Solitary actinic porokeratosis.

An 83-year-old man experienced abrupt onset of a solitary lesion of porokeratosis on his bald, actinically damaged scalp. The features in this case are unusual and represent another point in the broad spectrum of lesions characterized by the cornoid lamella of porokeratosis.

The influence of embryology of the mid-face on the spread of epithelial malignancies.

A review of 150 cases of mid-face skin cancers treated by the fresh tissue technique of microscopic controlled excision has revealed local epithelial cancer spread to be markedly influenced by embryological fusion planes. Knowledge of facial embryology which is reviewed in this article should allow the surgeon to better predict and treat mid-facial skin cancer.

Multiple tumors arising in nevus sebaceus.

A forty-three year old woman with nevus sebaceus of Jadassohn involving the scalp is described. The unusual features of this case include the presence of adenoidal basal cell epithelioma, pigmented basal cell epithelioma, syringocystadenoma papilliferum, and nevocytic nevus. This case suggests the importance of prophylactic removal and close follow-up of nevus sebaceus.

Combined modalities in the management of hypertrophic scars and keloids.

A surgeon has many treatment options available during the longterm management of a patient with keloids or hypertrophic scars. The thoughtful combination of multiple modalities in a staged manner may produce a more favorable result than the application of a single technique. A case is presented which illustrates the concept of preliminary treatment with cryotherapy and low dose intralesional triamcinolone followed at interval by surgical excision.

[Microscopically controlled excision of malignant eyelid tumors and subsequent reconstruction (author's transl)].

35 patients, most of whom had recurrent or large malignant eyelid tumors (33 basal cell carcinomas and 2 squamous cell carcinomas), underwent microscopically controlled excision (Mohs' fresh tissue technique). This approach guarantees a complete removal of the tumor, while conserving as much healthy tissue as possible. Short time follow-up studies have shown that plastic reconstruction gave good functional and cosmetic results.

A multispecialty approach to the excision and reconstruction of eyelid tumors.

An ongoing study indicates that the Mohs' fresh tissue technique of tumor removal followed by eyelid reconstruction may be the most efficacious management of eyelid malignancies. Forty-seven recurrent, invasive or large eyelid and canthal tumors were excised by this method followed by eyelid reconstruction. The advantages of this approach are (1) a high cure rate, (2) conservation of tissue, (3) reduced operating room time, and (4) maximum utilization of specialty training.

Microscopically controlled excision of malignant neoplasms on and around eyelids followed by immediate surgical reconstruction.

Mohs' fresh-tissue technique of ablating malignant neoplasms is a highly reliable, yet conservative method of treating cutaneous cancers. Removal of such lesions on eyelids by this technique, followed by immediate reconstruction is a most efficacious way to manage them. Forty-seven cases of recurrent, invasive, or large carcinomas on eyelids and canthi were excised by this method followed by immediate reconstruction. The excisions were done by dermatologists specializing in Mohs' techniques and the reconstruction by an oculoplastic surgeon. The advantages of the combined procedures are: 1) high cure rate, 2) conservation of tissue, 3) reduced operating time, and 4) maximum utilization of specialized competence.

The use of the cryostat for histographic surgery.

Microscopically monitored excision for the treatment of skin cancer is becoming increasingly popular among dermatologists. One decision faced by beginning histographic surgeons involves the choice of apparatus for making the frozen sections. This paper compares the freezing microtome with the cryostat and gives details of the University of Iowa method, which employs the Slee cryostat.

Nodular grouped milia.

A young girl had a distinctive nodule with white spherules. Histologically, the lesion was composed of multiple small keratotic cysts.

Foot ulceration and vertebral osteomyelitis with Corynebacterium haemolyticum.

Diphtheroid or "coryneform" bacilli are usually considered to be nonpathogenic "normal flora" of human skin and mucous membranes. Because bacterial cultures are frequently contaminated with these organisms, the correct diagnosis and proper treatment may be delayed by the failure to recognize serious infections caused by them. This is a report of a 71-year-old woman with a diabetic foot ulcer and Corynebacterium haemolyticum osteomyelitis with bacteremia.
