A scoring system is presented for selection of essential drugs, using criteria of efficacy, safety, cost of a course of therapy, compliance, multiple usage and storage, ease of administration and local availability. Such a system allows for different weighting of factors whose relative importance varies from country to country and would help in choosing the most appropriate and cost-effective drugs for use in developing countries. The importance of such factors as cost and compliance has been illustrated with suitable examples. This approach could also be used for individual patient decisions with the aid of a computer program.

Indomethacin (IM), a potent inhibitor of prostaglandin synthetase, significantly inhibited implantation in rats when administered at a single subcutaneous dose of 15 mg/kg on day 2 or 3 of pregnancy or at a dose of 800 micrograms administered intraluminally in the uterus on day 4 of pregnancy. Indomethacin, administered subcutaneously at a dose of 6 mg/kg on day 3 or 4 of pregnancy and intraluminally at a dose of 800 micrograms/rat on day 4 of pregnancy, also significantly reduced uterine prostaglandin levels. It is suggested that a certain level of uterine prostaglandins is essential for normal implantation and that indomethacin inhibits implantation by reducing uterine prostaglandin levels.

1 The antipyrine half-life, metabolic clearance rate and the apparent volume of distribution were measured in six subjects handling malathion. These results were compared with six appropriate controls. 2 Occupational exposure of malathion in subjects reduced the antipyrine half-life from 9.18 +/- 2.16 h in controls to 4.73 +/- 1.65 h. 3 It is concluded that malathion exposure increases the rate of antipyrine elimination, possibly by inducing the hepatic microsomal enzymes.
Three antiprostaglandin drugs, acetylsalicylic acid, indomethacin and ibuprofen, were administered intraluminally at different does to pregnant rats on Day 4 of pregnancy. It was observed that ibuprofen administered at a dose of 400 micrograms inhibited implantation in all rats. Ibuprofen when administered at a lower dose of 100 or 200 micrograms on Day 4 of pregnancy or at a dose of 400 micrograms on Day 3 or 5 of pregnancy did not exert such an antiimplantation effect. Acetylsalicylic acid and indomethacin also induced, but to a lesser extent, an antiimplantation effect when administered at a dose of 400 micrograms on Day 4 of pregnancy.
