[Changes of the lipoprotein pattern in acute respiratory diseases in infancy].

The lipoprotein profile during an acute respiratory disease (ARD) was determined in 75 children from 8 Dresden day-nurseries. At time "O" when the children are free of any ARD the serum concentrations of TG, TC, LDLC and Apo B are high whereas those of HDLC, Apo AI and Apo AII are age-related. The acute stage of an ARD is characterized by high levels of TG, Apo B and Apo AII and an decrease in TC, LDLC, HDLC and Apo AI. After 4 weeks of reconvalescence some of these disturbances persist.

[Diagnosis of mucoviscidosis by examination of sweat: value of conductometry].

Two methods for the determination of sodium in the sweat after stimulation by pilocarpine-iontophoresis were compared in 227 patients, among them 27 with a known cystic fibrosis. These two methods were 1. the standardized method by flame fotometry (State Pharmacopoeia of the German Democratic Republic) and 2. the determination of the sweat conductance by the new apparatus "CF-Conductognost". Both methods showed a good correlation. We recommend the conductometry for the decentralized screening because it saves time and gives results immediately. But in all patients with a questionable or pathologic value of conductance the sweat test should be repeated with the standardized method. This concerns only about 10 per cent of all patients.

[Use of analytic density gradient ultracentrifugation for the study of cholesterol distribution between the lipoprotein fractions of serum].

1 ml serum were fractionated by analytical ultracentrifugation in a density gradient in the lipoproteins chylomicron/VLDL, LDL, HDL-2 and HDL-3 and the bottom-fraction. The sudan black prestained lipoprotein fractions were separated under visual control. The cholesterol content in the different lipoproteins was determined with the methods according AB (D.L.) and with the cholesterol oxidase/catalase-method. Both methods gives good reproducible and highly correlated results. With the AB (D.L.) method about 97% of the serum cholesterol concentration can be found in the lipoprotein fractions whereas with the enzymatic method about 87% can be detected. By a correlation coefficient of r = 0.989 the AB (D.L.) method gives about 0.30 mmol/l higher cholesterol values compared with the cholesterol oxidase catalase-method. The analytical ultracentrifugation can be recommended for the investigation of the cholesterol distribution between the lipoproteins also in clinical and smaller epidemiological studies.

Lipoprotein changes in familial hypercholesterolemia after extracorporeal immunoadsorption of low density lipoproteins.

The effects of extracorporeal immunoadsorption of LDL on the lipoprotein metabolism in two patients with familial hypercholesterolemia are reported. The immunoadsorbent consisted of F(ab')2 fragments of sheep anti-LDL antibodies, which had been coupled to Sepharose CL 4B. Within a time period of 3 to 3.5 hours a mean reduction of the level of total cholesterol by 76 +/- 4 p. cent could be obtained. The level of LDL cholesterol was reduced by 78 +/- 4 p. cent and the level of apo. B by 84 +/- 5 p. cent. Both LDL and VLDL were bound to the immunoadsorbent, while HDL was predominantly lowered by the plasma-dilution, which was in the order of 20 p. cent. The same was true for other serum proteins, not related to LDL or VLDL. The relative distribution of the different lipoprotein classes was again reached 3 days after the treatment, the initial lipid and apolipoprotein levels two to three weeks after the treatment. In a long-term therapy consisting of 45 treatments with a mean interval of 18 days between two treatments a mean cholesterol lowering of 42 p. cent could be achieved. No adverse effects and no sensitization to be heterologous protein were observed.

Effects of a bezafibrate sustained release formulation on plasma lipoproteins in patients with hypercholesterolemia. Importance of timing of tablet intake for efficacy.

The therapeutic effect of a single sustained release tablet of bezafibrate (Cedur retard, 400 mg) in primary hypercholesterolemia type IIa and type IIb was investigated in a placebo-controlled randomised study comparing the efficacy of morning vs. evening intake. The decrease in total cholesterol with the morning intake was 18.5% vs. 16.5% for the evening intake (n.s.). HDL-cholesterol increased more in patients taking bezafibrate retard at morning (29.6% vs. 22.4%, p less than 0.05). Bezafibrate was well tolerated. Animal experiments and precursor studies of cholesterol synthesis in man indicate peak activity of HMG-CoA reductase between 1 a.m. and 3 a.m. The data suggest that with normal eating habits during day time other modes of action of bezafibrate besides HMG-CoA reductase inhibition such as reduction of VLDL-synthesis in the liver and an increased fractional catabolic rate, could contribute to the therapeutic effect.

Effect of anal electrostimulation with the 'Incontan' device in women with urinary incontinence.

An integrated, automatic electrical stimulator has been developed to treat urinary incontinence. A group of 121 women with either stress-, motor urge- or mixed incontinence was selected for treatment, but 23 discontinued the treatment within 3 months and were excluded from the study. In the study group of 98 women, anal stimulation was applied for an average of 9 months (range 3-36 months). Between 9 and 36 months after the treatment was completed, 91% were either continent or had improved significantly, but in 9% the stimulation had no effect. Significant improvement occurred on clinical and urodynamic testing. There were no serious side-effects. Intra-anal electrostimulation therapy provides an alternative to surgery in patients with stress incontinence, and could be the first choice of treatment in women with combined stress and motor urge incontinence.

Anal electrostimulation in urinary incontinence. Technical description of a new device.

A new electrical anal stimulator has been developed to treat urinary incontinence. The device is automatic and fully integrated with both battery and electronics in the plug. Shape, plastic material, electrodes, electrical parameters and electronic circuits have been carefully selected for optimal acceptability and therapeutic effect. A pilot study was performed in 24 women with stress, motor urge and mixed stress and motor urge incontinence where traditional incontinence therapy had failed. When the stimulator had been used for an average of 3.5 months, 25% were continent, 54% much improved and 21% without any effect of the treatment. Improved incontinence status was verified by positive changes in the urodynamic parameters in some of the patients after the treatment. No serious side effects have been observed. The present device is simple to handle for the patient and has satisfactory technical and mechanical properties. The preliminary clinical results are promising.

Electrical stimulation in multiple sclerosis. Comparison of transcutaneous electrical stimulation and epidural spinal cord stimulation.

Forty-nine multiple sclerosis patients with bladder symptoms and/or walking disability were subjected to a therapeutic trial with electrical spinal cord stimulation and transcutaneous electrical stimulation, a second aim being to compare these two treatments. A clear subjective improvement in bladder symptoms was achieved in the majority of the cases, and this was substantiated by objective parameters. In a proportion of cases a more moderate improvement seems to have been achieved in a variety of symptoms. Transcutaneous electrical stimulation seems to be a useful selection procedure for later electrical spinal cord stimulation.

Intestinal biosynthesis of apolipoproteins during active fat absorption in rabbits.

To gain some insight in the biosynthesis of apolipoproteins in small intestinal walls of rabbits we investigated the in vivo incorporation of [14C]leucine into mesenteric lymph lipoproteins during active fat absorption. The labeled amino acid was predominantly incorporated into the apo B-containing lipoproteins, but also in HDL. More than 40% of total lymph radioactivity were associated with the lipoprotein bound apolipoproteins of chylomicrons, VLDL and HDL. About half of this radioactivity was incorporated into apo B, predominantly into that of the triglyceride-rich lipoproteins. Apo A-I and apo A-IV contained about 17 and 3% of total lymph radioactivity, respectively. In contrast, no radioactivity was detectable in the gel zones corresponding to plasma apo E or lower molecular weight peptides (apo C-II and C-III). During active fat absorption the small intestine of rabbits secretes apo B and apo A into mesenteric lymph, which are obviously necessary for the lymphatic transport of lipids.

Multiple sclerosis in Iceland: 1. Evidence of a postwar epidemic.

Since 1974 we have attempted to ascertain all cases of MS beginning 1900-1975 in Iceland. As of October 1979 they numbered 168; all but 5 met all diagnostic criteria of the Schumacher Committee. Virtually all cases had been examined by a least one of the authors. Cases were few and sporadic from 1900 to 1922, then increased to a plateau for 1923-44, then again increased in 1945 with an irregular plateau thereafter. Average annual incidence rate for 1945-1954 was 3.2 per 100,000 population, significantly higher than the 1.6 for 1923-1944 or the 1.9 for 1955-1974. Age at onset was significantly decreased for cases with onset 1945-1949 and then sharply increased for those with 1950-1954 onsets. The occurrence of MS in 1945-1954 meets the criteria for a point-source epidemic, whose tail thereafter mérged into what may be "baseline" for Iceland. This postwar epidemic is then similar to that recently described for the Faroe Islands, a land that shares its history, culture, and peoples with Iceland.

The value of dorsal column stimulation in multiple sclerosis.

Ten patients with definite and one with probable MS, all markedly inflicted, but with a varying degree of motor and bladder dysfunction were subjected to spinal cord stimulation in a controlled study. None of the patients exhibited appreciable fluctuation in symptoms in the pre-study period. Bladder symptoms were most markedly influenced by electrical stimulation. The reduction in hesitancy and urgency was of great importance to the patients. In 9 of 10 patients reduction in voiding frequency took place, the all over reduction being 8%. Maximum extension torque increased by 9% and flexion torque by 29% during the stimulation when compared to the first placebo period. In selected MS patients, i.e. patients with bladder dysfunction and with a certain muscular reserve, electrical spinal cord stimulation may have an indication.
