Arteriovenous fistulas after median sternotomy--report of 2 cases and review of the literature.

We report 2 cases of arteriovenous fistulas as a very rare complication following median sternotomy. In the first case a fistula was observed between the right internal mammary artery and vein caused by a sternal wire. The second patient developed a fistula between the innominate artery and left innominate vein after suture repair of a slight hemorrhage from the innominate vein. The latter localization has not been previously described. In both cases the fistulas were discovered by a continuous murmur appearing 12 and 11 days postoperatively. The therapy consisted of resternotomy and resection of the fistulas. The literature is reviewed and etiology, diagnosis and therapy are discussed.

[A pragmatic assessment of sense of smell].

A new clinical odour test was developed and validated in a clinical trial including 50 healthy volunteers as well as 35 patients suffering from smell disorders. The aromas were incorporated in an inert gas under disclosure from oxygen, avoiding impairment caused by oxidative transformation. Conventional smell test by Elsberg bottles was used as standard. The spray test proved to be equally effective in discriminating subjects suffering from smell disorders as the standard procedure. Moreover the spray test is feasible and long term constance of the substances is guaranteed.

Device for monitoring locomotor activity of 120 animals: motility of offspring of dams exposed to haloperidol.

A device using four infra-red light photocells per cage which monitor simultaneously the motility of 120 individual rodents, was developed at our institute. The measuring interval can be chosen freely, the starting time may be set freely and the test may last several weeks. The motility of the offspring (day 35-39) of dams exposed to the neuroleptic haloperidol during lactation was investigated, because of the important stage of the brain development in the neonatal age and of the relatively high concentration of haloperidol in the milk. Mice dams were treated sc. with a single dose (1.5 mg/kg) of haloperidol postnatally on day 3 to 6. Compared to the controls the body weight gain of treated (via milk) pups was reduced (controls = 72%, haloperidol = 30%) and the mortality was higher (controls = 10%, haloperidol = 77%) during the treatment period. The quantitative and qualitative analysis of the locomotor activity from day 35 to day 39 shows a distinct shift in the kind of motility: the locomotor activity of haloperidol offspring is significantly reduced while the number of activity phases is increased. It appears that there is a persistent alteration in the central nervous system that leads to a higher frequency but a lower efficacy of activity.

Transcellular ion currents during sporangium development in the water mould Achlya bisexualis.

Changes in the pattern of electric currents that accompany the transformation of growing hyphae of Achlya bisexualis into sporangia have been examined. When hyphae were transferred to a non-nutrient buffer, they continued to extend for several hours and then gave rise to sporangia. Throughout this process, current (positive charge) flowed into the apical region that corresponds approximately to the future sporangium. The current ceased after the crosswall appeared. The sporangium then remained electrically quiescent, except for a brief intense burst of outward current at the 'homogeneous' stage of spore cleavage. The inward current during sporangium formation largely represents an influx of protons. Addition of nitrate abolished the flow of electric current with little effect on sporulation. The late burst of outward current is most probably an artefact, generated by the discharge of salts from the sporangial vacuole. The transcellular electric current apparently plays no role in sporangium formation or in spore cleavage. Calcium ions, however, are required and may traverse the plasma membrane.

[Development, structure and initial evaluation of a comprehensive psychosocial care program for children with cancer and their families].

The development and trial of a model care program carried out by psychologists in close collaboration with oncologists aimed at the comprehensive in- and off-ward care of children with cancer and their families is reported. A structured psychosocial anamnesis in interview-form ("Aufnahmebogen") and an information list ("Checkliste") are used shortly after diagnosis to identify problems and to systematically inform on psychosocial support schemes. The procedure allows a comprehensive, early and preventative psychosocial care. An evaluation of 646 carefully documented contacts was carried out according to the criteria: number of contacts, initiator, person contacted, reason for contact and function of the psychologist. Various prerequisites important for the integration of psychologists into the oncological team are specified and discussed.

[3 osteosarcoma therapy dropouts. Clinical cases].

We report on 3 cases of male late-adolescent/young-adult osteogenic sarcoma patients who refused complete chemical therapy according to the treatment plan "COSS 80". They all originated from disturbed family situations. Characteristic was their early independence and autonomy, dominance tendency, low level of self control, and high level of sociability. Primary motivation for refusal was the experienced intolerability of the chemotherapeutical side-effects and extreme fear of medical procedures (e.g. injections).

Decreased insulin sensitivity of forearm muscle in myotonic dystrophy.

Previous studies of patients with myotonic dystrophy have demonstrated hyperinsulinism after glucose loading. This hyperinsulinism has been attributed by some investigators to tissue insulin resistance. We have directly studied insulin sensitivity of forearm muscle in patients having such hyperinsulinism. The effect of an intrabrachial arterial insulin infusion (100 mu U/kg per min) on glucose uptake was determined in six cases of myotonic dystrophy, six normal subjects, and in seven disease control subjects with myotonia or wasting from other disorders. There was no significant difference in insulin tolerance comparing myotonic dystrophy patients to the normal and disease control groups. Glucose tolerance and basal insulin levels were normal in the myotonic dystrophy patients, but hyperinsulinism occurred after glucose ingestion. After 25 min of intra-arterial insulin, the mean peak muscle glucose uptake in myotonic dystrophy was 2.54 +/- 0.54 mu mol/min per 100 ml forearm compared to 5.24 +/- 0.86 mu mol/min per 100 ml for disease controls (P is less than 0.05). Myotonic dystrophy patients showed a peak glucose uptake increment of only 2.6 +/- 0.2-fold over basal contrasted with the disease control value of 6.5 +/- 1.0-fold (P is less than 0.02) and the normal control value of 8.8 +/- 1.1-fold (P is less than 0.01). Thus, there was an absolute as well as a relative decrease in muscle insulin sensitivity in myotonic dystrophy patients compared to both control groups. The peak increments in arterio-superficial venous glucose concentration differences after insulin infusion were not significantly different comparing myotonic dystrophy and control groups. These data suggest that in myotonic dystrophy, there is insulin insensitivity of skeletal muscle.

[Areas of psychosocial care of children with cancer and their families. Evaluation of a model of comprehensive care].

98 children with cancer and 7 children with other serious hematologic or oncologic illnesses and their families are counselled by psychologists in close teamwork with pediatric oncologists within the framework of a clinical pilot study in 1982. The problems and questions of interest posed on the psychologists in 646 such contacts are classified and presented according to frequency of problems and contacts. The resulting large number and variety of concerns demonstrates that in addition to the direct illness- and treatment-related coping strategies (52 %) and related social/economic questions (30 %), a considerable portion of the problems can be classified within the realm of the clinical psychologist: behavioral disorders (37 %), family problems (33 %), late effects (26 %).

[Detection of fetal urinary obstructions requiring therapy in routine prenatal ultrasound].

Routine ultrasound during pregnancy is also performed for the detection of fetal malformations. This retrospective study comprises 31 children treated for urinary tract infection within 19 months postnatally. The charts of the young patients were evaluated how often a dilatation of the upper urinary tract (more than 3 cm in diameter) was observed already during pregnancy. In 18 patients (58%) the dilatation was detected antenatally and was missed in 13 (42%). Reasons of the low sensitivity may include low technical standard of the sonographic equipment and limited skill of the investigators.
