The male facelift. An analysis of 115 cases.

Based on the anatomic differences in female and male cervicofacial skin, a retrospective study of 115 male facelifts was undertaken to identify those factors that contributed to a satisfactory esthetic result and the avoidance of complications. Incision modifications are reported, as well as the need for ancillary procedures (in 37% of the patients) to obtain facial rejuvenation. An increased incidence of hematomas (9.6%) was noted.

The lower trapezius island musculocutaneous flap revisited. Report of 45 cases and a unifying concept of the vascular supply.

The lower trapezius island musculocutaneous flap is valuable in head and neck reconstruction. It offers thin, pliable tissue and a long arc of rotation to reach virtually any defect in the head and neck. Recent reports have shown unacceptably high rates of failure and have questioned the reliability of the vascularity. We report our experience with 45 reconstructions using the lower trapezius island musculocutaneous flap. The literature is reviewed and the vascular anatomy is described in detail. The concept of angiosomes is applied to the trapezius muscle to help explain the reasons for flap failure and to provide guidelines for flap design and harvest.

Aggressive juvenile fibromatosis involving the paranasal sinuses.

Aggressive juvenile fibromatosis is a destructive, locally invasive process, with a propensity for local recurrence after inadequate excision. Although it does not metastasize, uncontrolled growth can eventually lead to death. The role for radiation and chemotherapy is controversial and these modalities are best reserved for advanced unresectable or recurrent lesions. The treatment of choice is en bloc surgical resection with wide margins.

Combined median mandibulotomy and Weber-Fergusson maxillectomy.

Monoblock surgical resection in combination with radiotherapy remains the most effective method of treatment for advanced carcinoma of the maxillary sinus. Extension of antral carcinoma into the infratemporal and pterygopalatine fossae decreases the probability of achieving an all-encompassing resection via the classic anterolateral Weber-Fergusson approach because of limited posterior access. Temporal and lateral infratemporal fossa approaches have been described and involve either mobilization of the zygomatic arch or a lateral mandibular osteotomy, respectively. We report the use of a median labiomandibulotomy combined with the Weber-Fergusson approach for lesions extending posteriorly. The labiomandibulotomy with paralingual extension along the floor of the mouth allows rotation of the coronoid process laterally and exposes the infratemporal fossa to its posterior margin. We present two cases in which this approach was used to extirpate lesions that had extended posteriorly. Relevant anatomy, operative technique, potential complications, and limitations, as well as a comparison with other methods of exposure, are discussed.

The low plasma testosterone levels of young Indian infarct survivors are not due to a primary testicular defect.

A case control study was performed to determine whether the hypotestosteronaemia described in men with coronary artery disease could be the result of primary testicular dysfunction. Testicular function was assessed by comparing the response of 10 young Indian men with myocardial infarction to human chorionic gonadotrophin (HCG) injection to that of 10 healthy age and weight matched controls. The basal testosterone levels in the patients were significantly lower (12.71 +/- 1.36 nmol/l vs 16.51 +/- 0.79 nmol/l; P = 0.01) and the basal oestradiol levels significantly higher than the controls (120.67 +/- 8.81 pmol/l vs 94.05 +/- 8.23 pmol/l; P = 0.02). There was no difference in the sex hormone binding globulin concentrations. However, following HCG stimulation the patients demonstrated a normal response with a 2-fold increase in testosterone. There was no difference in the testosterone and oestradiol levels of the patients and controls following HCG stimulation. This normal response in our patients demonstrates that the hypotestosteronaemia in Indian men with myocardial infarction is not due to a primary testicular dysfunction but probably is a result of increased aromatization of testosterone to oestradiol.

Subjective past, present, and future fit: effects on anxiety depression, and other indicators of well-being.

Hypotheses were tested regarding the effects of subjective current, retrospected, and anticipated person-environment fit on affective and somatic strain and well-being. Two-hundred seven university students preparing for annual academic examinations at an Indian university completed self-administered questionnaires measuring parameters of the hypotheses. A distinction was made between cognitive (e.g., meeting demands for intelligence, good memory) and motivational (e.g., being able to muster the effort) fit. Cognitive poor fit had a greater effect on predicting high levels of strain across all time frames but did best in the past and current frames. Motivational poor fit had its greatest effect on strain when poor fit dealt with anticipated or current fit. These findings are discussed in terms of hypotheses, which suggest that the more subjectively uncontrollable a dimension of fit, the more salient it is in the past time frame, whereas the more subjectively controllable the dimension, the more salient it is in the future time frame.
