Exercise testing of young, apparently healthy professional drivers.

OBJECTIVES: The purpose of this study was to examine the responses of young, apparently healthy professional drivers to exercise testing. METHODS: A bicycle exercise test was performed by a randomly selected group of 42 apparently healthy, male professional drivers aged 20 to 40 years and a group of 30 building workers, who formed a reference group. RESULTS: The mean maximal exercise level was significantly lower for the drivers [139 (SD 32) W] than for the referents [155 (SD 31) W] (P < 0.05). The mean systolic blood pressure and heart rate did not differ significantly between the groups at the end of the exercise, but the mean double product was significantly higher for the drivers. The mean diastolic blood pressure at the end of the exercise was 97 (SD 14) mm Hg [12.9 (SD 11.9) kPa] for the drivers and 79 (SD 19) [10.5 (SD 2.5) kPa] for the referents (P < 0.01). The exercise was stopped due to diastolic hypertensive reactions [diastolic blood pressure > 115 mm Hg (> 15.3 kPa)] in 13 (31%) of the drivers and 3 (10%) of the referents (P < 0.05). Eight of the thirteen drivers with a hypertensive reaction had three or more cardiac risk factors. CONCLUSIONS: There seems to be a high occurrence of diastolic hypertensive reactions to exercise among professional drivers. These reactions may be associated with risk of ischemic heart disease and hypertension. Further exercise testing of professional drivers is warranted. More sensitive methods may be indicated in selected cases, since the cardiovascular status of this cohort is a vital concern with respect to traffic safety.

[3 case reports of unsuccessful cadaveric pancreas transplantation].

Pancreas transplantation, according to the experiences of USA and Europe, must be done persistently, as only in this way can be acquired the precious experiences. By the gradual preparation one can acquire criticism for this more complicated explantation of graft than its implantation. Three our cases, two of which ended with exitus letalis and one with the successful kidney transplantation only, speaks in favor of the fact that the experiences must permanently be enlarged on the own casuistics, with the readiness, that in the beginning results can also be discourageous, what, unfortunately has happened in our case. In the paper was presented the clinical flow, surgical technique, immunobiological monitoring and some important complications of our first transplantations of the whole pancreas.

[New possibilities in kidney transplantation from live unrelated donors].

The kidney transplantation, as a method of medical treatment, could not be developed faster in our country for many years. A number of demands for transplantation grows much faster than our modest capability. The similar, but in a rather smaller degree, this problem occurs in some other countries in the world. The main cause is: the organization of corpse collecting and the conservatism of doctors. The latter problem, in the countries of western hemisphere, is surpassed more easily by a doctor codex and a legal obligation that involves doctors actively in this process, which is not the case in our country. The organization of corpse collecting in some states of the USA is highly developed, but however it does not give sufficient number of organs for the cadaveric transplantation. New, additional possibilities are found: the taking of even organs (kidneys) from alive unrelated donors, spouses, which excludes the possibility of greed and gives, only in the USA, about 1.500 additional alive transplantations per year. The complementary medical attitude towards the increasing of number of cadaveric transplantations is: considerably freely taking of kidneys from cadavers without tissue compatibility, only with the compatibility of blood groups of ABO system. Since the immunological criteria, in our country, are very intensified by the Zagreb immunology group, e.g. from 75-100% for alive transplantation, or the minimum of 50% tissue compatibility for cadaveric transplantation, this old-fashioned attitude has considerably lowered the number of transplantations in our country.(ABSTRACT TRUNCATED AT 250 WORDS)

[Personal experience with immunosuppression with cyclosporin A in kidney transplantation].

Nonspecific immunosuppressants have nonselective impacts, attacking the global human immune system. By such impacts, it is considered that selective immune response on implanted graft will be reduced. That would be a general therapy without a specific target, with toxic side effects. The application of nonspecific immunosuppression in transplantology has been used within well known immunosuppressive protocols, such as Minnesota and Massachusetts General protocol and many others. All those protocols are considered to be nonflexible schemes which are used in order to prevent the global immune response in recipients. Those schemes do not take into consideration the specificity and sensitivity of the recipients' body in each specific case. It is not necessary to emphasize that in period of the exclusive use of nonspecific immunosuppressants the death rate was higher when caused by the side effects of those. It is still early according to our experience, for definite conclusions regarding the advantages of cyclosporin suppression above the classic one.

[Complications of immunosuppressive therapy after kidney transplantation].

The complications of immunosuppresive therapy that occurred in a series of renal transplant patients presented were many. Of the total 28 renal transplants performed in Sarajevo, 25 were observed. These complications were diagnosed in 19 cases (6 of which exited), Death in each case was due to virual and not bacterial infection which occurred most often in the lungs. No urinary infections occured in any of these cases. The actual cause of death in most of the patients was most frequently due to pneumocystis and alveo-capillary block of the lungs which is very difficult to diagnose. When this complication presented itself the method of choice was rapid manipulation of the immunosuppressive therapy together with the administration of concentrated granulocytes which did not significantly impair the function of the grafted kidney in any of the cases save those with cortizone diabetes. High body temperature was also of diagnostic importance. The authors feel that quick action and accumulated clinical experience prevent the fatal outcome of many renal transplant patients with complications from immunosuppressive therapy.
