[Diagnostic clarifications and follow-up of 105 patients with syncope].

105 patients with syncope (56 males and 49 females aged 15-87 years) were followed up for 15 +/- 8 months after their first visit in an outpatient clinic. Diagnosis after initial evaluation was syncope of unknown origin (n = 24), vasodepressor syncope (n = 18), orthostatic syncope (n = 18), syncope of cardiac origin (n = 16), seizure disorder (n = 13), syncope occurring during hyperventilation (n = 6), micturition syncope (n = 6), cough syncope (n = 2), and vertebrobasilar transient ischemic attack (n = 2). In 55% of patients the diagnosis was based on the patient's history alone and only in 23 patients was additional laboratory workup of diagnostic importance. During follow-up 4 patients died, but only in one was death related to the syncope (recurrent ventricular tachycardial). In 1 patient the initial diagnosis after follow-up had to be changed (from syncope of unknown origin to cardiogenic syncope) due to sick sinus syndrome. In patients with syncope the history should be carefully evaluated, since it is diagnostic in more than half of the cases. An additional diagnosis workup including resting and 24-hour ECG, as well as EEG examinations, should be ordered not as screening but only in selected patients.

[Advantages and disadvantages of general radiomanometry in biliary surgery].

The records of 1459 cholecystectomies from 1975 through 1980 were reviewed and the data of the separate one-year periods compared. The use of operative cholangiography rose progressively from 31 to 91% and this change was associated with an increase of common-duct stones and a decrease in number of unnecessary choledochotomies. There was an increasing number of patients undergoing probing of the common duct. Sphincterotomies averaged 10% and there was an average of 3.49% of primary stenosis of the papilla of Vater. Overall mortality was 1.09% for cholecystectomy alone, 3.91% when choledochotomy was done and 4.26% for sphincterotomy.

[Diagnosis of retroperitoneal and intrathoracic Schwannomas (author's transl)].

Retroperitoneal and intrathoracic Schwannomas are difficult to diagnose in the preoperative stage. The following methods of examination may be used: 1. X-ray examinations to exclude diseases of the internal organs and to establish displacements.--2. Computertomography to determine localisation and size of the tumour.--3. Immediate section for microscopic examination under surgery to determine the dignity of the tumour and needlebiopsy with histological and cytological examination to obtain an exact diagnosis.

[Indications and technic of endoscopic laser therapy].

This report relates experiences with the Neodymium-YAG-laser in the treatment of upper gastrointestinal tract hemorrhage. Careful patient preparation within the framework of an overall therapeutic plan and the combination of various therapeutic approaches are prerequisites for a successful application of the endoscopic laser. Best suited for this mode of treatment are bleeding ulcers, least suited massive gastric mucosal hemorrhages (stress ulcers).
