[The anticardiolipin syndrome after breast reconstruction].

In a 38-year-old patient 17 years previously silicone prostheses of both breasts were implanted. Three years after operation spontaneous abortions developed, later repeated venous and arterial thromboses. Laboratory examinations revealed antibodies against cardiolipin and the diagnosis of anticardiolipin syndrome was established. The authors discuss the relationship between the performed operation and the present disease.

[Aspects of treatment of hypertension in rheumatic diseases].

The author deals with the reasons of the frequent incidence of hypertension in rheumatic diseases and some special aspects of its therapy (nephrogenic hypertension in systemic diseases, weakening of the action of antihypertensive drugs by nonsteroidal antiinflammatory drugs, chronic corticosteroid treatment and the unsuitability of some antihypertensive drugs in different rheumatic diseases).

[Amyloidosis in plasmacytic myeloma with dominant manifestations of joint involvement].

The authors describe a case of plasmocytic myeloma complicated by the development of systemic amyloidosis. The symptoms were dominated by marked macroglossia and arthropathy which by the objective finding and subjective manifestations imitated rheumatoid arthritis. In the discussion the authors emphasize general manifestations of amyloid arthropathy.

[Systemic rheumatic diseases and diabetes mellitus].

In a two-year retrospective study the incidence of diabetes was investigated in 481 patients with systemic rheumatic disease. The control group was formed by 120 patients with degenerative articular disease of the lower extremities. The authors found a significantly higher incidence of diabetes mellitus in both groups, as compared with the population of the Czech Socialist Republic, whereby in patients with systemic rheumatic disease probably the relatively frequent administration of glucocorticoids plays a part. The remarkably high incidence of insulin-dependent diabetes in 16% of patients with polymyositis might suggest a possible co-existence of these two autoimmune diseases.

Detection of Borrelia in the synovial tissue from a patient with Lyme borreliosis by electron microscopy.

We describe a woman who suffered for several years from joint pain with subsequent development of arthritis of her left knee. Because of these symptoms she was subjected to repeated studies including arthroscopy and menisectomy. IgG antibodies to Borrelia determined by ELISA were reported to be present and showed an increase of IgG titer to 1:1024. Histology from the last synovial biopsy disclosed only evidence of a nonspecific synovitis and marked inflammatory infiltration with lymphocytes and plasma cells. Borrelia-like structures were detected electron microscopically on semithin Epon sections from the lumen of the strongly infiltrated blood capillary. In our case the detection of Borrelia indicated the basis for increase of antibodies against Borreliae. It established the correct diagnosis and led to specific therapy of the disease after several years of unsuccessful attempts to control it.

Anticardiolipin syndrome in plastic surgery of the breast.

A 38-year-old female patient had bilateral silicone-made prosthesis implantation performed 17 years ago. Three years after surgery, she had a series of spontaneous miscarriages, later she developed recurrent venous and arterial thromboses. Laboratory tests detected antibodies against cardiolipin and the diagnosis of cardiolipin syndrome was established. The authors discuss the possible association between surgery and the patient's current disease.

Benign hypergammaglobulinemic purpura in a patient with ulcerative colitis and peripheric arthritis.

A 32-year-old patient is described presenting with ulcerative colitis and oligoarthritis who later developed benign hypergammaglobulinemic purpura. This diagnosis was confirmed by clinical investigation, immunological findings as well as by histological and immunofluorescent examination. He was successfully treated with steroids and azathioprin. The relevant literature is reviewed.

Acute cricopharyngeal obstruction in dermatomyositis.

We report a case of a 44-year-old woman with dermatomyositis complicated by acute total cricopharyngeal obstruction resistant to corticosteroid therapy. This condition was successfully resolved by cricopharyngeal myotomy.
