The measurements of the intravascular HbO2 saturation in tumor capillaries using the cryophotometric micromethod reveal that very low HbO2 saturation values predominate in malignant tumors. Under normoxic conditions only 8% of the measured values exceed 50% saturation. 53% of the intracapillary HbO2 saturation values are in the range of 0-10% HbO2 saturation. Great regional differences are seldom and can be found only in areas where a sufficient vascularization still exists. Taking into account the data of morphometric analysis of tumor vascularization and the parameters of respiratory gas exchange, the measured frequency distribution of HbO2 saturation values in tumor capillaries is simulated by means of different three-dimensional capillary structures; On the basis of these microcirculatory units, tissue pO2 values are computed in the intercapillary regions. As a result of these calculations it can be stated that the computed data derived from intracapillary HbO2 saturations are in sufficient agreement with pO2 values measured polarographically using gold- microelectrodes.
In our controlled longitudinal study of a representative high-risk sample of psychogenically impaired individuals, within a multidimensional model of mental health, the variables "life event", "social support" and "personality" are examined concerning their analysis of variance and their interdependence. We assume that a therapeutic influence on the variable "personality" causes a modified risk of becoming psychogenically impaired, and intend to prove this in terms of the altered maturity of defenses, the improved ego-functions, but also in terms of the improvement of the social network and the rarer constellation of critical life events. The modified effect on the variable "personality" is a differential offer of psychoanalytic therapy. By means of our instruments of investigation we tried to clarify if the acceptance of psychotherapy offered to resp. the motivational situation of our probands in regard to a psychotherapy covariates with certain sociodemographically or depth psychologically measurable personality patterns. There was a trend towards accepting the offer of therapy in case of high psychical and socio-communicative psychogenic impairment and in case of relatively high frankness on the FPI scale. The acceptance of an offer of therapy correlated highly significantly with the frankness scale of the Bond questionnaire extended for several defense items. The implications of this finding are discussed.
The psychological coping concept and the psychoanalytic theory of defense are the two most important models for assessing coping with chronic illness. Their interplay has hardly been investigated so far. A "layer model" is proposed to describe the connections between defense and coping. This model is empirically tested by applying a path analysis (latent trait model) to patients suffering from Crohn's disease during the course of the illness. The layer model can be verified, but is valid only during the acute phase of the illness. The results convincingly show that the relationship between defense and coping can only be described as a dynamic interplay that changes in the course of the disease process.
