recurrence of gastric cancer with serosal invasion, which is highly likely to reappear in the peritoneum.

CA72-4 compared with carcinoembryonic antigen as a tumour marker for gastric cancer.

Serum levels of a newly identified, tumour-associated antigen, CA72-4, were measured in 86 patients with histologically proven gastric carcinoma. Preoperative levels of CA72-4 in serum tended to be higher with increased dissemination of the cancer. Elevated levels of CA72-4 (above 5.0 U/ml) were significantly more frequent than those of carcinoembryonic antigen (CEA) (above 5.6 ng/ml) in patients with stage III or IV (P less than 0.01) carcinoma, in patients with Borrmann type 4 (P less than 0.01), and in patients with peritoneal metastasis (P less than 0.01). No correlation was seen between serum levels of CA72-4 and those of CEA. Serum levels of CA72-4 were lower 1 month after gastrectomy in 25 of 39 patients with resected cancers. In each of 4 patients with recurrence, lower levels of CA72-4 after gastrectomy were replaced by elevated levels on detection of the recurrence of cancer. These results indicate that CA72-4 is highly specific to gastric cancer and may be more reliable as a tumour marker than CEA for gastric cancer.

Vertical Banded Gastroplasty for Sleep Apnea Syndrome Associated with Morbid Obesity.

We report a case of morbid obesity accompanied by obstructive sleep apnea syndrome (SAS) and obesity hypoventilation syndrome (OHS). Satisfactory weight control was obtained without significant surgical complications after vertical banded gastroplasty. With the reduction in weight, the symptoms of SAS and OHS, as well as several other complications caused by the severe obesity, disappeared. Quality of life also improved remarkably, as exhibited by improved activity performance and disappearance of irritability at waking. Thus, it appears that vertical banded gastroplasty is efficacious in the treatment of morbid obesity with sleep apnea and hypoventilation.

A phase II pilot study of the combined application of hyperthermia and intra-hepato-arterial chemotherapy using cisplatinum and 5-fluorouracil.

Fourteen patients (group A) with unresectable metastasis to the liver from colorectal cancers (11 patients) and gastric cancers (3 patients) were treated with the combined application of hyperthermia and intra-hepato-arterial (IHA) chemotherapy with cisplatinum and 5-fluorouracil. Ten patients were treated with regional hyperthermia applied with a radiofrequency capacitive heating system and four patients were treated with total-body hyperthermia concurrently with IHA chemotherapy. Thirty-one patients (group B) with liver metastases from colorectal and gastric cancers received only IHA chemotherapy with the same chemotherapeutic regimen. In group B, partial responses (PRs) were obtained in 8 of 17 evaluable patients with colorectal cancer and in 1 of 10 evaluable patients with gastric cancer. In group A, PRs were found in 6 of 11 patients with colorectal cancer and in 2 of 3 patients with gastric cancer. Furthermore, improvements due to the combination of chemotherapy with hyperthermia were noted in 3 of 6 patients with colorectal cancer who had received unsuccessful prior IHA chemotherapy. The 50% survival period was prolonged to 23 months in group A from 11 months in group B, for patients with colorectal cancer. Toxicity of IHA chemotherapy was not potentiated by combination with hyperthermia. These results indicate that the combination of hyperthermia with IHA chemotherapy has a therapeutic benefit in the treatment of unresectable metastatic liver tumors derived from colorectal and gastric cancers.

Heating efficiency of radiofrequency capacitive hyperthermia for treatment of deep-seated tumors in the peritoneal cavity.

We analyzed heating profiles from 318 hyperthermic treatments of 39 patients with recurrent or inoperable cancers of the digestive organs whose deep-seated tumors were treated by radiofrequency (RF) capacitive heating of the abdominal region, and we investigated the heating efficiency and antitumor effect of such treatment. It was apparent that heating with a mean maximum RF output of 1,000 watts (700 watts at least), repeated four times or more, was necessary for a high rate of response by the tumor. Although it was difficult to heat tumors of the bile duct/pancreas to 42 degrees C or more, there was a strong positive correlation between maximum output of RF energy and maximum temperature of tumors (r = 0.839, P less than 0.001). The antitumor effect of RF hyperthermia was augmented with increasing output of RF energy. Therefore, the maximum level of RF output may be a useful index for expressing the heating efficiency with respect to intra-abdominal deep-seated tumors.

Gastric leiomyoblastoma in childhood.

A case of gastric leiomyoblastoma in a 7-year-old girl, who underwent partial gastrectomy combined with complete removal of perigastric lymph nodes, is reported. Characteristics of the behavior and prognosis of gastric leiomyoblastomas in childhood are discussed in a review of the world literature.

Diagnostic accuracy of combination of assays for immunosuppressive acidic protein and carcinoembryonic antigen in detection of recurrence of gastric cancer.

Two tumour markers, immunosuppressive acidic protein (IAP) and carcinoembryonic antigen (CEA), were assayed in gastric cancer patients. Levels of IAP and CEA were measured simultaneously in the preoperative and postoperative periods. The usefulness of the combined assay of these markers for detection of recurrence of cancer was investigated in terms of sensitivity, specificity and diagnostic accuracy. Sensitivity was not high (69.2%), but specificity and diagnostic accuracy were 96.7% and 86.9%, respectively. In cases with metastases in the liver, sensitivity (100.0%), specificity (100.0%) and diagnostic accuracy were high. In cases of peritoneal dissemination, these indices were low. The combination assay of IAP and CEA appears to be useful for detection of recurrence of gastric cancer, especially in patients with liver metastases.
