The effect of dipyridamole on experimentally induced stress ulcers.

Several doses of dipyridamomle were tested to assess a reduction in gastric bleeding and ulcer formation in rats exposed to cold stress restraint. When saline was compared to 12.5 mg/kg dipyridamole and 50 units/kg heparin, dipyridamole was significantly better than saline controls (P less than 0.05) in the reduction of both gastric bleeding and gastric ulceration, while the effects of heparin were not significant in either case. Finally, mean hematocrit, platelet count, and gastric acid secretion were not significantly affected by the dose of dipyridamole used (12.5 mg/kg). The results of this study show dipyridamole to be useful in the reduction of both stress ulceration and gastric bleeding induced in rats by cold stress restraint.

[Effect of postoperative long-term cancer chemotherapy (PLCC) on non-curative gastric cancer of stage IV].

We studied the effect of postoperative long-term cancer chemotherapy (PLCC) on the survival of the patients with stage IV gastric cancer, subjected to non-curative resection. The study was carried out with 90 patients receiving the PLCC from 1970 to 1978, and 95 patients received intraoperative one shot I.V. injection of Mitomycin-C from 1964 to 1978, and 106 patients without chemotherapy from 1964 to 1970. The PLCC consisted of the following combination: MMC intermittent intravenous injection (20 mg of MMC during operation followed by 10 mg on the next day, 4th week and every 3 months respectively): postoperative oral administration of FT-207 (600 mg/day) and; PSK (3 g/day) for more than 4 months after operation. The background factors were not significantly different among 3 groups. Two year survival rate was 18.9% in PLCC group, significantly higher than that of 7.4% in MMC group and 2.0% in no chemotherapy group which was respectively (P less than 0.05). The PLCC also demonstrated its be effective mess in prolonging the survival of the patients with hepatic metastasis or peritoneal dissemination.

Late results of postoperative long term cancer chemotherapy for advanced carcinoma of the stomach.

Postoperative long term cancer chemotherapy (PLCC) with the combination of Mitomycin-C, FT-207, a furanyl analog of 5-fluorouracil, and PSK, an immunopotentiator, was prescribed for patients with advanced gastric cancer. Five year survival rates for all stage III and stage IV patients were 52.8 and 19.3 per cent in the PLCC group. The rates were 26.7 and 2.2 per cent in the control groups (p less than 0.05). In curative cases of stage IV, the 5-year survival rate was 50.0% in the PLCC group while the rate was 11.1% in the controls. Mean survival time of patients with peritoneal dissemination or hepatic metastases was 12.8 and 10.9 months, respectively, for the PLCC group, in contrast to the lower 6.4 and 4.3 months for the controls. Thus, the 5-year survival rate of advanced gastric cancer patients in stage III and stage IV was markedly improved when these patients were treated with the protocol. Our findings clearly show that adjuvant chemotherapy should be administered for a long period postoperatively in order to achieve a significant improvement in patients with gastric cancer.

Critical evaluation of prophylactic splenectomy in total gastrectomy for the stomach cancer.

In gastric cancer surgery, an extended radical operation is commonly performed, and in cases of total gastrectomy, there is a tendency to perform splenectomy at the same time. However, some surgeons have reservations regarding this treatment in connection with the possible preservation of the host resistance. The question arises, is it reasonable to accept, by simple analogy with prophylactic lymphadenectomy the concept of the prophylactic splenectomy? The present study was designed to cast light on this problem. In order to examine the value of prophylactic splenectomy in gastric cancer surgery, a comparable patient group was followed up, and it was found that the non-splenectomized group showed a significantly better late survival rate than the splenectomized group (P less than 0.005), the 4-year survival rate being 63% in the former group and 36% in the latter group. Although these results do not necessarily contraindicate combined splenectomy, it seems desirable to reappraise the value of prophylactic splenectomy in cases having no metastasis in the splenic hilar and adjacent lymph nodes.

Enhancing effect of clamping of the portal vein on the effectiveness of antitumor agents against lymph node metastasis.

An experimental study was made on the suppressive effect on lymph node metastasis of an antitumor agent administered during the clamping of the portal vein. When mitomyaneously clamped, a higher concentration of the drug was detected in the mesenteric lymph nodes compared to the conventional intravenous administration. The growth of mesenteric lymph node metastasis was markedly suppressed by the combined use of mitomycin-C and portal vein clamping. These results suggest that the procedure applied in the present study directs a high concentration of antitumor preparations to the lymph nodes in the portal vein region, and thus is a good method for the suppression of lymph node metastasis.

Nine cases of acute gastric anisakiasis.

From 1983 to 1988, we treated 19 patients with acute gastritis which was highly suggestive of gastritis anisakiasis. Almost all of these patients complained of sharp, periodic, upper abdominal pains which occurred several hours after ingestion of raw fish. Among the 19 patients, 10 anisakis larvae from nine patients were successfully removed by endoscopic forceps. However, in the remaining 10 patients, who also showed the typical clinical course of anisakiasis, we could not find the anisakis larvae endoscopically. All 10 larvae were identified as stage III, anisakis type 1. The intolerable pain was relieved as soon as endoscopic removal of the larvae was accomplished in all nine patients. The purpose of this article was to report the clinical findings and emphasize the importance of early endoscopic management of acute gastric anisakiasis.

Bright loop appearance; a characteristic ultrasonography sign of early hepatocellular carcinoma.

Ultrasonography (US) and computed tomography (CT) are the most effective screening methodologies for hepatocellular carcinoma (HCC). In our US screening, 20% of small HCC nodules less than 20 mm in diameter were detected as hyperechoic tumors. Among these hyperechoic HCC nodules, we have often observed (BL) which is defined as hypoechoic nodules in the hyperechoic tumor. In this study, we report that the BL is a sign of dedifferentiation of early stage of HCC with fatty change by US. From 1994 to 1998, we performed tumor targeting needle biopsy in 938 hepatic nodular lesions. Among them, 284 nodules <20 mm in diameter, histologically diagnosed as HCC, were studied. BL is defined as a hyperechoic tumor containing a hypoechoic nodule >4 mm in diameter by US. Among 284 nodules, well, moderately and poorly differentiated HCC were 183 (64.4%), 100 (35.2%) and 1 (0.4%), respectively. On US, hypoechoic, isoechoic, and hyperechoic nodules were 188 (66.2%), 32 (11.3%) and 64 (22.5%), respectively. Forty-seven nodules of 64 hyperechoic HCC nodules <20 mm in diameter, 47 nodules (73.4%) showed fatty changes. Of 64 hyperechoic HCC nodules, we recognized 22 nodules (34.4%) as BL. The proportion of BL type hyperechoic nodules increased with the tumor size. Two hyperechoic nodules followed by US changed to BL with tumor enlargement. Histologic examination of a resected HCC with BL showed that hyperechoic HCC nodule represented well-differentiated HCC with fatty change and inner hypoechoic lesion represented moderately differentiated HCC without fatty change. In US screening for HCC, BL was often observed in HCC nodules from 11 to 20 mm in diameter. Histologic examination revealed that BL of HCC on US was associated with tumor progression and indicated dedifferentiation showing moderately differentiated HCC in well-differentiated HCC with fatty change.

Long-term follow-up of interferon-treated chronic hepatitis C and serum hepatic fibrosis markers.

BACKGROUND/AIMS: We investigated the clinical application of serum fibrosis markers in a long-term follow-up of patients with chronic hepatitis C treated with interferon-alpha. METHODOLOGY: This study included 52 patients treated with interferon-alpha (total: 480 MU) for 6 months. They each underwent liver biopsy before and after treatment. Twenty-eight patients who underwent liver biopsy less than 2 years after treatment were classified as group 1, and 24 patients as group 2. The two groups were subdivided into HCV RNA-negative responders and HCV RNA-positive nonresponders. Liver specimens were estimated using grading and staging scores. Serum hyaluronan, PIIIP, and type IV collagen levels were measured before and after treatment. RESULTS: In the responders of groups 1 and 2, grading score after treatment was significantly decreased compared with that before treatment. Staging score after treatment was significantly improved only in the responders of group 2. In the responders of group 2, serum hyaluronan level was significantly decreased compared with that before treatment. In group 2, the grading score was significantly correlated with serum PIIIP and type IV collagen levels, and the staging score was significantly correlated with only serum hyaluronan level. CONCLUSIONS: These findings indicate that the serum PIIIP and type IV collagen levels reflect the activity, and serum hyaluronan reflects the degree of fibrosis in liver specimens of HCV RNA-negative patients in a long-term follow-up of patients after interferon-alpha treatment.

Dipyridamole potentiates adriamycin cytotoxicity by a mechanism other than inhibiting nucleoside uptake.

Nitrobenzylthioinosine (NBTI) and dipyridamole (DP) are competitive inhibitors of cellular nucleoside uptake. Although combined treatment of HeLa cells with adriamycin (ADM) and DP enhanced ADM cytotoxicity in cell growth and clonogenic assays, the combination of ADM and noncytostatic levels (less than 1 microM) of NBTI did not change the cytotoxic potential of ADM in vitro. DP enhanced the inhibition of clonogenicity by ADM, even in nucleoside-enriched mediums. These results suggest that the synergy between ADM and DP was hardly due to the inhibition of nucleoside uptake by DP, but was due to the enhancement of intracellular ADM accumulation by DP.

Simultaneous adenocarcinoma of the stomach and fourth portion of the duodenum: case report and review of the literature.

We treated a woman with simultaneous adenocarcinoma of the stomach and fourth portion of the duodenum. She complained of symptoms of obstruction in the duodenum, and both lesions were correctly diagnosed, preoperatively. Noncurative resection was done because of distant lymph node metastasis. The new adjuvant chemotherapy using THP-adriamycin and UFT was prescribed, and one year after surgery she remains in recession.

Modulation of cytotoxic effect of anticancer drugs by dipyridamole in HeLa cells in vitro.

Exponentially growing HeLa cells were treated with various antitumor drugs and dipyridamole (DP), and the cell growth inhibition ratio was determined. Enhanced growth inhibition was found in combined treatment with DP and 5-fluorouracil, 1-hexylcarbamoyl-5-fluorouracil, methotrexate, adriamycin, daunomycin, 4'-0-tetrahydropyranyl-adriamycin, actinomycin D and vincristine. In contrast, reduction of the cytotoxic effect of cytosine arabinoside and enocitabine was found when these were combined with DP.

Gastrointestinal hemorrhage in end stage renal disease patients.

Over a six year span, 41 patients with end stage renal disease (ESRD) were treated for 53 episodes of upper and lower gastrointestinal (UGI & LGI) hemorrhage. Thirty-two patients (78%) were diagnosed as having UGI bleeding and nine patients as having LGI bleeding. Mucosal inflammation of the esophagus, stomach and duodenum were the predominant etiologies of UGI bleeding (64%). The predominant cause of LGI hemorrhage was diverticular disease (33%). Angiodysplasias were found in six patients (15%), four with UGI bleeding and two with LGI bleeding. In the UGI hemorrhage group 20% of the bleeding episodes were recurrent and 13% were from multiple sites. There were no recurrent bleeding episodes in the LGI hemorrhage group. We therefore conclude that UGI bleeding is much more common than LGI bleeding in ESRD patients, and that there is a significantly higher tendency for recurrent bleeding episodes and for multiple bleeding sites in the UGI hemorrhage patients. We also note a higher prevalence of angiodysplasias of the upper and lower gastrointestinal tract in ESRD patients.

Minute gastric carcinoid tumor with regional lymph node metastasis.

A minute carcinoid tumor of the stomach developed into a regional metastasis around the celiac axis. There was marked lymphangitic and venous invasion of the tumor cell cluster around the tumor, in the submucosal layer. We wish to emphasize that radical gastrectomy with extended lymphadenectomy should be performed, even for a minute carcinoid tumor, as there will probably be a lymph vessel-related metastasis.
