Soft-tissue augmentation with Fascian.

For a century, under various implant circumstances, the propensity of pFGs to be replaced with host fibroblasts and collagen has been established. Harvested from screened human donors, Fascian is an injectable formulation of the same substance that is applicable for soft-tissue repair and augmentation. Provided in preloaded syringes, the material has many advantages over presently available substitutes. This freeze-dried material does not require refrigeration, and, because of low allergenicity, test implantation is not indicated. Because it is available in multiple particle sizes that can even pass through a 27-gauge needle, Fascian is suitable for fine defects, and because of the material's volume and density, lesions usually not considered for injectable augmentation may be treated with larger particles. The process of tissue particulation has not altered the inherent graft properties, and Fascian seems to evoke the same process of collagen replacement that routinely occurs in larger grafts.

Preserved particulate fascia lata for injection: a new alternative.

BACKGROUND: Preserved particulate fascia lata, derived from screened human cadavers, has recently become available. This injectable form of the material can be injected when soft tissue augmentation is desired. Historically, preserved fascia grafts have proven efficacy and an excellent safety record over the past 73 years. OBJECTIVE: To examine the clinical response to preserved fascia lata injections in human subjects. METHODS: Clinical subjects (N = 81; 74 women, 7 men, age range 19-56 years) requiring deep, soft tissue augmentation to repair various cosmetic deficiencies were injected with a total of 109 syringes of preserved particulate fascia lata. Three different preparations-<2.0 mm, <0.5 mm, and <0.25 mm particle sizes-were hydrated in 3-5 cc of 0.3% lidocaine solution and injected with needles ranging in size from 16 to 25 gauge. RESULTS: The patients were followed for 6-9 months after implantation without incidence of infections, allergic reactions, or acute rejection. After the treatment day, patients experienced no further discomfort. No dermal inflammation was evident and the local echymosis associated with injections was typically minor. Soft tissue augmentation was evident 3-4 months after grafting or longer in most cases. CONCLUSIONS: This patient series indicates that injectable preparations of preserved fascia lata have the same high biocompatibility as experienced with whole-tissue implants. The safety record of preserved fascia lata implants is reviewed.

The anterior-posterior otoplasty.

OBJECTIVE: To introduce a simple, modified method for correcting prominauris. DESIGN: Case series. SETTING: Private, facial plastic surgery practice. All operations were performed in an office surgical suite on the campus of a university medical center. PATIENTS: Fourteen patients (7 women and 7 men; age range, 22-46 years), constituting 28 ears, underwent otoplasty between February 1995 and November 1997. Follow-up ranged from 7 to 26 months (average, 11.5 months). INTERVENTION: Anterior-posterior otoplasty was performed. OUTCOME MEASURES: Subjective analysis by the author and patients, guided by photograph and chart review. RESULTS: Four ears had solely helical correction, while 22 ears had combined helical and conchal corrections. Within a few degrees, the desired posterior rotation occurred in all cases. Minor, persistent asymmetry, in auricle rotation or retrusion of either helical or conchal correction, was evident in 4 cases. CONCLUSION: Anterior-posterior otoplasty is a predictable, safe method for surgically retracting ears.

Miniflap hair restoration.

BACKGROUND: Flap reconstruction of the scalp has traditionally required multiple, delayed procedures, an extensive principle operation, and several minor revision surgeries to produce the optimal end result. The entire protocol had to be repeated for each subsequent flap. Flap reconstructions only covered a moderate portion of the bald scalp of the later-staged patient. This hair restoration process required considerable surgical skill on the part of the operator and tenacity on the part of the patient. OBJECTIVE: A system of short, nondelayed flaps was developed to shorten this comparatively long, multi-staged process and reduce the risk of flap compromise. METHODS: Initially, the patient underwent a two-step, scalp-lifting procedure to reduce the width of bald scalp to be spanned. At later sessions, anteriorly based flaps were outlined from the margin of the lifted scalp and rotated into the desired configuration. RESULTS: In 29 flaps rotated to date in 13 patients, flap viability was excellent and interflap scarring acceptable. Hair direction was readily adapted to the patients' desires. Case examples are presented. CONCLUSION: Staged scalp-lifting with miniflap reconstruction of the scalp is a preferred option for hair relocation in selected cases.

The comparison of facial grading systems.

To evaluate the relative merits of objective and subjective facial grading systems, we compared the results obtained with the Fisch, House, and Linear Measurement grading techniques in 12 subjects exhibiting varying degrees of palsy. Although most grading systems correlate well with each other, systems can vary dramatically in patient discrimination, interobserver variability, relative underrating and overrating, properties of nerve function selected, objectivity, and clinical definition. Objective and subjective data offer selective advantages.

Tip points: defining the tip.

At the center of the tip, the tip points (TPs) are the single most defining feature of a nose. In a random survey (N = 146; females, 76; males, 70), TPs were 8.9 +/- 1.6 mm apart. TP quality was graded into four categories based on the criteria of presence, interdomal crease, base width, and distinction. The distributions of TPs according to TPG were as follows: I, 12%; II, 50%; III, 30%; and IV, 8%. The TPG system was applied to a collection of photographs of female Caucasian magazine models, which had a distribution similar to that of the Caucasian females in the study population, demonstrating the effectiveness of grading. Surgical guidelines for the preservation and generation of TPs are presented, especially the geometric consequence of various surgical maneuvers, e.g., dome division and tip graft.
