Isolation of spirochetes from the skin of patients with erythema chronicum migrans in Austria.

Spirochetal organisms were isolated from biopsies (skin punches) of the erythematous anular skin lesion of three patients with erythema chronicum migrans. The organisms were cultivated in modified Kelly's medium and - after an incubation period of 2 weeks - discovered by dark field microscopy (X 200). Compared with the Lyme disease agent Borrelia burgdorferi the organisms did not differ in shape, motility and reaction with polyclonal antiserum. Differences to B. burgdorferi were found in (i) very slow growth of the isolated organisms in modified Kelly's medium and in (ii) non reactivity with monoclonal antibodies against the outer membrane protein of B. burgdorferi. It is concluded that the Austrian isolates are closely related to but not identical with the Lyme disease agent.

Sebum suppression by benzoylperoxide.

Sebum excretion rate (SER) was determined by thin-layer chromatography and based on densitometric data in 9 male and 5 female patients suffering from acne during treatment with 10% benzoylperoxide (BPO). BPO was applied to the forehead (treatment area), whereas gel only was applied to the chest (control area) for 6 weeks. SER decreased in 10/14 subjects, no change was found in 1 subject and 3 patients exhibited an increase in SER. Suppression varied between 12 and 86%. Concomitant SER reduction in control and treatment areas was noted in 6 patients. When treatment and control areas of individual patients were considered for data evaluation, SER decrease was noted in only 4 out of 14 subjects. Similar results were obtained when free fatty acids, triglycerides and squalene data were analyzed. Combined, these data suggest that BPO may affect SER in some patients. The present experiments indicate the importance of adequate controls in such studies.

[The importance of adrenergic receptors for dermatology].

Receptor research is a rather new but most promising field of research. This review summarizes the data available on the adrenergic receptors. in addition, information is provided about the important function of these membrane receptors as control or modulator of dermatophysiology and dermatopathy.

Solution intolerance among users of four chemical soft lens care regimens.

The incidence of solution intolerance in 161 soft contact lens patients was about 33% in those using chlorhexidine plus thimerosal chemical disinfecting systems. Those who disinfected their lenses with systems based primarily on thimerosal developed intolerance in 5 to 8% of cases. A family history of allergy appears not to be significant.

Strategy for the use of biliary scintigraphy in non-iatrogenic biliary trauma.

Biliary scintigraphy was used to examine 21 patients who had suspected non-iatrogenic biliary trauma. Seven patients (33%) had scintigraphic evidence of biliary leakage. Ultimately, surgical biliary repair was required for only three of these patients. Visualization of the gallbladder did not occur in eight trauma patients, but only one patient was shown to have cholecystitis. In this series, 16 patients had Tc-99m sulfur colloid scans that offered no significant advantage over cholescintigraphy in the detection of hepatic parenchymal defects. Biliary scintigraphy provides clinically useful information in cases both of blunt and penetrating trauma.

Improved intrinsic resolution: does it make a difference? Concise communication.

The purpose of this study was to determine what effect further improvement in an Anger camera's intrinsic resolution has on lesion detection. We studied 52 patients undergoing bone imaging and 58 undergoing liver imaging. All patients had images performed in rapid sequence on ZLC -75 and ZLC -37 Anger cameras, both by Siemens. The two imaging systems are virtually identical except for the number of photomultiplier tubes and crystal thickness; these resulted in differences in intrinsic resolution ( ZLC -75 less than 3.8 mm FWHM at 140 keV, ZLC -37 less than 4.9 mm) and sensitivity ( ZLC -75 approximately 0.91 of ZLC -37 at 140 keV). Observer performance, measured by ROC curves, for detection of abnormalities was virtually identical with the two instruments. Subjectively, there was a trend toward preference of the ZLC -75 images, but this was not associated with any significant improvement in lesion detectability even in the subgroup in which a preference for one or the other instrument was noted.

[External erythromycin therapy of acne].

24 patients suffering from papulo-pustular acne were treated with mono-therapy with 2% erythromycin-containing alcoholic solution for eight weeks. Pustular lesions responded promptly, followed by inflammatory papules. Microbiological investigations revealed a lack of propionibacteria as well as reduction of pathogenic staphylococci. Patients after extended local erythromycin treatment did not show any erythromycin serum levels.

Acne Vulgaris

Tomographic gallium-67 citrate scanning. Useful new surveillance for metastatic melanoma.

Conventional gallium scans are not useful to evaluate patients with metastatic melanoma. We evaluated a new method of tomographic gallium imaging. One hundred fourteen tomographic scans were obtained in a prospective surveillance study of 67 patients over a 3-year period. Scans were evaluated and compared to findings of independent clinical evaluations. Sensitivity of gallium identification of tumor involving peripheral lymph nodes and soft tissues, abdomen, mediastinum, and osseous sites was 68% to 100%; overall sensitivity of this technique is 82% with specificity of 99% in 570 organ system assessments. Analysis of discordant findings when a site was clinically occult but gallium-positive showed gallium uptake to be true-positive in six of seven lymphatic sites, three of three lung and mediastinal sites, six of six abdominal sites, but in no brain or bone sites. Gallium lesions identified by computed tomographic scans proved to be false-positive at one lymphatic and one bone site, and false-negative at four otherwise clinically evident lymph node and soft tissue sites, seven pulmonary sites, and four brain sites. Gallium tomographic scanning provides a composite assessment of melanoma and may eliminate the need for other studies.
