Evidence for spirochetal origin of circumscribed scleroderma (morphea).

Acrodermatitis chronica atrophicans (ACA) and morphea are clinically distinct skin diseases with some common features and possible coexistence. We found antibodies to Borrelia burgdorferi in eight of fifteen patients with morphea. Six of them had IgG antibodies and two both IgG and IgM antibodies. Four of the eight seropositive and five of the seven seronegative patients had been treated with high dose penicillin previously. Spirochetal organisms could be cultured in Barbour-Stoenner-Kelly's medium from a skin biopsy of one seropositive untreated patient. Spirochetes were recovered from histological sections in three of eight, two seropositive and one seronegative morphea and in one of three erythema chronicum migrans patients by an avidin-biotin immunoperoxidase method. The similar clinical picture of ACA and morphea, the response to penicillin therapy in both entities, the presence of antispirochetal antibodies, the isolation of spirochetes in culture and the detection of spirochetal organisms on histological sections suggest a close relationship among these diseases. We conclude that morphea may represent a Borrelia infection. The correlation to ACA is discussed.

Compliance and results of treatment for amblyopia in children more than 8 years old.

Our prospective study of 350 amblyopic children divided them into three age groups: 2 to 5 1/2 years, 5 1/2 to 8 years, and 8 years and older. Treatment consisted of occlusion of the good eye. Compliance with treatment was analyzed by age group for the whole study population, but visual acuity results were evaluated only in children who complied fully with treatment and who cooperated at the initial visual acuity test with a Snellen chart. Younger children were significantly more compliant than older ones. This was probably the primary reason for the high incidence of treatment failure in older children. Children older than 8 years who complied with treatment showed a marked improvement in visual acuity--one almost as good as that in the younger children. A better initial visual acuity can be taken as a good prognostic sign, especially for this age group. In each group, most of the improvement occurred during the first three months of treatment. Improvement after this period was marginal.

Treatment of polymorphous light eruption with nicotinamide: a pilot study.

In a pilot study, 42 patients suffering from polymorphous light eruption (PLE) were treated with oral nicotinamide, 3 g daily, for 2 weeks. Twenty-five patients remained free from lesions despite extensive sun exposure. We suggest that an abnormality in tryptophan metabolism is important in the aetiology of PLE, and that nicotinamide administration partially corrects this.

[Extramammary Paget's disease associated with stomach cancer].

A case of extramammary Paget's disease in the perianal region is reported. The disease presented as a whitish lesion with central erosion. Examination revealed an underlying adenocarcinoma of the stomach. The disease was treated by cryosurgery.

Evaluation of the Noiles hinged knee prosthesis. A five-year study of seventeen knees.

The Noiles hinged knee prosthesis has been reported to diminish sheer and tensile loading on the cement-bone interface. Theoretically, this should eliminate the complication of loosening, commonly reported in hinged total knee prostheses. We reviewed the cases of all of the patients who have been treated with the Noiles knee prosthesis at our institution. Eighteen knees in fourteen patients were available, seventeen of which were followed for more than five years. Although all of the prostheses did well initially, ten knees in eight patients (56 per cent) had failed by an average of thirty-two months postoperatively. These eight patients included four of the five who weighed more than ninety kilograms and all of those who had had a prior arthroplasty. The failures were clearly defined along the lines of sex distribution. Measurements on the radiographs showed a significant difference between sexes in the metaphyseal diameter and thus in the percentage of the femoral canal that was filled by the prosthesis and cement. We concluded that prosthetic design must incorporate cortical cement support to reduce stress at the cement-bone interface, especially in a medullary canal with a larger diameter.

[Depressive syndromes within the scope of schizophrenic psychoses].

In the present study two groups of 30 patients were compared. The first group included 30 acute schizophrenic patients, who were rated immediately after admission and 3 months later under depot-neuroleptics. The second group consisted of chronic schizophrenic outpatients, who were rated at the beginning of Fluphenazindecanoat-medication and 8 months later. As documentation PSE (present state examination) was used. Psychotic symptoms of the acute schizophrenic patients could be influenced well by the treatment, not in the same way, however, depressive and neurotic symptoms. The frequency of psychotic symptoms is lower in chronic schizophrenics than in acute ones at the beginning of therapy. 8 months later there are little more psychotic symptoms in chronic patients than in acute schizophrenics; the depressive symptoms follow the same design in a lower extant however. The scores of depressive symptoms in the acute-group are similar to those of chronic schizophrenics and do not change significantly in the course of 8 months lasting depot-neuroleptic therapy. Another study could show 1982, that 67 outpatients from 87 in the year 1972 still were in psychiatric care and control, 23% of them received antidepressiva because of depressive symptoms in connection with residual syndromes and only 4% because of exhaustion-syndromes after remission of the disease.
