[Primary malignant lymphoma of the parotid gland. Report of a case].

Primary malignant lymphoma of the parotid gland is often correlated, in literature data, with inflammatory autoimmunological disease of the salivary gland. Finding of blasticlymphoid cells in myoepithelial scialoadenitic areas in a case of parotid disease is described by the Authors. Needle may be useful in detecting these high radio-chemosensitive neoplasms to reduce the occurrence of complications of total parotidectomy.

[Choledocholithotomy and papillosphincterotomy in surgery of the bile ducts].

The Authors argue about the different surgical choices feasible in case of CBD stones. They consider a case-record of 74 patients that underwent, from January 1988 to June 1990, choledocholithotomy or transduodenal sphincterotomy for non-neoplastic pathology of the CBD in the I Divisione di Chirurgia Generale OO.RR. Bergamo. Analysis of the data shows a significant pre-operative alteration of the gamma-GT plasmatic levels. That represent a risk-index useful to suspect a silent CBD lithiasis. The good results attained with the choledocholithotomy and with the transduodenal sphincterotomy show the efficacy of both methods, that keep up different indications. The Authors point out the efficacy of the antibiotic prophylaxis in the prevention of the operative infections.

[Neoplastic involvement of pulmonary fissures. Diagnostic possibility of high resolution x-ray computed tomography].

Acknowledging fissure invasion by pulmonary tumors located in contact with or in the vicinity of a fissure is an important piece of information for the therapeutic choice, especially in patients whose functional respiratory impairments are a contraindication of pneumonectomy. Fifteen patients with pulmonary neoplasms adjacent to a fissure were studied with standard computed tomography (10 mm sections), completed by high-resolution CT. The findings of the CT studies were compared with those operative reports. On surgery, the oblique fissure was infiltrated in 12 patients, unharmed in 2 others, while the horizontal fissure was infiltrated in 3 patients. Standard CT allowed diagnosing the involvement of the oblique fissure in 4 patients. Thin sections with high-resolution reconstruction algorithms allowed detecting the involvement of the oblique fissure in 13 cases, including one false-positive result. On thin sections, the fissures appear as well-delineated, dense lines. This allows an accurate study of the relationships between the tumor and the fissure, thus increasing the sensitivity of CT for the detection of tumoral extension across the fissures. The orientation of the horizontal fissure, which is almost parallel to the plane of section, makes the study of its relationships with an adjacent mass difficult, even in high-resolution CT.

Pseudopathologies of hemostasis and dental surgery.

Values of hemostasis tests are grossly abnormal in persons with factor XII deficiency, dysfibrinogenemia, the lupuslike anticoagulant, and pseudothrombocytopenia, yet these persons do not usually bleed spontaneously or even after surgery. We reviewed the records of 66 patients with laboratory diagnosis of one of the aforementioned abnormalities to determine whether they bled excessively after challenges to hemostasis such as dental extractions. Only three of them bled excessively, but two of these had other concomitant causes that could explain bleeding. Hence the dentist should not give up dental extractions or other surgical procedures when results of hemostasis tests are grossly abnormal, because these are not necessarily associated with abnormal hemostasis. Obviously, the correct interpretation of abnormal hemostasis values warrants close collaboration and reciprocal consultation between the dentist and the hematologist.

[Regional hemodynamic evaluation of heterotopic liver transplantation by the determination of the hepatic artery resistance index].

Hepatic artery resistance index (I.R.) was evaluated in six pigs through Doppler scan after progressive gauge reduction of the portal vein up to complete clamping. The I.R. values were (mean +/- S.D.): 0.48 +/- 0.01 in basal conditions; 0.37 +/- 0.03, and 0.33 +/- 0.01 with a reduction of 1/3 and 2/3 respectively; 0.27 +/- 0.02 after complete portal clamping. These data confirm the close relationship existing in the hepatic circulation between changes of the arterious resistence and those of the portal flow. The same animals, subsequently, underwent heterotopic liver transplantation in the right paracolic gutter. Immediately after surgery the I.R. values of the transplanted hepatic artery were recorded without clamping (basal portal flow 0.52 +/- 0.02) and after clamping of the transplanted portal vein (0.33 +/- 0.03), while maintaining clamped the recipient portal vein at the hilus. These values were comparable to those obtained in the recipient hepatic artery. These data suggest that, in spite of parenchymal damages secondary to surgical procedure and of the haemodynamically unfavorable site in which the organ is transplanted, heterotopic liver transplantation does not produce significant changes in hepatic arterious resistance.

[Neoplastic involvement of pulmonary fissures. Usefulness of high resolution CT].

In order to plan an adequate treatment, it is very important to recognize the transfissural spread of lung cancer near a pleural fissure, especially in the patients who cannot bear pneumonectomy because of impaired pulmonary function. Fifteen patients with lung cancer near a fissure were examined by means of conventional CT (10 mm-thick slices) followed by high-resolution CT (HRCT). The results obtained with both techniques were compared with surgical findings. At surgery, major fissure involvement was seen in 12 cases, minor fissure was involved in 3, and 2 patients were normal. In 4 cases fissural involvement was correctly demonstrated by conventional CT scans. Neoplastic involvement of the major fissure was identified in 13 cases by HRCT scans, with only 1 false positive. Pleural fissures appear as thin and sharp lines on HRCT scans. Thus, the relationship of lung cancers to fissures is better identified and the accuracy of the method in defining neoplastic spread is higher. However, the relationship of a neoplastic mass to the minor fissure is difficult to evaluate, because the fissure is roughly parallel to the scanning plane.

Complementary reactivities of anti-carcinoembryonic antigen and antitumor-associated glycoprotein 72 monoclonal antibodies in lung carcinomas.

Monoclonal antibodies (MAbs) COL-4 and COL-12, to the carcinoembryonic antigen (CEA), and B72.3, CC-49, CC-83, to the tumor-associated glycoprotein 72 (TAG-72), were used to study the expression of distinct epitopes of the two molecules in 71 cases of lung carcinoma of differing histotype. These MAbs reacted with the majority of adenocarcinomas by immunoperoxidase on tissue sections, but demonstrated a more restricted reactivity with squamous carcinomas. MAb CC-49 detected the highest percentages of adenocarcinoma cells while the B72.3 epitope was expressed more in squamous carcinoma cells. No significant reactivity with any of these MAbs was observed in small cell carcinomas. The expression of the CEA and TAG-72 epitopes in non-small cell lung cancers was highly heterogeneous: a distinct epitopes in non-small cell lung cancers was highly heterogeneous: a distinct epitope could be expressed by the majority of cells, whereas another of the same antigenic molecule was either poorly or not expressed. In adenocarcinomas, mixtures of anti-CEA, anti-TAG-72, and anti-(TAG-72 plus CEA) MAbs resulted in additive reactivity with an increase of the immunopositive tumors and of the percentages of immunostained cells. This was particularly evident for the anti-(TAG-72 plus CEA) mixture. In squamous cell carcinomas the increase was modest and was mainly related to anti-TAG-72 reactivity. These studies suggest variability in the antigenic structure of tumor-associated antigens expressed by carcinomas and indicate that anti-(TAG-72 plus CEA) mixtures may represent an immunological adjunct for clinical application in adenocarcinoma patients. On the other hand, TAG-72 should be considered a better target antigen, as compared to CEA, in the detection of squamous cell carcinomas.

Real-time system for robust spectral parameter estimation in Doppler signal analysis.

In assessing the level of stenosis in extracranial Doppler analysis, spectral analysis has until now been used qualitatively, for the most part. Owing to the many variables affecting the measurements (mainly noise level and instrument setting made subjectively by the operator), the reliability of the inferences on the degree of stenosis is not clearly definable. Under such conditions the need arises for algorithms and systems that can estimate spectral parameters with a higher degree of accuracy, to verify whether reliable inferences can indeed by made or if this technique is only a qualitative one. In the paper a real-time spectral analysis system is described. The system relies on a new spectral estimation algorithm which gives estimates with good robustness with respect to noise. Moreover, a clear measurement procedure which eliminates the many subjective factors affecting the estimates has also been proposed and used. The system has been evaluated with simulated signals and in clinical trials and has shown better performance than the commonly used commercial analysers.

[Pseudopathology of hemostasis in dental practice].

The authors demonstrate, with a 66 patients research, that also when laboratory tests on hemostasis indicate pathology like XII factor penia, disfibrinogenemy, lupus (Lac), pyastrinopenia, we manage pseudopathology. In these cases haemostasis is completely normal, even after surgery, like tooth extraction.

[Surgical treatment of pheochromocytoma: our experience].

The Authors report their experience of pheochromocytoma surgical treatment based on four recent cases one of which with extra-adrenal location. Newest and more refined diagnostic techniques as well as modern pathophysiologic data allow an early detection of the disease. Furthermore, the availability of proper medico-surgical therapeutic modalities assures less risks and better results.

[Chylous ascites].

Starting from one case of chylous ascites of their own observation, the authors attempt an etiological classification of this clinical entity, taking into account pertinent published material and the fact that in many instances chylous ascites is nothing more than a symptom. The authors then proceeds to a critical review of diagnostic and therapeutical resources; in that respect, they stress the extreme difficulties often encountered both in diagnosing the condition and in trying to correct it.
