[Automated management of information in the microbiology department of a hospital analysis laboratory. Practical evaluations and operational proposals].

The computer has now an important role in analysis laboratory. Particularly the microbiological sector may receive a considerable support by the computer. In fact it's possible to form an effective epidemiologic hospital observatory, by making a systematic and continuous control of bacterial ecology in hospital wards, through the statistical results elaborated by computer. This is the object of the microbiological sector in the hospital's laboratory in Pistoia Town.

Renal vein renin activity and prostaglandins A and E in hypertension.

The relation between renal vein renin activity (RVRA) and renal vein-prostaglandin A (PGA) and prostaglandin E (PGE) concentrations was examined in 50 patients with hypertension who underwent renal vein catheterizations for suspected renovascular hypertension. In 14 patients wih unilateral renal artery stenosis, mean renal vein PGE concentrations were higher in the renal vein draining the nonischemic kidney, while RVRA was increased in the renal vein draining the ischemic kidney. PGE ratios from the two sides were inversely related to RVRA ratios in this group. In the other 36 patients with either bilateral renal artery stenosis, essential hypertension or nonrenovascular unilateral renal disease, RVRA and PGE concentrations were similar in both kidneys. No relation between RVRA and PGA could be established for any group.

Adjuvant radiochemotherapy in high-risk rectal cancer results of a prospective non-randomized study.

AIMS AND BACKGROUND: In 1990 the National Institutes of Health Consensus Conference recommended adjuvant combined therapy for patients with radically resected rectal cancer at high risk for relapse (ie, stage II-III). The purpose of our prospective non-randomized study was to verify the feasibility and effectiveness of postoperative radiochemotherapy in terms of improvement in disease-free and overall survival in this patient subgroup. STUDY DESIGN: From January 1990 to October 1998, 191 consecutive patients with radically resected stage II-III rectal cancer were treated. A total of 159 patients with a 24-month follow-up were assessable for toxicity and survival. Anterior resection was performed in 129 (81%) and abdomino-perineal resection in 30 (19%) patients. Fifty-four (34%) stage II and 105 (66%) stage III patients entered the study. Within 45-60 days of surgery, all patients received 5-fluorouracil chemotherapy at the dose of 500 mg/m2 as an i.v. bolus on days 1-5, every 4 weeks, for 6 cycles. Chemotherapy cycles III and IV were administered at the same daily dose on radiotherapy days 1-3 and 29-31. Radiotherapy consisted of 45 Gy/25 fractions plus a boost dose of 5.4 Gy. RESULTS: After a median follow-up of 57 months (range, 25-123), overall recurrent disease was reported in 58 (36%) patients: local, systemic, and both local and systemic relapses in 12 (8%), 37 (23%) and 9 (6%) cases, respectively. According to local extension, recurrence rates were 15% and 48% in stage II and III, respectively. Five-year overall and disease-free survival were 71% and 66%, respectively. Overall survival was 87% in stage II and 62% in stage III patients, and disease-free survival was 84% and 56% in stage II and III disease, respectively. According to univariate and multivariate analyses, significant prognostic factors for better tumor control were: stage (II vs III, P <0.001), the number of involved nodes (< or = 3 vs > 3, P <0.0001), and no extracapsular node invasion (P <0.0001). The recommended dose of the combined radiochemotherapy regimen was generally well tolerated. The incidence of any > or = grade 3 acute toxicity (according to the WHO scale) was 13% diarrhea, 11% proctitis, 5% perineal dermatitis and 4% myelosuppression. Four (3%) patients had radiotherapy-related severe late toxicity which required surgery. CONCLUSIONS: The study provided recurrence rates and survival similar to other adjuvant radiochemotherapy regimens published in the literature. However, in view of the low 5-year survival rate recorded in stage III patients, a different approach should be investigated.

[Statistical and epidemiological on the incidence of salmonellosis in Valdinievole's district (author's transl)].

The Author, on the basis of the data of his Laboratory in Pescia's Hospital, put in evidence the diffusion of the endemic state due to Salmonella in his district and calls competent authority's attention for a greater conscience of this problem, so as to embank a situation that, controlled efficaciously, could cause epidemical episodes like those happened in other zones of our Country.

[Group B beta-hemolytic Streptococcus: study on colonization of mother-neonate couples observed in January-May 1988 at the obstetrics and gynecology unit and the neonatal section at the Pistoia Hospital].

In a previous study, carried out on women at delivery recovered at the obstetric ward of the Pistoia Hospital, we noted a high incidence of group B Streptococci cultured from vaginal site. Because group B streptococcal infection is very serious in neonates we decided to examine the frequence of vertical and horizontal transmission among the mother-neonate couples. We have also compared their rate of colonisation by doing rectal and vaginal swabs on women and rectal, vaginal and external auditory canal swabs on babies. In fact we known that infants moderately to heavily colonised with group B Streptococci at birth, appear to have a significant higher risk of developing serious disease than do infants only lightly colonised. In our work we haven't observed any acute group B streptococcal infection, but we have identified 18 couples mother-neonate colonised by the same group B Streptococci serotype, which is proof of an easy vertical transmission of group B Streptococci whilst horizontal transmission is very rare (only one case).

[Relation between a rapid polyvalent test and single antistreptococcal tests].

ASO and Streptozyme tests have been comparatively tried on 11,200 sera. The former detected 11.96% positive and the latter 13.5%. The difference was due to a greater sensitivity of the Streptozyme test since the most of the Streptozyme positive and ASO negative sera resulted positive in one or more of the single antistreptococcal tests (SK, ADN, SJ, DNA). Only few of the sera which give rise to a positive Streptozyme reaction were otherwise negative and the positive result could be considered aspecific.
