A retrospective morphological research is made on 11 patients with squamous cell carcinoma of the cervix for a three-years period of time (1995-1997). Preoperative radiotherapy (intracavitary radiotherapy with Cs and/or external radiation) is carried out. A control group of 10 patients with cancer of uterine cervix without preoperative radiotherapy is also studied. All of the women undergo a wider hysterectomy by the method of Piver class IV. The radiation alterations in the tumor and the surrounding normal uterine tissues are also researched. A full regression of the tumor Ts size. There are demonstrative changes in the tumor and surrounding normal tissues, which are the prove the answer to the ionizing irradiation.
The case of a 60-year-old female with severe and rapidly progressing Cushing's syndrome with fatal consequences is described. Both the clinical and the biochemical findings were consistent with ectopic ACTH production. A computed tomographic (CT) scan revealed intact pituitary and enlarged adrenal glands. Liver metastases were seen but the primary neoplasm was not found. Treatment with aminoglutethimide and other therapeutic measures were unsuccessful. A carcinoma of the right ovary was discovered on autopsy. The tumour cells showed immunoreactivity for neuron-specific enolase (NSE), ACTH and beta-endorphin with differing degree of intensity.
HLA DRB1, DQA1 and DQB1 alleles were determined by DNA PCR-SSO typing in a sample of 99 individuals originating from Wielkopolska (midwestern Poland). A high number of alleles (38 DRB1, 8 DQA1 and 14 DQB1) was detected at each locus, many of them presenting notable frequencies in this population. The three HLA loci are thus characterized by very high heterozygosity levels (93% for DRB1, 85% for DQA1, and 88% for DQB1), which confirms the results found for other European populations. A total of 6 DRB1-DQA1-DQB1 haplotypes are detected with an estimated frequency higher than 5%, namely, DRB1*1501-DQA1*0102-DQB1*0602, DRB1*0701-DQA1*0201-DQB1*0201, DRB1*0101-DQA1*0101-DQB1*0501, DRB1*1101-DQA1*0501-DQB1*0301, DRB1*03011-DQA1*0501-DQB1*0201, and DRB1*1301-DQA1*0103-DQB1*0603. A genetic distance analysis between the Polish and other world populations tested for HLA class II indicates that the Wielkopolska community is close to geographically close, rather than linguistically related populations from Europe. More generally, a good agreement between genetics and geography is found for DRB1 and DQB1 polymorphisms in Europe, suggesting that these two loci are highly informative for assessing historical relationships among humans.
Frequency and characteristics of mixed mesodermal tumors (MMT) and some terminology disparities in the comparatively scarce literature are discussed. The low frequency of MMT described and difficulties in diagnosing have been pointed out. A final diagnosis can be made on operative material only, since curettage contains one (more often the epithelial) of the components of biphase tumorous. The authors emphasize on guided attention to MMT, as well as the necessity of serial sections. The most important factor is depth of invasion.
We describe for the first time the use of PCR based techniques to analyze the MHC class II polymorphism of the Bulgarian population. The present study provides the HLA-DRB, DQB1 allele frequencies in 116 Bulgarian individuals and DQA1 alleles frequencies in 100 subjects. DNA from these individuals was typed for DRB and DQB1 typed by the PCR-Allele Specific Amplification (PCR-ASA) method and DQA1 by PCR followed by hybridization using Sequence Specific Oligonucleotides (PCR-SSO). Allele and haplotype frequencies and linkage disequilibria are computed by the standard methods used for the XIth International Histocompatibility Workshop. The highest frequencies are 0.159, 0.109 and 0.085 for DRB1*1101, DRB1*1601 and DRB1*1301 respectively. Among the eight DQA1 alleles detected, DQA1*0501 (0.344) is found to be much more frequent than the two most frequent alleles DQA1*0102 (0.225) and DQA1*0101 (0.151). Twelve DQB1 alleles are found and three of them, DQB1*0301 (0.280), DQB1*0502 (0.153) and DQB1*0201 (0.133) showed the highest frequencies. The haplotype DRB1*1101-DQA1*0501-DQB1*0301 (0.079) predominate clearly, followed by DRB1*1601-DQA1*0102-DDQB1*0502 (0.055) and DRB1*0101-DQA1*0101-DQB1*0501. These results indicate that the Bulgarian population is characterized by features representative of the European anthropological type with a substantial contribution from the Southern Belt of Europe.
Molecular genotyping of HLA class II genes is commonly carried out using polymerase chain reaction (PCR) in combination with sequence-specific oligotyping (PCR-SSO) or a combination of the PCR and restriction fragment length polymorphism methods (PCR-RFLP). However, the identification of the DQB1 type by PCR-SSO and PCR-RFLP is very time-consuming which is disadvantageous for the typing of cadaveric organ donors. We have developed a DQB1 typing method using PCR in combination with allele-specific amplification (PCR-ASA), which allows the identification of the 17 most frequent alleles in one step using seven amplification mixtures. PCR allele-specific amplification HLA-DQB1 typing is easy to perform, and the results are easy to interpret in routine clinical practice. The PCR-ASA method is therefore better suited to DQB1 typing for organ transplantation than other methods.
HLA class-II allelic diversity is commonly defined using polymerase chain reaction (PCR) in combination with sequence-specific oligotyping (PCR-SSO) or the combination of PCR and restriction fragment length polymorphism methods (PCR-RFLP). Nevertheless, the identification of the DRB polymorphism by PCR-SSO is a time-consuming procedure and the PCR-RFLP is cumbersome. A rapid technique which allows a precise and extensive HLA-DRB typing is required, particularly in order to study the role of class-II matching in organ transplantation. A DRB typing method based on the detection and length of PCR products amplified using combination of allele specific primers has been developed. Thirty-four DRB alleles (29 DRB1, 4DRB3, 1DRB4) can be detected using 29 primers distributed into 19 amplification mixtures.
Four murine hybridoma clones secreting monoclonal antibodies (mAb) directed against lipopolysaccharide (LPS) antigen of S. newington, serogroup E1, were obtained after a fusion of spleen cells of mice immunized with formaldehyde-killed bacteria and mouse myeloma cells of the X63-Ag8.653 line. Antigen binding properties and specificity of the mAb were studied in bacterial agglutination tests, passive hemolysis and its inhibition, passive hemagglutination tests, passive hemolysis and its inhibition, passive hemagglutination and immunoenzyme tests (ELISA and immunoblotting). Three of the mAb (24E6, 29E1 and 45F6) were agglutinating and were active in all tests used, while mAb 31H12 did not agglutinate bacteria but revealed a high reactivity in the immunoenzyme reactions. It was found that the mAb reacted with LPS and Salmonella strains from serogroup E (E1, E2, E3 and E4) as well as from serogroups C (C1 and C4), F and S thus showing that the O3 antigen possesses more than one epitope, one of which is represented on the LPS antigens of the serovars from the cross-reacting groups mentioned. According to the known chemical the most probable recognized epitope consists of mannose with beta-linkage to the next monosaccharide residue in the LPS chain.
In 80 patients with Basedow's disease (20 nontreated and 60 under treatment with methimazole) were examined. The antibodies inhibiting the binding of TSH (TBII) by the radioreceptor method (Thyrotropin-Receptor-Anti-Körper Assay = TRAK), TRH-test, T3, T4, FT4, FT. TRAK-positive) values above 15 u/l) were 80% of the nontreated and 20% of the treated patients. In the course of 3 up to 6 months from the start of the treatment 60% of the examined patients were negative and at the end of the 18th month--92% were negative. A significant difference in the frequency of the recurrences was found between the TRAK-positive and the TRAK-negative patients. Between the TRAK, FT3, FT4, the size of the struma, the presence of ophthalmopathy [correction of ophthalmycthere] are positive relations. The following conclusions are made: 1. The determination of TBII and TRAK is not only of diagnostic but also of prognostic importance. 2. Methimazole exerts a specific immunosuppressive action and can lead to an immunologic remission in a comparatively short time.
In order to determine the role of thrombosis in the acute coronary syndromes the blood levels of fibrinopeptide A and protein C were examined with an enzyme-immune test in 48 patients treated in the cardiological clinic of the National Centre for Cardiovascular Diseases. 27 patients were with transmural myocardial infarction and 21 patients were with non-transmural myocardial infarction. The average time of the test from the onset of pain is 18.4 +/- 12.2 hours (from 3 up to 72 hours). The mean level for fibrinopeptide A for the whole group of patients is 4.95 +/- 3.1 ng/ml and that of protein C is 70.1 +/- 9.8%. For the group of patients with transmural myocardial infarction the level of fibrinopeptide A is 6.09 +/- 3.49 ng/ml and of protein C is 65.3 +/- 8.0%. For the patients with nontransmural myocardial infarction the levels are respectively 3.49 +/- 1.7 ng/ml for fibrinopeptide A and 76.3 +/- 8.3% for protein C. The difference between the two groups is statistically significant (p less than 0.005). In the patients with non-transmural myocardial infarction from whom the blood for the test was taken before the 24th hour the fibrinopeptide A level is 4.8 +/- 2.4 ng/ml and the protein C level is 69.0 +/- 7.8%. The deviations from the reference group are statistically significant (p less than 0.04). The practical importance of these results is discussed.
A case is presented of a woman with a breast cancer who developed Basedow's disease several months after the diagnosis of the cancer. In spite of the antithyrotoxic treatment the Basedow's symptoms persisted till the end of life. The case is compared with other similar cases in which the combination of Basedow's disease with a metastatic tumour of the thyroid gland is not described. Probably the tumour invasion of the thyroid gland triggers the appearance of various clinical syndromes of hyperthyroidism including the full classical picture of Basedow's disease as is the case in the reported patient. The intimate etiopathogenetic mechanisms of these thyrotoxic syndromes are not yet clear.
Acupuncture of 50 persons suffering from myopia, glaucoma, retinitis pigmentosa (degeneratio retinae pigmentosa), optic nerve atrophy, aged 5 to 71 was carried out. One to three courses were conducted, each consisting of 10-15 seances. All the patients were treated with acupuncture at these bilateral points: Extra 2, BL 2, GB 14, 4, TH 17, St 1. In cases of optic nerve atrophy, the authors also needled points T 15 and BL 60. By indirect electroacupuncture (by Chinese apparatus WQ 10), points BL 2 and St 1 were also stimulated. In all the patients, a subjective improvement of visual acuity was observed. In children with myopia, relative accommodation was enlarged. In patients with retinitis pigmentosa, an enlargement of the borders of the visual field, investigated by perimetry and isopter perimetry was observed, as well as light difference sensitivity established by static perimetry. Electrooculograms showed a reduction of the basic values and of Arden's ratio. 3 of the patients with glaucoma had a decrease in pressure of the eye, measured by Maklakow's tonometer.
Some lipid indices were studied in the course of acute bacterial and viral pneumonias in 102 patients, aged from 18 to 45. Statistically significant increase of total lipids, NEFA and EFA was established as well as insignificant increase of triglycerides and significant decrease of alpha-lipoproteins. Bacterial pneumonias are distinguished by a more manifested increase of total lipids and EFA, the viral pneumonias--by a more markedly decrease of alpha-lipoproteins. The discrepancies are statistically significant. The changes are of a transitory character and on the 40 th day of the treatment significant tendencies for normalization of the indices changed were established.
Various gram-negative bacteria were subjected to mild acid hydrolysis. The acid-treated bacteria exhibited strong cross-reactivity with fluorescein isothiocyanate-conjugated antiserum to the Re mutant of Salmonella minnesota. Hydrolyzed bacteria showed considerably stronger fluorescence than heat-treated bacteria. It is assumed that acid hydrolysis uncovers shared glycolipid determinants that are responsible for cross-reactivity. Isolated homologous and heterologous lipopolysaccharides were allowed to react with antibody to S. minnesota Re insolublized by covalent binding to aminohexyl Sepharose 4B. The resulting antigen-antibody complexes were visualized by exposure to the fluorescent antiserum. This treatment allows the demonstration of glycolipid structures of intact lipopolysaccharides.
Lipopolysaccharides (LPS) derived from a single Salmonella serotype or a combination of serotypes were submitted to insolubilization by means of glutaraldehyde. Cross-linked LPS proved to be efficient immunoadsorbents. They were employed for the removal of cross-reacting anti-O antibodies from various Salmonella antisera. LPS immunoadsorbents offer a number of advantages over bacterial cells concerning the preparation of specific bacterial antisera: they regain the original antibody binding capacity after tryptic digestion of the anti-LPS antibodies adsorbed and can be repeatedly used (15 times at least), adsorption of the sera is complete within 30 min, and the procedure is simplified. Prolonged storage (over 3 years) has no deleterious effect on the adsorption properties of the immunoadsorbents. Various applications of LPS immunoadsorbents are described.
