[Serum enzyme alterations during diabetic ketoacidosis. Prospective study of the behaviour of AP, AsAT, AIAT, and GGPT in 24 cases (author's transl)].

Because of the scarcity of data on the changes of serum enzymes during diabetic ketoacidosis, the authors have prospectively studied the alterations of gamma-glutamyl transpeptidase (GGTP), alkaline phosphatase (AP), aspartate aminotransferase (AsAT), and alanine aminotransferase (AIAT) in this metabolic disturbance. The most significant finding was the frequent increase of AP activity on admission, with a systematic fall of the serum levels within the first 24 hours (p less than 0.001). This phenomenon was negatively correlated with patients' age (p less than 0.05), and the same occurred with the bone isoenzyme of AP (p less than 0.01). The remaining enzymes studies were always normal, thus suggesting that any increase of the serum levels of GGTP, AsAT, and AIAT found in a patient with diabetic ketoacidosis must arise the suspicion of an associated disturbance.

Renal function in protein-energy malnutrition.

So far, most studies on renal function in children with advanced protein-energy malnutrition have shown an impairment of glomerular filtration rate and renal plasma flow, as well as diminished ability to excrete an acid load and a concentrated urine. Investigation of these functions in eight marasmic infants and eight children with kwashiorkor once they were free of obvious infections and acute electrolyte disturbances has shown a practically normal renal performance.

[Effect of the bilateral olfactory bulbs removal on the daily fluctuations on the plasmatic corticosterone levels (author's transl)].

The influence of the bilateral destruction of the olfactory bulbs on the circadian rhythm of plasmatic corticosterone in rat was investigated. Twentyfive days after bulbectomy, the values of corticosterone in the morning and in the evening were not significantly different. The possible involvement of the stria medularis, habenula, septum and preoptic area in the determination of this phenomenon is discussed.

Screening for malnutrition with arm circumference.

The opinions on the performance characteristics of the mid arm circumference as anthropometric measurement for malnutrition screening are quite contradictory. In this paper we analyze the specificity and sensitivity of the arm tape under different conditions and conclude that the design of the tape characteristics should aim at an instrument which can satisfy specific needs instead of a general purpose one. Comparative analysis of some anthropometric measurements.

The main objective of this work is to determine what anthropometric method to use under field conditions, given the kind of mainutrition to be detected. In the first part of the work some criteria and procedures are given by which to compare the field perfomance characteristics (i.e. transportability, facility of use, etc.), of the most commonly used instruments. Then we extrapolate these characteristics to the respective methods like weight for age, weight for height, height for age, etc. In the second part we present a correlation matrix among the various methods and analyze it in order to associate with each method a specific typology of malnutrition.

Effects of nebulized fenoterol, associated with ipratropium or steroids, on the heart rate of infants under one year of age with acute wheezing.

The main objective of this study was to evaluate the effect of fenoterol alone or associated with ipratropium bromide or steroid on the heart rate in young children. Ninety-four infants less than 1 year of age were randomly allocated to receive nebulized fenoterol alone, fenoterol plus ipratropium bromide, fenoterol plus corticosteroids, or normal saline solution. An increase in heart rate was observed in all four groups. The increases were statistically significant (P less than 0.001) in all three treatment groups, and no difference between them was observed (F = 0.65, NS). However, the heart rate remained within clinically acceptance limits. We conclude that nebulized fenoterol alone or combined with ipratropium or steroids can be safely used in the treatment of wheezy infants.

Use of nebulized bronchodilators in infants under 1 year of age: analysis of four forms of therapy.

The main purpose of this study was to evaluate four different forms of treatment in young infants admitted for acute wheezing (AW). Seventy-nine infants less than one year of age were randomly assigned to one of five groups. Group 1 received nebulized fenoterol plus ipratropium bromide, group 2 fenoterol, group 3 fenoterol plus steroids, and group 4 aminophylline, IV, plus steroids and oral fenoterol; the control group, or group 5, received nebulized normal saline solution. Clinical evaluation was done by means of a scoring system. The effectiveness of treatments was estimated by a score decrease in the first 24 hours, by the percentage of patients whose scores did not decrease during the same period, and by the number of days in the hospital. All infants had significantly decreased scores, except those in the control group; the aminophylline group included a greater percentage of patients who did not abate their scores, and they stayed in the hospital for more days than those in the other groups. The fenoterol group had the shortest hospital stay. All four treatments produced objective clinical improvement in bronchial obstruction. However, the nebulized bronchodilator treatments were more effective than aminophylline IV in decreasing scores on the first day, and they resulted in shorter hospitalization.

Anomalous creatine kinase isoenzymes in serum from patients with rectal carcinoma.

Sera from eight patients with biopsy-proven rectal carcinoma were assayed for creatine kinase (CK). Two of the patients had enzyme activity: one with the BB and the other with an MB isoenzyme. We think that our results may be of interest because the detection of anomalous isoenzymes of CK in serum ma serve as a diagnostic marker or as an indicator of the effectiveness of treatment.

Optochin-resistant variants of Streptococcus pneumoniae.

Ten Streptococcus pneumoniae clinical isolates, possessing physiologically typical pneumococcal characteristics, showed optochin-susceptible and optochin-resistant colonies around the optochin disk, when tested for optochin susceptibility. Equivocal optochin disk test results should be confirmed by bile solubility, agglutination tests, or both.
