[The value of endoscopy in acute hemorrhage of the upper gastro intestinal tract].

1808 esophage-gastro-bulboscopies were performed, in 156 cases (8.0%) because of acute haemorrhage, from 1975 to 1979. In 14 cases (9.0%) no lesion could be detected. In 3 patients (1.9%) an exact localization of the bleeding lesion could not be attained. Gastric and duodenal ulcerations were (57.1%) the most frequent causes of haemorrhage, followed by erosive lesions (12.8%). Multiple bleeding sources were realized in 24 patients (15.4%). At present endoscopy is the initial method for diagnosing the causes of acute bleeding in the upper digestive tract.

Universal structural features of prokaryotic and eukaryotic ribosomal 5S RNA derived from comparative analysis of their sequences.

An extensive comparative analysis of more than fifty available sequences of ribosomal 5S RNA has been made. Both for prokaryotic and eukaryotic 5S RNA a generalized secondary structure is presented which is similar to that suggested by Nishikawa and Takemura modified in few positions only. Both generalized secondary structures contain five main helical regions and a high base-pairing content of about 65 +/- 5%. The general structural architecture of prokaryotic and eukaryotic 5S RNA molecules appears to be very similar with minor modifications within particular subgroups of organisms. Conserved and semiconserved nucleotides are accumulated in the single stranded parts of 5S RNA. Functional importance was suggested for some of these regions; other short conserved nucleotide stretches may be involved in the folding of 5S RNA molecules. In particular, we propose a tertiary base-pairing interaction between the universal invariant GUA sequence (positions 76-78 and 75-77 in prokaryotic and eukaryotic 5S RNA, respectively) and the complementary conserved CPuU sequence (positions 38-40 and 36-38) in a parallel manner. A molecular model of the 5S RNA of human KB cells was constructed, which verifies the proposed tertiary interaction, probably stabilizing the two neighboured helices E and D and a stacking arrangement of the bases in the sequence positions 67-108 (and 70-106) in eukaryotic (and prokaryotic) 5S RNAs, respectively.

Temperature behaviour of human serum albumin.

Structural alterations of albumin, their dependence on concentration and the role of free --SH groups at thermal denaturation, as well as the reversibility of thermally induced structural changes, were studied. Application of various physical methods provides information on a series of structural parameters in a major concentration range. Apart from changes of the helix content, heat treatment gives rise to beta structures which are amplified on cooling and which are correlated with the aggregation of albumin. With rising temperature and concentration the proportion of beta structures and aggregates increases. At degrees of denaturation of up to 20% complete renaturation is possibly in every case. The structure content is concentration-dependent even at room temperature. It may be that intermolecular interactions induce additional alpha-helix structures which are less stable, however, than the ones stabilized by intramolecular interactions. Unfolding of the pocket containing the free --SH group of cysteine-34 enables disulphide bridges to be formed leading to stable aggregates and irreversible structural alterations. Through binding of N-ethymaleimide to free --SH groups, which blocks the formation of disulphide bridges, it is possible to prevent aggregation and irreversible conformational changes. At temperatures below 65--70 degrees C, oligomers are formed mainly via intermolecular beta structures.

[Medicamentous enzyme induction and hormonal contraception. First communication: effects of anti-epileptics on hormonal contraception of epileptics (author's transl)].

Studies were conducted into the effects of anti-epileptics on hormonal contraception of epileptics, with reference being primarily made to biochemical as well as clinicoexperimental findings regarding medication and its effects, through the mechanism of medicamentous enzyme induction, on biological transformation of steroid hormones. Disorders of bleeding, the most sensitive symptom of enzyme induction, occurred more often than they did in a control group, the difference in occurrence being secured with significance. Contraceptive protection of those patients was equally reduced. Conclusions were drawn for contraceptive practice.

[Medicamentous enzyme induction and hormonal contraception. Second communication: effects of phenobarbital and phenylbutazone on hormonal contraception (author's transl)].

The effects of phenobarbital and phenylbutazone on hormonal contraception of clinically intact women were studied for the purpose of obtaining information on consequences of medicamentous enzyme induction. Disorders of bleeding were evaluated from four groups of probands who had received complementary doses of phenobarbital or phenylbutazone along with hormonal contraception, and findings were compared with data obtained from a control group. Occurrence of medicamentous enzyme induction was clinically reflected in an accumulation of functional bleeding disorders, depending on the hormonal contraceptive used as well as on the additionally administered drug. Conclusions were drawn for contraceptive practice.

Infrared spectral studies of carbon monoxide complexes of microsomal cytochromes P-450 and P-448.

The characteristic infrared spectral parameters of the carbonyl stretching vibration (the maximum position nu CO, the band width at half height delta nu 1/2 and the apparent molar extinction coefficient epsilon M) have been determined for the CO complexes of microsomal cytochromes P-450 (PB-induced) and P-448 (3-MC-induced) from rabbits and rats. The cytochromes P-450 and P-448 from the same species as well as the cytochromes prepared by the same inducer but from the two different species yielded nu CO band parameters different from each other and from the bacterial cytochrome P-450cam [1]. These differences are discussed in connection with (1) the presence of different protein moities (multiple forms) of cytochromes, (2) changes in the order of the nearest neighbour environment (accessibility) of the CO binding site and (3) the presence of (endogenous bound substrate. The heme-carbonyl of the microsomal cytochromes senses subtle changes in the nearest heme environment by changing the solvent from H2O to D2O.

[Significance of a general umbilical-cord pH for the evaluation of acidosis morbidity].

The value of a general pH analysis of the artery umbilicalis as a parameter of the postnatal diagnostic of the newborns is shown. The classification of the values in different grades of acidity and the relation to the Apgar score show, that the light and middle-heavy acidoses are not determined sufficiently by the Apgar score. The CTG-monitoring and the stimulation of the Oxytocin had no influence on the frequency of the different grades of the acidity. The meaning of the general blood-gas analysis in the artery umbilicalis for the morbidity of acidity and the check of the obstetrical activity of the clinic is shown.

[Clinico-endoscopic problems in malignant changes of the gastric stump].

The authors give a report on occasional difficulties concerning the radiological and endoscopic clarification of the malignant changes at the gastric stump after treatment of resection. Four relevant cases are stated and explained. The endoscopy (glass fibre gastroscopy) together with the biopsy is at present the most reliable method for the diagnosis of the pathological processes in the remaining part of the stomach. A regular endoscopic and radiological control of patients with gastric resection is desirable.

[Simultaneous extra- and intrauterine pregnancy with the birth of a healthy child].

Report on the occurrence of a simultaneous intra- and extrauterine pregnancy. A live, mature infant was born at term. At the time of the operation of the ectopic pregnancy there was diagnosed the intrauterine pregnancy. The possible cause of a simultaneous intra- and extrauterine pregnancy are analysed and the clinic and diagnosis emphasized. In case of not having a child-wish the authors take into consideration a routine curettage of the uterus at the time of the operation of the ectopic pregnancy.
