[A case of dermatofibrosarcoma protuberans who underwent chest wall reconstruction].

Dermatofibrosarcoma protuberans (DFSP) is a slowly growing tumor that occurs in the dermis, and it has a tendency to recur repeatedly. We reported a case, 50-year-old woman, who underwent chest wall reconstruction due to recurrence of DFSP. Since the first operation for the tumor 35 years ago, she had undergone surgical procedures 30 times. We had to resect parts of the rib cartilages and the sternum because we suspected the tumor had reached to the anterior mediastinum. There has been no recurrence for 8 months since this operation. Importance of extensive resection of the tumor to prevent a recurrence and long term follow-up should be stress.

[A case of deep neck abscess and acute mediastinitis, secondary to peritonsillar abscess].

A 60-year-old female visited a hospital complaining of fever and pharyngeal pain. She was diagnosed as peritonsillar abscess. Initial conservative treatment was not curative, and deep neck and mediastinal abscess developed. After cervical drainage, she was referred to our hospital. Drainage tube was inserted via epigastrium into anterior mediastinum upwardly under local anesthesia. Postoperative course was uneventful, and she was cured and discharged after about 1 month of hospitalization. We stressed the importance of recognition of the mediastinitis as a complication of cervical infections. And immediate drainage procedure is required as soon as the diagnosis is established.

A cooperative randomized controlled study of adjuvant chemoendocrine therapy for breast cancer in Japan.

A randomized trial was carried out in 55 Japanese institutions to examine the significance of postoperative adjuvant chemoendocrine therapy in Stages II and IIIA breast cancer patients after radical mastectomy. A total of 619 patients were entered from November 1982 to November 1984, of whom 554 were eligible. Patients were administered MMC (13 mg/m2) after surgery, and were randomized by the envelope method into either group A (ftorafur, 400 mg/m2/day; 275 patients), or group B (ftorafur, 400 mg/m2/day, and tamoxifen, 20 mg/day; 279 patients). Treatment was continued for 1 year from postoperative day 7. There were no significant differences between the two groups with regard to age, menopausal status, ER status, method of operation, stage, or number of positive lymph nodes. There was also no significant difference between the two groups in the 5-year survival and disease-free survival (DFS) rates (Kaplan-Meier method). Stratification by ER status, nodal status, or menopausal status produced no significant differences between the groups. However, postmenopausal patients and lymph node-positive patients who were also ER-positive showed a significantly higher DFS in group B. Furthermore, group B in ER-positive patients with four or more positive lymph nodes also had a higher survival rate.

[An autopsy study of the mode of cancer metastasis on the esophagectomied patients for esophageal cancer].

The records of autopsy on 43 patients underwent esophagectomy were evaluated retrospectively. Fifteen patients were performed palliative surgical treatment and 28 were curative, there were 41 men and 2 women in the series. The mean age at the first operation was 59.5 yr. The distribution of the primary lesions were 1 cervical, 40 thoracic (3 upper intra-thoracic, 28 middle intra-thoracic and 9 lower intra-thoracic) and 2 abdominal esophagus. There were 42 cases of squamous cell carcinoma (13 well differentiated types, 25 moderately differentiated types and 4 poorly differentiated types) and 1 adenosquamous cell carcinoma. Their stage classification were 0 for 2 patients, I for 1, III for 13 and IV for 27. In cases of palliative surgical treatment, the residual tumor were present on 6 cases in trachea or bronchus and 2 in aorta. Others were the rest of metastatic nodes in 3 cases, positive surgical margin in 3 and visceral metastasis and peritoneal dissemination in 1. At autopsy, 14 patients (93%) had distant organ metastases, 11 (73%) local recurrent tumor, 11 (73%) node metastases and 9 (60%) disseminations. In cases of curative surgical treatment, the first recurrent sites were as follows. Lymph node metastasis observed in 20 cases (71%), local recurrence in 9 cases (32%), distant organ metastases in 8 cases (28%) and disseminations in 5 cases (18%). While at autopsy, 24 patients (86%) had lymph node metastases, 20 (71%) distant organ metastases, 15 (54%) local recurrent tumors and 11 (39%) disseminations. On 10 cases whose first recurrent site was observed only at lymph node, 9 cases (90%) had distant organ metastases at autopsy. In both cases of palliative surgical treatment and of curative surgical treatment, the most dominant metastatic site at autopsy was distant organ metastasis. Therefore, in the future, distant organ metastasis must be an important target for esophageal cancer treatment.

An evaluation of symptoms and performance status in patients after esophagectomy for esophageal cancer from the viewpoint of the patient.

We used a questionnaire method to investigate the complaints and the life conditions of 81 patients after esophagectomy for esophageal cancer. Patients were separated into three groups according to their follow-up time: less than 2 years, more than 2 years but less than 5 years, and more than 5 years after surgery. The distribution of complaints was similar regardless of the time elapsed since surgery. Only 22% of all patients had no complaints. Abdominal fullness and stenotic sensation were the major complaints. Body weight was unstable for 2 years following surgery, but subsequently stabilized. The content of meals, volume of meals, and Performance Status (PS) did not differ significantly among the groups and were indicative of satisfactory recovery. Our results suggest that in contrast to other surgeries, postoperative symptoms persist in esophagectomyed patients with long survival and that about half of the cases whose follow-up time was less than 2 years have unstable body weight.

The intrapleural administration of recombinant interleukin-2 (rIL-2) to patients with malignant pleural effusion: clinical trials.

Recombinant IL-2 (rIL-2) was administered intrapleurally according to an original protocol to 11 patients with malignant pleural effusion, 7 of whom suffered from breast cancer and 4 from esophageal cancer. The pleural effusions either disappeared or decreased roentgenographically, and malignant cells disappeared from all 13 pleural cavities in the 11 patients, confirming the validity of this therapy to be 100%. The mean survival time from the initial administration of rIL-2 was 15.9 months. We ensured that the concentration of IL-2 in the effusion was maintained at a high level for a sufficient period of time, and that the lymphokine-activated killer (LAK) activity of lymphocytes in the effusion was augmented. Fever, eosinophilia, and a transient increase in the pleural effusion were the main side effects, but the symptoms were temporary and not serious. The results of this study therefore suggest the efficacy of intrapleural rIL-2 for patients with malignant pleural effusion.

[Prevalence of bacterial pathogens and antimicrobial susceptibility: a multicenter study in Akita prefecture].

We report the prevalence of bacterial pathogens isolated from various clinical specimens in 17 medical centers in Akita during the period of December 1990 to February 1991, and their antimicrobial susceptibility. The results were as follows: 1. The total number of isolates was 8,929 and common bacteria were Staphylococcus aureus (18.7%), Pseudomonas aeruginosa (11.8%), Escherichia coli (10.1%), coagulase negative staphylococcus (6.7%) and Enterococcus faecalis (5.0%). This is similar to our previous result which was reported in the same season in 1987. 2. S. aureus was a common isolate from sputum, throat swab, pus, and blood. 53% of isolated S. aureus were methicillin-resistant. Arbekacin (ABK) had a high degree of activity (99%) against methicillin-resistant S. aureus (MRSA). Minocycline (MINO) retained activity against 70% of MRSA strains, and quinolones only 20%. 3. P. aeruginosa and E. coli were frequently found in urine from the patients in the small hospitals. More than 80% of the P. aeruginosa strains were sensitive to imipenem (IPM), cefsulodin (CFS) and amikacin (AMK). The susceptibility of P. aeruginosa to ceftazidime (CAZ) and piperacillin (PIPC) had been decreasing for the last 3 years. 4. CNS were frequently found in blood, pus, and pleural effusion from the patients in the large hospitals. The isolates were resistant to most antibiotics. 5. Enterococcus was common isolates from urine and ascites, the susceptibility to cephem was low. 6. We conclude that attention should be specially paid to S. aureus, P. aeruginosa CNS and E. Faecalis.

Preparation of highly purified human myelin oligodendrocyte glycoprotein in quantities sufficient for encephalitogenicity and immunogenicity studies.

Myelin oligodendrocyte glycoprotein, (MOG), a quantitatively minor central nervous system (CNS) myelin component, is a candidate target antigen for autoimmune-mediated demyelination. It is a highly hydrophobic protein present in very small amounts in CNS tissue and thereby difficult to purify. Our aim was to devise a purification procedure that would yield sufficient quantities of highly purified MOG to subsequently test its potential encephalitogenic activity, as well as investigate the humoral and cell-mediated responses to this antigen in naturally occurring and experimentally induced autoimmune demyelinating diseases. MOG was purified from human CNS white matter using immunoaffinity chromatography, a procedure that gave a final yield of MOG corresponding to 0.02% total white matter protein. The final product, which migrated as two bands of molecular weight 28 kDa and 58 kDa, was highly pure as shown also by specific reactivity with monoclonal anti-MOG antibodies on immunoblots in the absence of any detectable reactivity with antibodies specific for myelin basic protein, proteolipid protein and myelin-associated glycoprotein. Partial amino acid sequence was obtained from both MOG bands separated by SDS-PAGE and electroblotted onto PVDF. The sequence of the first 17 N-terminal amino acids had approximately 55% homology with the reported rat MOG sequence deduced from the cloned cDNA sequence; small internal sequences obtained showed also very high homology. Our purified MOG preparations have been used to investigate T cell response to MOG by peripheral blood lymphocytes of multiple sclerosis patients and to induce a relapsing remitting demyelinating disease in Lewis rats.

[The effectiveness of ulinastatin on granulocyte H2O2 production in acute lung injury rat model].

Oxygen radicals produced by granulocytes play an important role in acute lung injury. The effectiveness of ulinastatin in adult respiratory distress syndrome and an acute lung injury animal model has been reported. We investigated the effect of ulinastatin on oxygen radical production in the intact pulmonary circulation of a rat acute lung injury model using our new method, which demonstrated the granulocyte adhesion and H2O2 production in vivo. The pulmonary microcirculation was observed by intravital fluorescence microscopy and a TV monitor using a SIT TV camera. Then images were recorded on a videotape. The granulocytes and H2O2 were stained with an intravenous injection of acridine red and dichlorofluorescin diacetate, respectively. The images which were recorded on the videotape were digitized by a computer image digitizer and analyzed with a personal computer and the software Ripp II. When the rat received a continuous infusion of endotoxin (4.5 mg/kg/hour) for 2 hours, the granulocytes adhered to the pulmonary capillaries and generated H2O2 (n = 25 p < 0.001 respectively, compared with the control rat which received saline alone). Then, the acute lung injury model rat was infused with 50,000 U/kg of ulinastatin just before endotoxin infusion and also received a continuous infusion of ulinastatin (50,000 U/kg/hour) for 2 hours. Ulinastatin inhibited the granulocyte adhesion to the pulmonary capillaries (n = 25, inhibition rate: 46% p < 0.001) and H2O2 production (n = 25, inhibition rate; 51% p < 0.001) caused by the continuous infusion of endotoxin. In conclusion, ulinastatin significantly inhibits the granulocyte adhesion and H2O2 production in the intact pulmonary circulation of a rat acute lung injury model.

[A resected case of well-differentiated adenocarcinoma simulating fetal lung tubules which is a variant of pulmonary blastoma].

A case of well-differentiated adenocarcinoma simulating fetal lung tubules which is a variant of pulmonary blastoma is reported. A 31-year-old female with an abnormal shadow in the right upper lung field detected by chest X-P during a follow-up for sarcoidosis during pregnancy underwent right upper lobectomy. Post operative histological diagnosis was well-differentiated adenocarcinoma simulating fetal lung tubules. This tumor might be a variant of pulmonary blastoma lacking the sarcomatous features and showing one-sided development of the epithelial component. However, it has been reported by various names, and classifications of pulmonary blastoma differed slightly depending on the reporter. There are also some discrepancies in the clinical course and prognosis of the tumor. Therefore it is considered that further examination of this tumor is necessary.

[Study of metabolic dynamics under severe surgical stress--investigation by the method of respiratory gas analysis].

To elucidate quantitative analysis of the hypermetabolic status and classification of energy substrates kinetics, the metabolic dynamics after radical operation for thoracic esophageal cancer was investigated in 10 patients primarily by the method of respiratory gas analysis. 1) REE showed almost 30 kcal/kg/day and no significant change on each postoperative day. Energy intake in addition to surgical stress influenced the metabolic dynamics (specific dynamic action: SDA). So authors originally devised a postoperative metabolic index "PMI" in consideration for the effect of SDA. PMI is defined; PMI = (REE kcal/day-BEE kcal/day)/Energy Intake (non-protein kcal/day). according to PMI, it was clarified that the hypermetabolic status reached a maximum value on the first postoperative day, showed a steep decrease up to the 3rd postoperative day and afterwards slow recovery. PMI revealed a significant correlation (r = 0.792, p < 0.001) with daily urinary epinephrine excretion in the immediate postoperative period (up to the 3rd day). These results suggest PMI might be useful in the quantitative evaluation of the hypermetabolic status under surgical stress. 2) Energy substrates kinetics was clearly classified into six patterns according to three parameters such as energy balance, glucose balance, and predominant fat metabolism (fat oxidation or synthesis).

[The significance of body weight measurement of esophageal cancer patients in intra- and postoperative fluid administration].

We tried to clarify the intra- and postoperative pathological state in resected thoracic esophageal cancer patients and analyze the particularity of fluid administration by measuring body weight at certain time serially. We studied 26 resected thoracic esophageal cancer patients between September, 1989 and January, 1991 who underwent an operation through right thoracotomy prior to laparotomy at the same time, and all cases were under general anesthesia combined with epidural anesthesia. We measured body weight preoperatively, just after operation, on the first, second and third postoperative days by means of LIFTER (Ted Hoyer & Co. Inc), and also calculated water balance and measured pressures and cardiac output by Swan-Ganz catheter in 19 cases. As a result, change in body weight was remarkable in resected thoracic esophageal cancer patients intra- and postoperatively. The peak of weight gain was noticed and they gained about 3 kilograms in weight on the 1st postoperative day, while in circulation hypovolemia was found on the data by means of Swan-Ganz catheter. There was a high correlation between rate of change in weight (kg/hr) and water balance (ml/kg/hr) (r = 0.91). We calculated insensible loss by the difference between water balance and body weight. It was about 1.73 intraoperatively, 0 postoperatively, 0.35-0.5 ml/kg/hr on from the 1st through the 3rd postoperative day. It is concluded that body weight measurement is a very easy, precise and useful bedside technique in the intra- and postoperative fluid administration of esophageal cancer patients, because it is very important not only to understand the circulation volume but also to know the interstitial fluid change.

[Evaluation of the right ventricular function in postoperative esophageal cancer patients].

We evaluated the right ventricular function in pre- and post-operative esophageal cancer patients who had no complications after surgery by measurement of cardiopulmonary hemodynamics, right ventricular ejection fraction (RVEF) and right ventricular end-diastolic volume index (RVEDVI) using Swan-Ganz catheter and the thermodilution technique. The measurement of RVEF, RVEDVI and other cardiopulmonary hemodynamics were performed from the preoperative day to the 3rd postoperative day. The values of RVEF were stable about 40% from preoperative day to 3rd. RVEDVI was varied at 93.6 +/- 26.3 ml/m2 (preoperative day), 83.6 +/- 15.0 ml/m2 (after surgery), 70.2 +/- 10.6 mg/m2 (morning of the 1st POD), 95.6 +/- 19.4 ml/m2 (evening of the 1st POD), 103.2 +/- 17.9 m/lm2 (morning of the 2nd POD), 1108.3 +/- 14.2 ml/m2 (evening of the 2nd POD) and 112.4 +/- 31.0 ml/m2 (morning of the 3rd POD). The values of RVEDVI on the morning of the 1st POD were statistically lower than those at other times (p < 0.05). The values of RVEDVI were not correlated with mean pulmonary arterial pressure and pulmonary arteriolar resistance which were the afterload of the right ventricle. These data suggest that the right ventricular function of the patients who underwent esophagectomy with no complication does not change and that the changes of RVEDVI are influenced by the preload of the right ventricle.

Locoregional control for esophageal carcinoma treated with irradiation following surgery.

Locoregional failure was analyzed in a total of 34 esophageal carcinoma patients treated with postoperative prophylactic irradiation following curative surgery. All patients had squamous cell carcinoma and no prior treatment. Twelve patients had subsequent lymph node metastasis in the follow-up period. In the 12 patients with node metastasis, there were 5 instances of supraclavicular node metastasis, 7 instances of thoracic inlet node (uppermost part of mediastinum) metastasis, and 3 instances of mediastinal node metastasis. Three patients had 2 metastatic nodes and 9 patients had 1 metastatic node. Intervals between surgery and recurrence were a median of 12 months for mediastinal nodes, 19 months for thoracic inlet nodes, and 26 months for supraclavicular nodes. Ten (52.6%) of the 19 patients treated by using a 12-MeV electron beam had metastatic involvement at the supraclavicular and thoracic inlet nodes. On the other hand, 2 (13.3%) of the 15 patients had the above-described lymph node metastases when treated by using 15-MeV electron, 18-MeV electron, or 10-MV photon beams. The difference in the metastatic rate between these two groups was statistically significant at the level of p < 0.05 (chi-square test); this seems to be attributable mainly to the dose level at deep region. Doses of over 50 Gy to the thoracic inlet and supraclavicular nodes at deep location were necessary to reduce metastasis.

[Cervical esophago-gastrostomy via the posterior mediastinal route for carcinoma of the thoracic esophagus].

From 1981 to 1990, 297 cases of carcinoma of the thoracic esophagus were resected in our department. Among these cases, 225 cases (84.3%) were reconstructed cervical esophagogastrostomy via the posterior mediastinal route. In this paper, these 225 cases were analysed to evaluate the usefulness of this procedure which was developed by S. Abo in 1975. Operative mortality was 4.4% Anastomotic leakage occurred in 24.4% of the cases, but the majority of the cases were treated successfully without any drainage and 83.3% of the cases could start their diet within one month after operation. The volume of oral intake by the patients after discharge increased satisfactorily (1,500-1,800 kcal/day), but the body weight decreased gradually. So the ambulatory enteral nutritional support was started and proved useful for maintaining their body weight. Alkaline reflux evaluated using 24-hour pH-monitoring system was observed in about half of the cases but the degree of the alkaline shift was not strong. Some complaints (heart burn in 6.3%, regurgitation in 21.9%, feeling of fullness after eating in 40.6%, stenotic sensation in 20.7%) were present but not serious.
