The relative importance of HCO3- and blood flow in the protection of rat gastric mucosa during shock.

Recent research has shown adequate mucosal blood flow and blood bicarbonate availability to be crucial in the prevention of gastric ulceration, yet the relative importance of these two factors is unknown. We studied the incidence of ulceration in rats subjected to hypovolemic shock under varying conditions of blood flow and acid-base balance. At the start of the experiment 2 ml of 0.1 M HCl were instilled into the stomach. In the control series, severe systemic acidosis had developed after 2 h of shock (B.P. 40 mmHg) and 6 of 6 stomachs ulcerated. Mucosal blood flow, measured with radioactive microspheres, had drastically decreased. Prostacyclin dissolved in 0.01 M phosphate improved gastric blood flow but did not help to correct acidosis, and 6 of 8 stomachs ulcerated. A higher degree of protection was observed when the prostaglandin was dissolved in 0.16 M HCO3-. Intraarterial infusion of high concentrations of bicarbonate completely eliminated acidosis with ulceration occurring in 1 of 6 rats infused with 0.5 M and in 1 of 10 with 1 M bicarbonate. Blocking the carbonic anhydrase with acetazolamide (100 mg/kg) completely prevented the protective effect of a 1 M HCO3- infusion, and 6 out of 8 stomachs ulcerated. Substituting TRIS buffer for bicarbonate in the infusion also eliminated acidosis, but neither concentrations of 1 M or 5 M buffers prevented the stomachs from ulceration (6 out of 6 with 1 M buffer and 5 of 6 with 5 M buffer). We conclude that bicarbonate is the essential factor in prevention of ulceration in our shock model and that blood flow is also important as one component of a bicarbonate-dependent protection system involving carbonic anhydrase.

[Oestrogen receptors and prognosis in breast cancer].

The determination of oestrogen receptors seems to be of prognostic value in the radically-operated breast cancer patient. Patients with negative receptor values exhibit early recurrence in a significantly higher percentage of cases than those with positive values. The prognostic implications of oestrogen receptor determination seem to be even higher than an assessment of axillary lymph node involvement. In metastatic breast carcinoma a correlation exists between response to hormonal therapy and receptor assay. The clinical implications for therapeutic management are discussed.

[Papillary carcinoma of the thyroid gland (author's transl)].

Records were examined of 174 patients with papillary thyroid carcinoma operated on at the 1st Department of Surgery of the University Hospital in Vienna between 1948 and 1976. The overall mortality rates amount to 20 +/- 8% after 5 years, rising to 70 +/- 21% after 20 years. Clear relationships are demonstrated between lymph node metastases, distant metastases, extrathyroid tumours and higher mortality rates. An older age correlates in the extrathyroid tumour group only with a higher risk of death. In our material reclassified by the criteria of the WHO classification (1974) between 1965 and 1976 the incidence of papillary carcinoma is 25% of all thyroid carcinomas. The incidence of this tumour subgroup varies in the literature between 25 and 90% and is influenced by regional differences in the use of childhood radiation on the one hand and iodine uptake on the other hand, different methods of thyroid examination and the subjectivity of the pathologist.

[Anaplastic spindle and giant cell carcinoma of the thyroid (author's transl)].

77 cases of spindle and giant cell carcinoma of the thyroid were reviewed. This rapidly fata neoplasm extended beyond the capsule of the thyroid gland in all cases but one, with infiltration of the trachea or oesophagus in 3/4 of these cases at the time of diagnosis. 60% of the patients had lymph node metastases and 26% distant metastases. Due to the advanced stage of the primary tumour at the time of presentation, total resection was feasible in 13% of cases only. The prognosis is very poor; all patients died within 18 months. Correlations were demonstrated between lymph node metastases, distant metastases, tumour infiltration of trachea or oesophagus and shorter survival times. The survival time is prolonged in patients with partial tumour resection by adjuvant radiotherapy. 28% of all thyroid carcinomas seen at the 1st Department of Surgery of the University Hospital in Vienna between 1965 and 1976 were anaplastic. The high incidence in Austria is caused, in our opinion, by the higher rate of anaplastic transformation of long-standing, differentiated carcinomas in adenomatous goitres, which remain undetected clinically for years in this endemic goitre region because of their slow rate of growth.

Results with tamoxifen in advanced mammary carcinoma.

The results of a study with tamoxifen (TAM) (10 mg three times daily) in advanced breast cancer are reported. Of 44 patients, 24 showed a remission and 20 a progression. Of the 17 patients in whom an oestrogen receptor (ER) determination in tumour tissue had been carried out, 12 were receptor-positive (ER+) and five receptor-negative (ER-). Nine of the 12 ER+ cases (75%) responded to the treatment with TAM. Of the five ER- patients, one showed remission and four progression.

[The influence of prostacyclin on gastrointestinal blood flow during hypovolemic shock (author's transl)].

The influence of a continuous PGI 2 infusion on blood flow of various organs was tested in normovolemia and in hemorrhagic shock. PGI 2 increased the blood flow of stomach, pancreas and jejunum in normovolemic animals. The reduced blood flow due to hemorrhagic shock was increased to preshock values in stomach, pancreas, and jejunum. A linear correlation was found between the dose of PGI 2 and mucosal blood flow of the stomach.

[Prognostic value of estrogen receptors in breast cancer (author's transl)].

One hundred and ninety-one patients with operable breast cancer were followed for up to 34 months after operation and the early recurrence of disease was noted in relation to the presence or absence of estrogen receptor. Recurrence rates were significantly higher in patients whose tumors did not contain receptors than in those whose tumors did. The response to cytotoxic chemotherapy in patients with advanced breast carcinoma seems to be independent of the presence of E2R in cancer tissue.

Regional gastric mucosal blood flow in hypovolemic shock.

The effect of hypovolemia on the mucosal blood flow of the gastric corpus and antrum were tested with the microsphere method 30 and 60 min after induction of hemorrhagic shock in the rat. The results demonstrate a marked reduction after 30 and 60 min without any significant differences between the corpus and the antrum. This reduction is largely dependent on cardiac and circulatory function. It is concluded that the differences in energy metabolism, which have been implicated as explanation for the different susceptibility to develop stress lesions by Menguy and Masters, cannot be attributed tod different degrees of ischemia.

[Prognostic factors in the drug therapy of metastasizing breast cancer].

175 patients with metastatic breast cancer, treated with chemotherapy, were analyzed retrospectively to identify the characteristics of prognostic importance in predicting response to chemotherapy and survival from onset of the chemotherapy. The most significant factors were the sites of metastatic disease and an estimate of the total extent of disease.

[Various amounts of estrogen receptors in breast neoplasms].

An agar gel electrophoresis was performed to demonstrate various amounts of estrogen receptors of three different topics in 38 mammary carcinomas. A good correlation between estrogen receptors in primary mammary carcinomas and axillary lymph node metastases could be found.

[Topical differences in estrogen receptor content of primary breast cancer].

In 50 primary cancers of the mammary gland, tissue samples from three different parts of each tumor were analyzed for estrogen content by agar gel electrophoresis. In 12 tumors receptor-rich and receptor-poor zones were detected within the tumor for the 4-s fraction, and in 11 tumors for the 8-s fraction. Confidence limits (99%) indicate that these findings might range from 10.4% to 42.5% for the 4-s fraction, and 10.2%-44.1% for the 8-s fraction.

[Pharmacological modification of regional blood flow in rat stomach mucosa].

The effect of three agents with antisecretory function on regional gastric mucosal blood flow was measured using the microspheres technique. Salbutamol caused a significant decrease in blood flow, while DMPGE2 caused a significant increase. After calcitonin no change was observed. This is contrary to the belief that inhibition of acid secretion is always associated with a decrease in gastric mucosal blood flow.

The structure of tight junctions in human thyroid tumors. A systematic freeze-fracture study.

Tight junctions of normal human epithelium comprise 6--8 intramembranous ridges. Tight junctions of adenomas and one case of follicular carcinoma were morphologically identical with normal controls. Another follicular carcinoma (clinically less malignant than the case mentioned above), however, showed loss of junctional polarity with spreading of ridges on lateral cell membranes. One case of papillary carcinoma (clinically low malignancy) displayed only focal attenuations of the tight junction belt. Another more malignant tumor of the same histologic type showed breaks of ridges and focal interruptions of the tight junction meshwork. Cell membranes of two anaplastic carcinomas of high malignancy were completely devoid of tight junction ridges. We conclude that tight junction alterations are not necessarily implicated in malignant transformation and that they correspond with tumor differentiation rather than directly with a single parameter of thyroid tumor malignancy.

[Regional gastric blood flow in hypovolemic shock in rats].

The effect of hypovolemia on the mucosal blood flow of gastric corpus and antrum were tested with the microspheres method 30 and 60 min after induction of shock in the rat. The results demonstrate a marked reduction after 30 min, with return to control values after 60 min, without any significant differences between corpus and antrum. It is concluded that the differences in energy metabolism between these two regions of gastric mucosa, cannot be attributed to different degrees of ischemia.

[Familial bilateral renal-cell carcinoma and cerebellar haemangioma (author's transl)].

In three brothers of a family with a high incidence of cancer a bilateral renal-cell carcinoma was demonstrated. In the index case there was also a cerebellar haemangioblastoma. At necropsy primary bilateral renal-cell carcinoma was demonstrated while intrarenal metastases were excluded. The cases are considered to be predominantly renal manifestations of the von Hippel-Lindau syndrome with facultative oculocerebellar involvement. Urological and cytogenetic study of other family members provided no evidence of further manifestations of the syndrome. Clinical supervision of members of such families is indicated.
