Joint association of alcohol and folate intake with risk of major chronic disease in women.

Alcohol interferes with folate metabolism and has opposing effects on the risks of cardiovascular disease and cancer. The authors examined the joint association of alcohol and folate intake with risk of major chronic disease, defined as fatal or nonfatal cardiovascular disease or cancer, or other nontraumatic death. This study included 83,929 women aged 34-59 years with no previous history of cardiovascular disease or cancer who provided dietary data in 1980. During 16 years of follow-up, the authors documented 10,666 new cases of major chronic disease. Overall, heavy drinkers (>30 g/day) with a lower total folate intake (<180 micro g/day) had the highest risk; in comparison with abstainers with a folate intake of 400-599 micro g/day, the multivariate relative risk was 1.36 (95% confidence interval: 1.10, 1.70). However, the increased risk of major chronic disease associated with heavy drinking was largely diminished among women with a higher folate intake (p for interaction = 0.02). The positive association between heavy alcohol/low folate intake and risk of major chronic disease was most apparent among women younger than age 60 years. Adequate folate intake may be important in the primary prevention of overall major chronic disease in women, especially among younger women consuming more than two alcoholic drinks per day.

Open boundaries in a cellular automata model for synchronized flow: effects of nonmonotonicity.

In this paper, we have discussed the traffic situations arising from the open boundary conditions (OBC) of a cellular automata model that can reproduce the synchronized flow. The model is different from the slow-to-start (STS) model in that the upper branch of the fundamental diagram in the periodic boundary conditions (PBC) is not monotonous but has an extremum. The phase diagram and the fundamental diagram of the model in the OBC are investigated. The results are compared with those of the STS model and those in the PBC. The current in the OBC as well as the density profiles in the different phases is also investigated.

Cellular automata model simulating complex spatiotemporal structure of wide jams.

According to the empirical observation of highway traffic, inside wide moving jams there is a complex spatiotemporal structure: jam is not compact and relatively large values of the time and the distance headway are visible. We present a cellular automata model by introducing "jam headway" and "jammed status" to simulate that complex structure. Using computer simulations, the fundamental diagram, the space-time plots, the time series of the density in the jams, and the 1-min average data of this model are analyzed. It is shown that compared to other existing models, this model can display the experimental characteristics of the wide moving jams.

Folding and everting distal end of graft flap to reduce orifice stenosis following onlay urethroplasty.

AIM: Orifice stenosis remained to be a common complication of hypospadias repair. We had modified the preputial island flap urethroplasty by folding and everting the distal end of the pedicle graft flap to prevent the neo-orifice from stenosis. METHODS: Sixteen patients had undergone hypospadias repair using a modified onlay island flap technique. A urethral catheter was retained for 8 days to 10 days after operation. RESULTS: Satisfactory results were seen in all the patients with a cosmetically fine appearance. One patient had a urinary tract infection and another, urethrocutaneous fistula and both were amply treated. No glanular adhesion or stenosis occurred. A long-term follow up of 6 months to 4 years (mean: 2 years) in 15 patients did not find any complication. CONCLUSION: The modified preputial island flap urethroplasty technique is an easy, reliable and effective approach to reduce orifice stenosis in hypospadias repair.

Male genital schwannoma, review of 5 cases.

AIM: To study the clinical features of male genital schwannoma. METHODS: Five male patients with genital schwannoma admitted from 1991 to 2000 were reviewed. The lesions were located in the prostate, spermatic cord, testis or penis. Tumors were simply resected in 3 patients and radically eradicated in 2. RESULTS: The average age of the cohort was 37 years. The most common sign at presentation was a palpable genital mass accidentally discovered by the patient or detected by the physician during a physical check. Diagnosis was made through postoperative pathological examination. Follow-up ranged from 2 years to 6 years (mean 4.5 years). Four cases were cured by simple excision and 1 patient with malignant testis schwannoma died of recurrence 1 year after surgery. CONCLUSION: Owing to the lack of characteristic clinical manifestation, the final diagnosis relies on postoperative pathological examination. S-100 and vimentin are useful markers for the diagnosis of these tumors.

Nut and peanut butter consumption and risk of type 2 diabetes in women.

CONTEXT: Nuts are high in unsaturated (polyunsaturated and monounsaturated) fat and other nutrients that may improve glucose and insulin homeostasis. OBJECTIVE: To examine prospectively the relationship between nut consumption and risk of type 2 diabetes. DESIGN, SETTING, AND PARTICIPANTS: Prospective cohort study of 83 818 women from 11 states in the Nurses' Health Study. The women were aged 34 to 59 years, had no history of diabetes, cardiovascular disease, or cancer, completed a validated dietary questionnaire at baseline in 1980, and were followed up for 16 years. MAIN OUTCOME MEASURE: Incident cases of type 2 diabetes. RESULTS: We documented 3206 new cases of type 2 diabetes. Nut consumption was inversely associated with risk of type 2 diabetes after adjustment for age, body mass index (BMI), family history of diabetes, physical activity, smoking, alcohol use, and total energy intake. The multivariate relative risks (RRs) across categories of nut consumption (never/almost never, or =5 times/week) for a 28-g (1 oz) serving size were 1.0, 0.92 (95% confidence interval [CI], 0.85-1.00), 0.84 (0.95% CI, 0.76-0.93), and 0.73 (95% CI, 0.60-0.89) (P for trend <.001). Further adjustment for intakes of dietary fats, cereal fiber, and other dietary factors did not appreciably change the results. The inverse association persisted within strata defined by levels of BMI, smoking, alcohol use, and other diabetes risk factors. Consumption of peanut butter was also inversely associated with type 2 diabetes. The multivariate RR was 0.79 (95% CI, 0.68-0.91; P for trend <.001) in women consuming peanut butter 5 times or more a week (equivalent to > or =140 g [5 oz] of peanuts/week) compared with those who never/almost never ate peanut butter. CONCLUSIONS: Our findings suggest potential benefits of higher nut and peanut butter consumption in lowering risk of type 2 diabetes in women. To avoid increasing caloric intake, regular nut consumption can be recommended as a replacement for consumption of refined grain products or red or processed meats.

Cellular automata model simulating traffic interactions between on-ramp and main road.

In this paper, we study the on-ramp system using the cellular automata traffic flow model. Different from previous works, we consider not only the influence of the on-ramp flow on the main road but also the opposite influence. The update rules are given in detail and the concept of priority is introduced. The numerical simulations are carried out and the phase diagram is presented. Two different types of phase diagram are distinguished and the currents of the on-ramp system are discussed.

Capacity drop due to the traverse of pedestrians.

In this paper, we have proposed a simplified model to describe the traffic flow when there are pedestrians traversing the road. The numerical simulation shows that the capacity of the road decreases in the presence of pedestrians. If the traffic flow rate is small, the traffic flow is basically unaffected even if some pedestrians traverse the road. However, if the flow rate exceeds a critical value, the vehicles cannot pass without delay, and a traffic jam appears. We also discuss simplified conditions of the model and accordingly present a modified model, which predicts qualitatively the same results except with a different capacity.

[Vitrectomy for severe Coats' disease].

OBJECTIVE: To investigate the outcome of vitreoretinal surgery for severe Coats' disease. METHODS: From December 1995 to December 1999, trans pars plana vitrectomy was performed in 11 patients (11 eyes) diagnosed as Coats' disease. Ten of them were complicated with total retinal detachment, and the other one had tractional retinal detachment caused by epiretinal proliferation. All these patients were followed up from 3 months to 4.5 years. RESULTS: All of the patients obtained useful vision except 1 patient whose operation was given up because of severe shortening of the retina. Three patients had the visual acuity higher than 0.02, and the best vision was 0.2. Obliteration of all abnormal vessels and resorption of all exudate was found postoperatively from 1 month to 3 months. Although the retina had been reattached in 10 patients during the operation, re-detachment was found in 4 patients postoperatively. After therapy all re-detached retina was reattached. The success rate was 90.9%. CONCLUSIONS: Vitrectomy with endolaser or scleral cryotherapy is an ideal therapy for severe Coats' disease, although the visual prognosis is still limited. It can preserve the limited vision, and prevent the severe complications such as rubeosis iridis, glaucoma, and phthisis bulbi.

Platelet-derived growth factor receptor kinase inhibitor AG1295 and inhibition of experimental proliferative vitreoretinopathy.

PURPOSE: Receptor tyrosine kinase (RTK) activation is critical for growth factor-mediated cell proliferation. The present study was designed to determine the effect of tyrphostin AG1295, a selective blocker of platelet-derived growth factor (PDGF) RTK, on proliferative vitreoretinopathy (PVR) development. METHODS: Rabbit conjunctival fibroblasts cells (1 x 10(4)) were seeded into 96-well plates and maintained in Dulbecco's modified essentialmedium (DMEM) with 0.5% fetal bovine serum. The cells were exposed to 50 ng/mL PDGF-AAor PDGF-BBor phosphate-buffered saline with or without AG1295 (1 microM, 10 microM, and 100 microM). After 3 days, the viable cells in each well were measured by 3,(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl-2H-tetrazolium bromide (MTT) assay. Homologous rabbit conjunctival fibroblasts were injected intravitreally, followed by injection of 100 microM of AG1295. The development of tractional retinal detachment (TRD) was assessed to evaluate the effect of AG1295 in vivo. Electroretinography and histologic studies were performed after intravitreal injection of AG1295 into untreated eyes to evaluate toxicity. RESULTS: Two concentrations of AG1295 (10 and 100 microM) significantly inhibited rabbit conjunctival fibroblast cell growth stimulated by PDGF-AA or PDGF-BB in vitro. Development of TRD was significantly attenuated (P <.01) with 100 microM of AG1295 until day 21. No significant histologic or retinal functional damage was found in the AG1295-treated group. CONCLUSIONS: PDGF receptor specific inhibitor AG1295 attenuated PVR without significant side effects in rabbits. This reagent could be a useful treatment to prevent PVR.
