[Problems encountered by the anesthetist-intensive care specialist during aorto-coronary bypass surgery].

The authors analysed a series of 400 aorto-coronary bypasses performed between 1970 and April 1978. A notable improvement in the statistics was seen during the second period between 1976 and April 1978. During this period, 248 patients were operated upon with a mortality of 4 per cent. The progress made was in large part attributed to improved preoperative assessment, in particular a complete ventricular assessment, improved peroperative myocardial protection and the use of the intra-aortic counter-pressure balloon and of sodium nitroprussiate.

[Monitoring of anaesthesia breathing systems by capnography (author's transl)].

Continuous measurement of the CO2 concentration in inhaled gas is the best method to know accurately the importance of rebreathing. Four anesthetic breathing systems were studied in 60 adult patients (Dighby-Leigh valve, circle system with and without soda lime. Waters system without soda lime, Bain circuit). Fresh gas flows (FGF) inspired and end-expired concentrations of CO2 (FI CO2, FE' CO2) and PaCO2 are measured. The FI CO2 seems to be negligible with Dighby-Leign valve and circle system with soda lime but important with other systems, even with FGF as high as 200 ml/kg, the FI CO2 is about 1 p. cent for Bain circuit, 2 p. cent in spontaneous ventilation, 2,7 p. cent in controlled ventilation for Waters valve. It is possible to theoretically determine that the FI CO2 would not exceed 1 p. cent.

[Favorable course of a bilateral postoperative diaphragmatic paralysis].

Writing about a case of post-operative diaphragmatic bilateral palsy (bilateral thoracotomies for oesocoloplasty and oesophagectomy necessited by a caustic oesophagitis) on a 8 years old boy, palsy which was cured 6 months after, the authors tell about causes, prognostic and treatment of diaphragmatic palsies. In that observation, treatment consisted essentially in artificial ventilation for months and a half, and on intensive kinesitherapy.

[Peridural anesthesia for all women in labor: utopia or reality?].

Peridural anesthesia in obstetrics is more and more widely used, improving comfort and security for delivery. In 1989, 74% of the deliveries occurred under peridural anesthesia in our unit. In order to assess if this percentage can be increased, we analysed during one month the reasons why some women did not get a peridural anesthesia.

[Diagnosis of susceptibility to malignant hyperthermia in children].

The realisation and reliability of the halothane-caffeine contracture tests in children to detect the susceptibility to malignant hyperthermia (MH) is still controversial. The present study concerned 26 children aged 2 to 13 years, (mean 9.5 +/- 1.3 years) who were tested either because of a personal symptomatology (14 cases) or as a member of a susceptible MH family (12 cases). Half of the children had a positive test (MHS and MHE) as is found in adults. Furthermore comparison of threshold concentrations of halothane and caffeine as well as the 32 nmol caffeine-induced contractures dit not show significant differences related to age. These results support the possibility to perform under good conditions and with good reliability the diagnostic test of susceptibility to malignant hyperthermia in children from 2 years on.
