[Krukenberg tumor. A seldom considered condition?].

In 1896, Krukenberg described what he presumed was a new type of primary ovarian neoplasm. The true metastatic nature of this lesion was established six years later. Some 10% of all ovarian malignant lesions are regarded as metastatic. Of these, approximately 50% are Krukenberg tumours. The tumour is well defined histologically and is usually secondary to a tumour in the gastrointestinal tract. We describe the case of a 48 year old woman with gastric cancer who, three years after diagnosis and subsequent total gastrectomy, was referred to us for observation with peritoneal involvement. We stress the importance of careful clinical and radiological follow-up of female patients with gastrointestinal cancer.

Ki-67 index in fine needle aspiration cytology specimens from primary and recurrent rectal cancer.

A Ki-67 index was determined in 63 fine needle aspiration cytology (FNAC) specimens from patients with rectal cancer, 21 with primary cancers and 42 with local recurrences. Seventeen of the patients had received external irradiation before the examination. In primary cancers, in recurrences as well as in the post-irradiation cases, the mean percentage of Ki-67 positive cells was about 55% with a wide variation. The percentage of Ki-67 positive cells was independent of the length of the recurrence-free period. In conclusion, the Ki-67 antigen may well be studied in FNAC specimens. The presence of antigen did not correlate with development of recurrence, length of recurrence-free period or irradiation prior to the analysis.

CDw75 antigen expression in breast lesions.

Alterations on the cell surface of the oligosaccharide portion of glycoproteins and glycolipids are thought to play a role in tumorigenesis. Sialyltransferase catalyzes the incorporation of sialic acid to the carbohydrate group of glycoconjugates. Sialyltransferase has been found elevated in different tumour tissues and in the serum of cancer patients. In the present study we have examined the expression of the beta-galactosyl alpha 2,6-sialyltransferase requiring epitope CDw75, with the monoclonal antibody HH2. 142 breast lesions were included. 21% of the carcinomas in situ and 35% of the invasive carcinomas showed a diffuse cytoplasmic staining. Seven cases of invasive carcinomas also showed a distinct membrane immunoreactivity. We found no correlation between reactivity for CDw75 in malignant lesions and their metastatic potential. Only five out of 11 primary tumours with metastases expressed CDw75 in the primary tumour. In the benign lesions, there was a positive reaction in proliferating lesions, e.g. intraductal papillomas (2 out of 3 cases) and in epithelial proliferations in fibrocystic disease (10 out of 14 cases). None of the four fibroadenomas and phyllodes tumours and only one out of 22 cases of normal breast tissues showed immunoreactivity for HH2. In the malignant lesions, CDw75 was more frequently expressed in the carcinomas of high malignancy grade. The high frequency of immunoreactivity among the benign breast lesions can be indicative of activation of the epithelial cells.

Malignancy grading of the deep invasive margins of oral squamous cell carcinomas has high prognostic value.

Several recent studies have indicated that cells at the invasive tumour margins often are different from cells within other parts of various human cancers. In this work, we have studied all squamous cell carcinomas of the floor of the mouth registered in Norway during the years 1963-1972 (N = 96). Borderline cases and cases given no treatment were excluded. Of the remaining 79 cases, biopsy specimens acceptable for histological grading were obtained from 61 patients. Only the most invasive margins of the tumours were histologically graded independently by two pathologists according to a multifactorial grading system. The results confirmed our previous findings that grading of invasive tumour margins is an independent prognostic factor in Cox's multivariate survival analysis (P less than 0.01). Inter-observer agreement was calculated by kappa statistics, and good agreement was obtained (kappa = 0.63). Neither agreement nor prognostic value was improved after calibration of the pathologists. Conventional Borders' grading of the whole biopsy had no prognostic value (P less than 0.38). We conclude that invasive cell grading may be of value for treatment planning of oral cancers, and that further studies of the deep, invasive parts of oral and other cancers are needed in order to obtain a better understanding of tumour cell invasion and metastasis.

C-erbB-2 protein and neuroendocrine expression in intraductal carcinomas of the breast.

A total of 52 intraductal carcinomas were classified according to nuclear size and histologic subtype and immunostained for neuron-specific enolase (NSE) and c-erbB-2 oncoprotein. Eleven out of 13 cases of the large cell comedo variant of intraductal carcinoma exhibited strong c-erbB-2 protein immunoreactivity, while only one was NSE positive. Nineteen out of 23 intraductal carcinomas of small cell type exhibited NSE immunoreactivity. None of these was c-erbB-2 protein positive. Some 72% of NSE positive cases were also immunoreactive for other neuroendocrine screening markers and/or hormones. We conclude that there is an inverse relationship between NSE immunoreactivity and c-erbB-2 protein expression in intraductal carcinomas.

Prognostic value of Rhesus blood groups in oral squamous cell carcinomas.

In the current study of the prognosis of all patients (N equals 70) with squamous cell carcinomas (SCC) of floor of mouth in Norway during the period 1963 to 1972, the authors found that patients with Rhesus (Rh) (D)-negative blood group had significantly poorer prognosis (mean 5-year survival, 8%) than patients with Rh (D)-positive blood group (5-year survival, 30%) (P equals 0.04). This extends the authors' previous observations in another group of oral cancer patients. The authors do not know the explanation for this association. However, the Rh gene locus is located on the short arm of chromosome 1 which reportedly has shown rearrangements in some head and neck SCC and other human neoplasms. The authors therefore speculate that the Rh gene locus may be linked with chromosome 1 changes of importance for the progression of oral SCC.

A multivariate study of the prognosis of oral squamous cell carcinomas. Are blood group and hemoglobin new prognostic factors?

Because blood groups and hemoglobin concentration have been associated with the risk of the development of some cancers, this study evaluated the significance of ABO and Rhesus blood groups and hemoglobin concentration as prognostic factors in oral squamous cell carcinoma (SCC). The authors examined all registered primary SCC of buccal and maxillary alveolar mucosa in the Norwegian population between 1963 and 1972. The biopsy specimens from these patients were reevaluated and borderline cases excluded. The remaining 111 cases were included in the study, and features recorded on first admission were included in the survival analyses. ABO and Rhesus blood groups were found in 99 of these patients. Multivariate survival analysis showed that tumor size, hemoglobin concentration, stage, and Rhesus blood groups were significant prognostic factors, but sex, age, treatment, duration of symptoms, ABO blood groups, and clinical appearance of the tumors were not. The prognostic value of Rhesus blood groups and hemoglobin concentration has not been previously reported for oral SCC.

[Mammography--watch out for false negatives].

Patients with breast tumours should be examined by a combination of physical examination, mammography and fine needle aspiration biopsy. The authors discuss the history of four patients with palpable breast lesions. All of these patients had tumours which had not been discovered by mammography. The diagnosis was delayed 2,11,12 and 12 months respectively. Patients with palpable lesions must not be referred to mammography performed under "screening conditions", but must be examined in a clinical context, preferably by the "triple-test".

[Aspiration cytology in palpable breast changes of the breast. Too many specimens without cell material to examine].

During the first half of 1988, 927 fine needle aspiration cytology samples from palpable non-cystic breast lesions were examined in our laboratory. Among 582 aspirations performed by members of the laboratory medical staff the frequency of technically non-diagnostic specimens was 4.3 per cent, as against 34.2 per cent among 345 referred specimens. In the latter group there was a clear relationship between the frequency of inadequate specimens and experience in aspiration technique as judged from the number of referred specimens from the individual clinician. It is concluded that education and training in aspiration technique is needed to improve the quality of this diagnostic method.

[Fine needle aspiration cytology of the thyroid. Based on a 5-year case load].

The article describes a follow-up of 595 fine needle aspiration cytology samples of the thyroid gland taken during the five-year periode 1983-87. Of these samples, 133 (22.4%) were unsatisfactory for diagnosis. Among the remaining samples there were seven false negative and two false positive diagnoses of cancer. The false negative diagnoses were due to inadequate cytologic material not representative of the malignant tumor. The false positive diagnoses were due to cytomorphologic misinterpretation.

New malignancy grading is a better prognostic indicator than Broders' grading in oral squamous cell carcinomas.

The prognostic value of histopathologic grading of oral squamous cell carcinomas (SCC) has varied from not any to highly significant. We have retrospectively studied all (130) SCCs registered in Norway 1963-72 in the buccal and maxillary alveolar mucosa. From 68 of these cases biopsy specimens of acceptable quality were obtained. Broders' method of grading was compared with a modification of a recent malignancy grading system recommended by Anneroth et al. which was performed only within the histologically most invasive areas of the tumors. Cox's multivariate survival analyses showed that this grading in the invasive sites had highly significant prognostic value. Broders grade had no prognostic value. The stage of tumor had also prognostic value. These highly significant results indicate that the histologically invasive areas may be primarily responsible for the clinical behavior of the tumor, and this may be of importance for the choice of therapy for oral SCC.

Effect of acute potassium depletion and thiazide treatment on blood glucose in the normal rat.

The effect of potassium depletion and a thiazide diuretic on glucose metabolism was investigated in male rats. Potassium depletion was induced by potassium-deficient diet and ion-exchange resin by stomach tube for 5 days. Plasma and muscle potassium was reduced by 30% (P less than 0.001), and fasting blood glucose concentration was elevated by 20% (P greater than 0.025) by potassium depletion. Hydroflumethiazide 10 mg/kg given orally once daily for 8 days produced no further alteration in any parameter. We conclude that acute potassium depletion in the rat may increase fasting blood glucose concentration, whereas addition of short-term thiazide treatment does not seem to potentiate this effect.

Lack of hyperglycemic effect of chlorothiazide in normal rats fed diets with different carbohydrate content.

The effect of chlorothiazide (CT) on blood glucose and liver glycogen was investigated in rats fed either a standard, a carbohydrate-rich, or a carbohydrate-poor diet. CT did not alter blood glucose concentration or liver glycogen content in rats fed the carbohydrate-rich diet. After 5 days on the carbohydrate-poor diet, an oral glucose load produced significantly smaller rate constant for glucose disappearance from blood and significantly greater area below the glucose concentration--time curve than after a standard diet, but CT did not further alter these parameters. Nor did oral CT for 19 days alter glucose tolerance. In conclusion, CT in single dose or short term studies does not seem to alter glucose metabolism in the normal male rat.

Low-grade cribriform ductal carcinoma in situ of the breast. Fine needle aspiration cytology in three cases.

This paper presents the cytologic features of fine needle aspiration biopsy specimens from three cases of ductal carcinoma in situ characterized by small and uniform tumor cells growing in a predominantly cribriform pattern without comedo necrosis (low-grade cribriform ductal carcinoma in situ). On cytology, most of the tumor cells were clustered in three-dimensional ductal structures. Occasionally in the clusters the tumor cells were seen bordering central lumina, quite similar to the architecture in histology. A few single tumor cells and no myoepithelium were seen. The background was clear or slightly hemorrhagic, without necrosis. The tumor cells were uniform and had a cylindroid shape, with round or oval nuclei. Morphometrically the mean largest nuclear diameter was 1.5-1.6 times that of a red blood cell. The chromatin was finely granular, with a minute nucleolus and slight condensation along the nuclear membrane. In cut sections all three tumors showed strong immunoreactivity for neuron-specific enolase. Unless the cribriform growth pattern is recognized in the smear, the cytologic diagnosis of this entity is difficult.
