Effects of six conditions of ocular fixation on caloric nystagmus.

Caloric tests were performed on 20 normal volunteers under the following six conditions of ocular fixation: (1) eyes open with lighted +20-diopter lenses; (2) eyes open with lighted +20-diopter lenses plus strobe lighting; (3) eyes open in darkness; (4) eyes closed in darkness; (5) eyes closed in a dimly lighted room; and (6) eyes open with unlighted +20-diopter lenses plus strobe lighting. Latency, amplitude, frequency, duration, and speed of the slow component were measured from a permanent recording. Significant differences were found between the lighted +20-diopter lens conditions and all others for all variables except latency. Tests performed with lighted +20-diopter lenses should be cautiously interpreted due to the noticeable response suppression noted in these conditions. The present procedure of performing caloric tests with the eyes closed in a dimly lighted room was found to be unqeustionably preferable.

Performance of male and female medical students in a medicine clerkship.

Little has been published about objective evaluation and comparison of performance of female and male medical students in clinical clerkships. Data were collected on objective grading and examination results of a three-month medicine clerkship involving 269 medical students, 222 men and 47 women, during 1975 and 1976. There were no significant differences in final course grades between the two sexes. Performance in wards and clinics and on written and oral examinations was also without significant differences when the two sexes' scores were compared. Thus, gender would appear to have no relationship to medical student performance in this clinical clerkship.

Childhood cancer: Parental discord and divorce.

Parents of 191 children who were treated for cancer over a seven-year period completed and returned a questionnaire concerning marital status. Contrary to the prediction of a high divorce rate among these families, the study found a person-year divorce rate of 1.19%. This is slightly lower than the 2.03% person-year divorce rate among married couples with children in the states of Kansas and Missouri. The Arnold sign indicator analysis of the Minnesota multiphasic personality inventory was used to measure marital stress in 38 intact couples whose children were treated for cancer. The study sample experienced more stress than a comparison group of 23 couples who had hemophilic children. Both of these populations had more stress than a normal standardization group but less than a standardization group of marriage counselees.

Amitriptyline in the prophylaxis of migraine. Effectiveness and relationship of antimigraine and antidepressant effects.

Amitriptyline was evaluated as a prophylactic antimigraine agent in 110 patients with severe migraine. This agent improved the migraine more than 50 percent in 72 percent of patients and more than 80 percent in 57 percent of patients. Most of the 31 patients with less than 50 percent improvement had virtually no response. Depression, measured with the Zung Self-Rating Depression Scale, was absent in 40 patients, borderline in 53, and moderate to severe in 17. Overall, depression ratings improved minimally with therapy. There was a weak relationship between improvement in depression and improvement in migraine. Subgroups with a stronger correlation of these could not be found. This work suggests that amitriptyline is effective in migraine prophylaxis and that it has a primary effect on migraine that is relatively independent of its antidepressant action.

Comparison of five glucose meters for self-monitoring of blood glucose by diabetic patients.

This study collected data regarding the accuracy of five currently marketed meters for home use of self-monitoring of blood glucose (SMBG). Data regarding ease of operation of each glucose meter as well as cost and availability of meters and necessary materials were compiled. Fasting blood glucose (BG) samples were drawn from patients in the adult and pediatric diabetes clinics at the University of Kansas College of Health Sciences and Hospital. Specimens were tested by a clinical laboratory as well as by each meter. Unadjusted meter readings of whole BG were plotted against laboratory readings of serum glucose and displayed in scattergrams. Scattergrams showed decreased machine accuracy in upper and lower BG ranges. Whole BG values (meter readings) were adjusted to serum glucose levels by multiplication by 1.12. Adjusted values were compared with laboratory serum values using ANOVA for repeated measures and Dunnett's Multiple Comparisons Test: ANOVA and Dunnett's Multiple Comparisons Test showed only Glucochek II (Medistron, Ltd., West Sussex, England; distributed in U.S. by Larken Industries, Lenexa, Indiana) using Dextrostix reagent strips (Ames Division, Miles Laboratories, Elkhart, Indiana) having a statistically significant difference from laboratory values (P less than 0.01). This meter was recently revised and may show changed accuracy readings. Each meter exhibited inherent advantages and disadvantages regarding price, calibration, strip utilization, and ease of operation, which must be considered before recommendation by the health care provider.

A morphologic analysis of the cells of ductal carcinoma of the breast and of adenocarcinoma of the ovary in pleural and abdominal effusions.

The cells in serous effusions from 14 cases of histologically proven ductal adenocarcinoma of the breast and from an equal number of proven adenocarcinomas of the ovary were morphologically analyzed, with quantitative values assigned to various cell parameters. The resulting data were then subjected to stepwise discriminant analysis. The presence of coarse chromatin proved to be the most significant discriminating factor, resulting in a correct classification of 100% of the mammary carcinomas (14 of 14) and 71.4% (10 of 14) of the ovarian carcinomas, for an overall rate of 85.7% correct classifications. When combined with other cell parameters, such as the presence of polygonal cells, multiple nucleoli and an isolated cell arrangement, the accuracy rate increased to 92.9% correct classifications. Ovarian carcinoma was more likely to have coarse chromatin and to occur in three-dimensional cell groups, in contrast to mammary carcinoma, which tended to have fine chromatin and more polygonal cell shapes and to occur more commonly in syncytial cell groups and as isolated cells.

Growth patterns for infants weighing less than 801 grams at birth to 3 years of age.

Anthropometric measurements of infants weighing < 801 gm were monitored to evaluate growth patterns from birth to 36 months chronologic age, using both standard growth percentiles and Z score descriptors. Mean gestational age was 25.8 weeks, and birth weight was 708.4 gm. The infants had growth delay at hospital discharge with some catch-up growth occurring in the first year. Discrete changes in body growth using the Z score as a descriptor showed movement from a negative SD at 12 months toward the reference mean of zero at 36 months. However, mean length remained < 5th percentile, and mean weight was at approximately the 10th percentile for chronologic age. At 36 months 45% of infants had a head circumference smaller than the 5th percentile for chronologic age. Birth weight < 701 gm was a significant predictor of smaller occipital-frontal circumference at 3 years (p = 0.03). Interruption of intrauterine growth after extremely premature birth appears to have long-term effects on growth outcome much like prenatal growth retardation.

Determinants of exercise and aerobic fitness in outpatients with arthritis.

Factors that influenced exercise behaviors and aerobic fitness were identified in 100 outpatients with rheumatoid arthritis or osteoarthritis. Data included perceived health status, benefits of and barriers to exercise, and impact of arthritis on health; demographic and biologic characteristics; and past exercise behavior. Exercise measures included range-of-motion and strengthening exercises, 7-day activity recall, and the exercise subscale of the Health-Promoting Lifestyle Profile. An aerobic fitness level was obtained on each subject by bicycle ergometer testing. The theoretical model predicted 20% of the variance in composite exercise scores but none of the variance in aerobic fitness levels. Perceived benefits of exercise was a significant predictor of exercise participation. Subjects with less formal education, longer duration of arthritis, and higher impact of arthritis scores perceived fewer benefits of exercise, while subjects who reported exercising in their youth perceived more benefits of exercise.
