Reticuloendothelial system phagocytic activity in cirrhosis and its relation to bacterial infections and prognosis.

The reticuloendothelial system phagocytic activity, estimated by the plasma elimination rate constant of 99mtechnetium-sulfur colloid, was studied in 41 decompensated cirrhotics and 10 normal subjects. The results were related to the incidence and type of bacterial infections occurring during hospitalization and follow-up, and to survival. The elimination rate constant of 99mtechnetium-sulfur colloid was lower in cirrhotic patients (0.168 +/- 0.007) (means +/- S.E.) than in normal subjects (0.220 +/- 0.005) (p less than 0.01). Cirrhotics were divided into two groups. Group I (16 patients) and Group II (25 patients) had normal or reduced elimination rate constant of 99mtechnetium-sulfur colloid, respectively. Both groups were similar in relation to clinical and biochemical data, hepatic blood flow, and wedged hepatic venous pressure. However, the liver scan and the elimination rate constant of indocyanine green were more altered in Group II. Patients in Group II developed acute bacterial infections more frequently than did patients in Group I. During hospitalization (24 +/- 2 days), bacteremia occurred in six patients in Group II and in none in Group I (p less than 0.05). During follow-up (28 +/- 3 months), 5 patients in Group II and none in Group I developed bacteremia (p less than 0.05). The cumulative survival rate of Group I patients was higher (p less than 0.05) than that of Group II patients at 3 months (100 vs. 80%), 6 months (94 vs. 68%), 24 months (74 vs. 42%), and 48 months (68 vs. 34%). We suggest that decompensated cirrhotics with depressed reticuloendothelial system phagocytic activity are at great risk to acquire bacteremia, and that reticuloendothelial system phagocytic activity has prognostic value in cirrhosis.

[A method for permanent fixation of eggs of enteroparasitic helminths].

The results using the Hoyer method for examining eggs of helminths enteroparasites are presented. This method is particularly suited for teaching and on research purposes. Using this technique in fecal sample containing eggs of A. lumbricoides, T. trichiura, Uncinaria sp., Taenia sp., Diphyllobothrium sp., H. nana, H. diminuta and F. hepatica allowed the correct identification of then after 24 hours up to 180 days after the samples were obtained.

Determinants of quantitative sensory testing in non-neuropathic individuals.

We have studied characteristics of subjects that could potentially influence quantitative sensory measurements in 156 non-neuropathic individuals. All subjects had measurements of vibration perception at the hallux and index finger with the vibration sensitivity tester and measurements of cool and warm perception at the hallux with the thermal sensitivity tester. Age, height, and skin temperature were all inversely related (P less than 0.05) to vibration sensitivity at the hallux, while age was inversely related (P less than 0.01) to vibration sensitivity at the index finger. In multiple regression analyses the significant associations persisted (P less than 0.01). Neither cigarette smoking nor the occasional use of alcohol were related to the quantitative sensory indices. These data indicate that age, height, and skin temperature should be considered in the assessment of vibration perception and that thermal sensitivity is relatively resistant to these factors.

The role of fine needle aspiration biopsy cytology in the evaluation of the clinically solitary thyroid nodule.

The imputation that a clinically solitary nodule is a suspicious sign of carcinoma has been the cause of too many surgical procedures as well as the subject of much controversy. This study evaluated the effectiveness of fine needle aspiration (FNA) biopsy cytology in diagnosing the uninodular goiters in 286 patients who presented with clinically solitary nodules. The final diagnoses in these cases included carcinoma (4.7%), adenoma (6.3%), autonomous nodule (11.0%), colloid goiter (45.8%), colloid cyst (17.4%) and chronic thyroiditis (13.4%). The proportion of patients with cancer in this group was the same as in patients with multinodular and diffuse goiters. These findings call attention to (1) the fact that any thyroid disease may appear as a uninodular goiter and (2) the frequency with which lymphocytic thyroiditis was cytologically diagnosed, even in cases with negative antibody titers. The cytologic diagnosis of benign disease has contributed to a reduction in the number of unnecessary surgical procedures; only 24.1% of our patients with uninodular goiters underwent surgery.
