Familial polycythemia vera.

The occurrence of polycythemia vera in a father, mother, and two sons is reported. Thirteen kindreds with familial polycythemia vera in 31 members are reviewed. Comprehensive records were available for all four patients as well as other family members, since all were diagnosed and treated at the author's institution over a period of nearly 50 years. The mean age at diagnosis, sex predominance, symptoms, and incidence of chromosomal abnormalities, leukocytosis, thrombocytosis, and elevated leukocyte alkaline phosphatase levels were similar to those of nonfamilial cases. The mean RBC volume at diagnosis and the incidence of splenomegaly appear to be higher in familial than nonfamilial cases. The mode of inheritance is unclear, but genetic factors may be involved in the pathogenesis of this myeloproliferative disorder.

Reveal for Salmonella test system.

The Reveal for Salmonella (RSS) test system is a presumptive qualitative test that detects the presence of Salmonella organisms in foods within 21 h total testing time, allowing the user to release negative products 24 h earlier than when using other rapid test kits. Foods are enriched with a proprietary resuscitation medium called Revive and then selectively enriched with either Selenite Cystine or Rappaport-Vassiliadis selective media. The enriched culture is used to inoculate the RSS detection device, which initiates a lateral flow through a reagent zone containing anti-Salmonella antibodies conjugated to colloidal gold particles that capture antigens present in the culture. The antigen-antibody complex migrates farther and is captured by an additional anti-Salmonella antibody, causing the colloidal gold to precipitate and form a visual line, indicating a positive result. A procedural control line also will form regardless of the presence of Salmonella organisms to indicate the test is working properly. Existing AOAC Official Methods for Salmonella organisms require a 48 h enrichment before testing. Hence, a food product has to be held before release, adding extra cost to the company and the consumer. The RSS test system was evaluated by quantitative spiking studies. Although AOAC encourages inclusion of naturally contaminated foods, almost all microbiological AOAC validation studies have been performed with artificially contaminated foods for absolute control over the study. The RSS test system is designed to test many food types for Salmonella organisms and has a limit of detection of 5-10 colony-forming units (cfu)/25 g with a false-negative rate of < 1% and a false-positive rate of < 5.0%. It showed an 81% overall agreement with the traditional procedure of the U.S. Department of Agriculture's Food Safety Inspection Service.

Longitudinal electromyographic study of chewing patterns in complete-denture wearers.

A longitudinal electromyographic study of the anterior temporal and masseter activity during habitual chewing of apple was performed on 21 subjects provided with immediate complete dentures. At the pretreatment stage, when the patients having an anterior residual dentition were chewing, the peak mean voltages showed low values (range, 48 to 58 microV). After denture placement, there was a further decrease of the temporal activity that persisted to the 6-month stage. After the dentures were relined, a significant increase in chewing force of the temporal muscles was noted at the 1-year stage. During the second year, when no corrections of the dentures were made, the temporal activity decreased below the pretreatment level. The masseter muscle chewing activity generally showed no significant changes during the 2-year observation period. The mean durations of the chewing strokes and chewing cycles were longer than values reported in dentate samples and generally displayed no significant changes during the 2-year period.

Longitudinal study of soft-tissue profile changes in patients receiving immediate complete dentures.

The gradual changes in the skeletal and soft-tissue profile of complete denture wearers resulting from residual ridge reduction were studied using profile cephalometric films obtained from 21 subjects during a 2-year period of denture use. The variables for each observation stage were calculated from 58 digitized reference points. During the first 6 months of denture use, a marked anterosuperior rotation of the mandible as a result of ridge resorption caused a similar profile change of the soft-tissue chin and mandibular lip. Continuing ridge reduction resulted in a marked change of mandibular position at the 2-year stage, leading to a pronounced protrusion of the chin and mandibular lip and a more outward position of the maxillary lip.

Alkaptonuria in a cynomolgus monkey (Macaca fascicularis).

An eight year old wild caught Cynomolgus monkey was diagnosed as having alkaptonuria, a condition characterized by the passage of a normal-colored urine which darkens upon standing. The underlying cause is the congenital lack of homogentisic acid oxidase with subsequent passage of homogentisic acid in the urine. No other clinical manifestations, such as deposition of pigment in the skin or mucous membranes or development of an ochronitic arthritis, were observed in this animal.

Longitudinal electromyographic study of swallowing patterns in complete denture wearers.

A longitudinal electromyographic study of the swallowing activity of the anterior temporal and masseter muscles was performed on 21 subjects with complete dentures. In six patients, the lip activity was recorded. At the pretreatment stage, when the patients had a residual anterior dentition, the peak mean voltages of the elevators showed low values (range 18 to 30 microV), and after denture placement, the anterior temporal muscles showed a further decrease. After denture relining, slight increases in temporal activity were noted at the 1-year stage. During the second year, when no corrections of the dentures were made, the temporal activity decreased below the pretreatment level. The masseter activity showed no significant changes during the 2-year observation period. The mandibular lip showed significantly stronger swallowing activity than the maxillary lip. The mean durations of the swallows were markedly longer than values reported for subjects with a complete natural dentition.

Bacterial and Ureaplasma colonization of the airway: radiologic findings in infants with bronchopulmonary dysplasia.

OBJECTIVE: We designed this retrospective study to compare radiologic findings in premature infants with bronchopulmonary dysplasia (BPD) in whom gram-positive cocci (GPC), gram-negative bacilli (GNB), or Ureaplasma urealyticum were colonized. Another objective was to correlate the radiologic findings of these patients with the clinical severity of BPD. STUDY DESIGN: We correlated serial tracheal aspirates with radiographic findings from 183 infants whose birth weight was < or = 1250 gm. BPD severity was assessed by oxygen dependency at 36 weeks of postconceptional age (36 w PCA) and at the time of discharge. Two radiologists independently scored films taken at birth and 1, 7, 14, 21, 28, and 35 days of life. RESULTS: Of the study population, 55% were male and 35% were black; 80% received surfactant and 69% received dexamethasone; 91% survived. GPC isolates from throat cultures were mainly Staphylococcus [corrected] epidermidis and Streptococcus haemolyticus. A superimposed GNB colonization was present in 37% of these infants. Most common isolates were Klebsiella pneumoniae, Enterobacter cloacae, and Escherichia coli. Sepsis caused by GPC developed in 16% of all patients; 7% had sepsis caused by GNB. Infants infected with GNB remained receiving oxygen at 36 w PCA and at the time of discharge twice as often as those noninfected. RADIOLOGIC FINDINGS: Hyperinflation, interstitial changes, and generalized or localized emphysema were prominent features throughout. Mean radiologic scores increased over time in a pattern similar among GPC, GNB, and U. urealyticum infected and noninfected infants. High radiologic scores were not predictive at any time of infants who needed supplemental oxygen at 28 days and at 36 w PCA. Infants infected with U. urealyticum were neither clinically nor radiologically different than noncolonized neonates. CONCLUSION: GPC, GNB, and U. urealyticum airway colonization is not associated with particular radiographic changes at any time. GNB-infected infants had the most severe BPD course, and yet they were radiologically indistinguishable from the other patients. U. urealyticum colonization does not result in more clinically severe BPD or demonstrate a unique radiologic course.
